海南航空股份有限公司宠物运输申请书（中文版正面）旅客姓名联系电话日期/ 航班号/始发地/目的地/重量（含宠物箱）宠物种类、品种、颜色猫 / 狗 品种： 颜色：宠物年龄宠物昵称提示航空公司注意事项 （此栏由旅客填写）温馨提示 宠物可能对航空运输过程中存在的高空压力、密闭空间等环境因素产生不适，从而产生情绪及生理变化，进而造成宠物受伤或死亡现象。因此，为了您的宠物安全考虑，我们请您慎重选择航空托运方式进行运输。 对于旅客托运的宠物在运输过程中发生的意外事故，如无证据证明海航有过错，则运输中造成的宠物伤亡，海南航空股份有限公司一律免责。且因宠物自身疾病、宠物咬破宠物箱、宠物箱打包不严、宠物排泄物溢出造成其他旅客同机托运的行李损毁、污染，甚至导致旅客索赔时，海航一律免责且保留追究宠物托运人责任的权利。特此提示！ 协议项目我已阅读并确认了解以下协议条款：1、本人所托运宠物须符合中国政府及其它相关国家小动物运输规定。2、本人所托运宠物须符合海南航空宠物运输的适用条件，本人也确认所托运宠物不在“不适合航空旅行的宠物”范畴。3、本人清楚知晓运输宠物所需要携带的证明文件，并将确保在运输之日前完成所有文件准备。4、本人将在办理值机手续之前准备好宠物箱，并在办理托运手续时接受航空公司工作人员对宠物和宠物箱的检查。 5、本人清楚知晓如宠物在怀孕期间、出生未满六个月、具有心脑血管、呼吸系统、消化系统、耳鼻喉科疾病，以及起飞前48小时内刚分娩或接受了手术的宠物是不适宜乘机的。在航空公司工作人员检查询问宠物健康状况时，如有隐瞒，本人将承担一切后果。6、本人对承运人（海南航空）所收取的宠物运输费用无异议。7、本人对协议中所规定的责任划分条款无异议。8、国际航班可能因承运人或非承运人原因发生备降，本人已了解由于国际航班航班备降涉及航权和备降机场地面保障限制的原因，宠物在运输过程中有出现意外情况的隐患。旅客声明我，即宠物托运人，已阅读本申请书中列明的条款内容，对以上条款无任何异议，并保证提供的相关信息完全、准确。我清楚知晓宠物托运可能存在的风险，愿意对托运宠物承担全部责任。 托运人（旅客）签字 年 月 日如果您托运的宠物品种、或宠物健康状况、或宠物托运箱、或宠物箱打包方式与海航要求不符合，请填写以下附加声明。附加声明我已检查并确认了解我的 与承运人要求不符，本次运输存在风险，由此被拒绝入境或者运输过程中出现宠物受伤、丢失、逃逸、患病、死亡或者宠物箱破损等情况，责任由我本人完全承担，请海南航空予以协助运输。 托运人（旅客）签字 年 月 日 售票单位/经办人始发站值机员签字到达站旅客接收签字到达站交付人员签字备注：此单一式四份，售票单位一份，始发站值机单位一份，旅客一份，到达站宠物交付工作人员一份。 海南航空股份有限公司宠物运输申请书（中文版背面）一、宠物运输申请限制和条件1. 宠物是指在重量限制范围内，可随主人同机托运的家庭驯养的狗、猫。2. 请提前24小时向海南航空售票单位进行申请。如未提前申请，可在乘机当日航班起飞前2小时，携带宠物、宠物托运箱、宠物运输申请书与相关证明，前往机场海航值机柜台办理宠物托运加急手续。3. 除导盲犬、助听犬及救助犬等工作犬外，宠物均不能作为客舱行李运输，必须办理托运或货运。4. 每位旅客最多可以托运两只宠物（以入境国规定要求为准，入境中国限带一只），两只宠物需要分开独立包装。5. 宠物作为托运行李运输时，宠物和笼子（含宠物箱内的食物和水）的总重量不得超过32Kg。6. 导盲犬、助听犬、救助犬等工作犬运输不受本条4及5款规定限制。7. 对于出现的以下情况，承运人无法办理宠物托运： 托运人所托运的活体动物为国家禁运的动物之列。 具有传染病征候,或疑似传染病载体的动物(如发生地区禽流感的家禽)。 出生不满六个月的宠物。 怀孕宠物或是在飞机起飞前48小时之内刚刚分娩过的动物。 性格焦躁、娇气、弱小，对高温高空环境敏感或不能长时间呆在宠物箱里的宠物。 服用镇静剂或安眠药的宠物。 浑身散发恶臭或让人难以忍受的刺鼻气味的宠物。 患有耳鼻喉科、心血管系统、脑血管系统、呼吸系统、消化系统疾病，以及48小时内进行过手术的宠物。 需要托运人完成的宠物飞行运输准备未完成。 所有属于不适合航空旅行的宠物种类（详见海航官方网站）。 因航班机型原因，不适于运输活体动物（如货舱无适运舱位）。 不符合始发、经停、目的地城市或国家动物运输的相关规定 旅客不认可承运人宠物运输条件或对宠物箱的要求、或拒绝填写宠物运输申请书、未准备好托运要求的相关文件。二、运输文件要求1. 乘坐国际航班，旅客应在至少出发前7天为宠物备妥下列文件 相关政府（目的地/中转地）核发的有效输出/输入文件。 有关当局核发的有效健康声明书及狂犬病疫苗注射证明书。 妥善的备齐所有行程中含盖的入境/过境国要求的入境许可、健康声明书及疫苗注射证明。 任何行程中含盖的入境/过境国政府要求的额外特殊文件2. 国内运输所须文件 动物卫生监督所出具的动物检疫合格证明，其中“运载工具消毒情况”一栏必须填写且单据上需盖有动物卫生监督所检疫专用章。 小动物疫苗注射证明。3. 导盲犬、助听犬、救助犬等工作犬运输所须文件：除须备齐本条1或2款证件外，还应携带有效的动物训练合格证明书、动物工作证或/和动物身份证。三、宠物箱要求1. 请注意，我们不会于机场提供宠物箱供乘客使用。携带宠物外游的乘客须自行准备航空专用宠物箱，并应确保宠物箱可以锁上、能防止宠物逃出、能防止泄漏及清洁卫生。2. 宠物箱必须由坚固材料制成且顶部固定，至少三面通风，箱门必须由金属制成。所有配件（包括螺母、螺丝以及门）确保安装完整。箱门牢固须有锁闭装置，有固定的把手或凸起边缘。3. 宠物箱通风口应为牢固安装在宠物箱上的金属格栅，如通风口非金属材质则须为圆型或其它形状透气孔。4. 宠物箱的空间应足够大，可以保证宠物自由站立或坐下、转身和以正常姿势躺卧。宠物箱的底部平稳，能够固定在平整的面上而不滑动。5. 带轮子的宠物箱，需要预先将轮子固定或将轮子拆除，保证在运输过程中宠物箱不滑动；宠物箱必须经过打包处理，中号箱顶部及底面每面至少横竖捆扎2匝，呈“井”字形；大号箱顶部及底面每面至少横竖捆扎3匝，打包带分布均匀；为避免打包时翻转宠物箱惊扰小动物，宠物箱侧面不进行横向打包，侧面的打包带均须呈竖状平行状态。6. 宠物箱箱内须铺上吸水性衬垫，例如毛巾、毯子、宠物尿布或者白色的纸，以防止宠物排泄物外溢污染其它行李。 海南航空股份有限公司 APPLICATION FOR PETS TRANSPORTION OF HAINAN AIRLINES （英文版正面）NameTelephoneDate/Flight No./Original /Destination/Weight (including the box)SpeciesCat / Dog Species： color：Age of PetName of PetNotice For Hainan Airlines （此栏由旅客填写）Warm NoticePets may feel discomfort caused by the environment factors in the air transportation, like altitude pressure and confinedspace, and then they may have emotion and physiology changes which may cause pets hurt or die. Therefore, we requestpassengers choose the air transportation for pets prudently. Hainan Airlines has liability exemption for all the accidents that pets die in the transportation if there is no evidence proving that Hainan Airlines has fault, when accidents happen to pets during the transportation. Hainan Airlines reserves the right to investigate the passengersliability for pets when other passengers baggage are damaged or polluted on the same plane, or other passengers claim for compensation due to the pets disease, breakage to the container or inadequate package.Prompt specially!Agreement ItemsThe s I have read and confirmed the items. 