WD)患儿的临床特点及实验室检测结果，提高对该病的认识和诊断能力。( 2)研究WD患儿的ATP 7B基因测序结果，探讨其基因型和临床表型之间的关系，为该病的诊治及预后提供遗传学依据。方法：( 1)回顾性分析7例WD患儿的临床资料，总结其临床特点。( 2)采用人类基因组DNA提取、聚合酶链反应( P C R)及DNA序列分析等分子生物学技术对上述WD患儿进行AT P 7 B突变基因检测。( 3)分析ATP 7B基因检测结果，与其性别、年龄、临床分型、血清铜蓝蛋白及尿铜等进行比较，分析基因型和临床表型的相关性。结果：( 1)本组7例患儿均出现不同程度的肝脏损伤症状，其中1例伴神经系统症状，1例伴溶血性贫血，1例以肾脏损害就诊。( 2)实验室检查结果显示，7例患儿血清铜蓝蛋白均明显减低，24小时尿铜增高，4例患儿K- F环阳性。( 3) ATP 7B基因检测结果：1例患儿未检测出突变，余6例患儿共检测出6个突变：Ar g778Le u ( R 7 78L)、Gl y711Tr p ( G711W)、As n1270Se r ( N1270S)、Al a 887Le ufs X14( A887Lf s X14)、930del I l e ( del 930I)、Va l 1 121Al a ( V1121A)。其中del 930I、V1121A为首次发现的突变，检出率分别为为28.6%、14.3%；R778L为我国WD患者的热点突变，本研究检出率为28.6%。山西医科大学硕士论文2结论：( 1) WD患儿以肝脏损害表现为主。( 2)血清铜蓝蛋白、24小时尿铜及K- F环是WD诊断重要的临床指标。( 3)本研究检测出的1 2号外显子del 930I突变及1 6号外显子V1121A突变是首次发现的突变。R 7