1. The shipper (passenger ) should know the necessary documentation for pet transportation and make sure all the papers prepared before the date of transportation.4. The shipper (passenger ) should prepared the pet container before check-in and accept the airlines inspection for the pet and container.5. The shipper (passenger) know that the pet is not suitable for flight, which is pregnant, born within 6 months, or has cardiovascular disease, respiratory disease, digestive disease, ENT(ear, nose and throat) disease, or has delivered a baby pet or gotten surgery within 48 hours. When the airlines examine the pets health condition, the shipper (passenger) need take full responsibility if concealing the fact.6. The shippers(passenger)has no objection to the charges for the checked pet of the carrier ( Hainan Airlines)。7. The shipper (passenger) has no objection to the responsibility provisions set out in the Agreement.8. International flight may alternates due to the reasons of carrier or others. Limitations and Conditions of the Transportation (1)”Pet” refers to only limit to the domesticated dog or cat within weight limitation, which can be transported with its owner on the same flight . (2)Please apply to the ticket agency of Hainan Airlines at least 24 hours before departure. If there is no application in advance, please check in the pet as expedited one at the check-in counter of Hainan Airlines with the container, and relevant documentation.(3) Except the service dogs, such as the seeing-eye dogs, hearing-ear dogs and the rescue dogs, all the other pets can not be transported as cabin baggage and must be checked in or shipped as cargo.(4) Each passenger can check in two pets at most, which should be packed respectively( Subject to regulations of the entry. Only one pet can be checked in when enter China.)(5) When pet is transported as checked baggage, the total weight of the pets and container (including the food and water in the container) cannot exceed 32Kgs.(6) The seeing-eye dogs, hearing-ear dogs and the rescue dogs are exempt from the limitations of the item 4 and 5.(7)The pet transportation may be rejected in case of the following situations.The pet is in the list of animals forbidden transportation in the country.The pet with communicable disease symptoms or the one is a suspected communicable carrier, such as the poultry with regional flu.The pet under 6 weeks.The pregnant pet or it has just given birth within 48 hours before the departure.The pet is irritable, emotional, vulnerable, sensitive to high altitude and high temperature, or can not stay in the container for a long time. The pet has taken sedative or hypnotics.The pet is fetid or has pungent smell. The pet has cardiovascular disease, respiratory disease, digestive disease, ENT(ear, nose and throat) disease, or has delivered a baby pet or gotten surgery within 48 hours.
