类似物如拉米夫定 阿德福韦酯等长期广泛应用于抗乙型肝炎病 毒感染 有效地抑制病毒复制 但随着应用时间延长 其较高的耐药率也受到了广泛的关注 替诺福韦酯 2008 年被批准用于慢性乙型肝炎 的抗病毒治疗 并显示了良好的疗效 多项研究证实 替诺福韦酯在拉米夫定耐药者及阿德福韦酯耐药者均发挥了良好的抗病毒作用 关键词 慢性乙型肝炎 替诺福韦酯 拉米夫定 阿德福韦酯 耐药 治疗 Research variety of nucleotide and nucleoside analogues such as lamivudine and adefovir are widely applied in the management of hepatitis B virus infection but the high resistant rate earns more attentions as the application time prolonged Tenofovir was approved for antiviral treatment for chronic hepatitis B in 2008 and showed good curative effect A number of studies have confirmed that tenofovir could Tranditional Chinese Medicine Yantai 264100 China doi 10 3969 j issn 1674 4985 2013 19 076 乙型肝炎病毒 HBV 感染是危害人类健康的公共卫 生问题 在亚洲地区 感染率达到 9 12 血清 HBV DNA 载量与肝硬化 肝癌的发病直接相关 1 目前拉米夫 定 LAM 阿德福韦酯 ADV 已在临床广泛应用且取得 了良好的收益 但相关的耐药问题也提出了新的考验 我国 慢性乙型肝炎防治指南 2010 年版 指出 2 拉米夫定使 用第 1 2 3 4 年病毒耐药率分别为 14 38 49 和 66 HBeAg 阳性患者使用 ADV 治疗 1 2 3 年耐药率分 别为 0 1 6 3 1 HBeAg 阴性患者使用 ADV 治疗 5 年 病毒学耐药发生率为 20 研究显示 ADV 耐药与 rtN236T rtA181V T 变异有关 3 替诺福韦酯 TDF 是第一个用于人类免疫缺陷病毒 HIV 感染治疗的核苷酸逆转录酶抑制剂 NRTI 2008 年 8 月在 期临床实验的基础上 被美国食品和药品管理局批 准用于 CHB 的治疗 4 替诺福韦酯 TDF 作为抗 HBV 的 一种新型药物 在抑制 HBV 方面表现了良好的疗效 现就 TDF 在作用机制 临床疗效 尤其在 LAM ADV 耐药者的疗 效 不良反应方面作一综述 1 作用机制 替诺福韦口服吸收差 通过添加两个酯基转换为替诺福 韦酯后 肠道吸收显著增加 在体外实验中其疗效提高约 50 倍 TDF 以剂量 300 mg d 口服生物利用度为 25 予高 脂饮食后可升高 5 TDF 吸收后脱去酯基转换成替诺福韦 以被动内吞的形式进入细胞 在磷酸激酶作用下激活为活性 二磷酸形式 与 5 三磷酸脱氧腺苷酸竞争结合 HBV DNA 逆转录酶 由于缺乏 3 OH DNA 链的延长被阻断 从而 抑制病毒复制 6 TDF 主要通过肾小球滤过和肾小管主动分 泌排泄 服药 72 h 后 70 80 的药物以原形随尿液排出 基底膜外侧的尿酸盐转运蛋白将药物运输至近曲小管细胞 由顶膜转运体 MRP 4 和 MRP 2 分泌至小管内腔 7 2 临床疗效 TDF 最初作为 HIV 感染者的抗病毒药物应用 在 HIV HBV 合并感染者观察到 TDF 对 HBV 也有强大的抑制 作用 2008 年获批准用于 CHB 的抗病毒治疗 2 1 TDF在LAM耐药者的疗效 Lada 等 8 的体外研究显示 对 LAM 耐药者 包括 YMDD 酪氨酸 蛋氨酸 天冬氨酸 天冬氨酸基序 变异者 TDF 仍有效地抑制病毒 Van B mmel 9 等报道了108名CHB患者 93人曾应用LAM治疗 其中 65 人存在 YMDD 变异 采用 TDF 治疗 随访数据显示 LAM 经治 既往曾应用 LAM 治疗 和 YMDD 变异均不影响 TDF 应答 Kuo 等 10 报道了 9 名 LAM 耐药者加用 TDF 治疗 48 周 HBV DNA 平均下降 4 5log10 copies ml 且 2 人发生 HBeAg 血清学转换 Manns 等 11 也证实了下述结论 70 名 患者 LAM 治疗 12 周以上 其中 10 人存在 YMDD 变异 和 571 名初治者以 2 1 随机分组 分别予 ADV 和 TDF 治疗 48 周后 TDF 组 LMV 经治者和初治者 ALT 正常率 78 74 组织学改善率 80 72 血清 HBV DNA 不可测 率 88 86 随后 Manns 等 12 又报道了 51 名初治者和 375 名 LAM 经治者 其中 10 存在 LAM 相关变异 予 TDF 治疗 96 周 分别有 84 和 92 达到 HBV DNA 不可测 400 copies ml 一项意向性治疗分析 13 初治组和 LAM 经治组 患者采用 TDF 治疗 96 周后 两组病毒不可测率无显著差异 在 LAM 治疗 5 年以上的患者中 TDF 仍能持续抑制病毒 14 上述研究均提示无论先前是否接受过 LAM 治疗 TDF 均可 157 中国医学创新 第 10 卷 第 19 期 总第 265 期 2013 年 7月 综 述 Zongshu Medical Innovation of China Vol 10 No 19 Jul 2013 发挥良好的抗病毒活性 2 2 TDF 在 ADV 耐药者的疗效 TDF 是一种无环核苷酸类 似物 替诺福韦的酯性前体 在结构上与 ADV 相似 Villet 等 15 分别在体内和体外对 ADV 耐药株进行 TDF 敏感性试 验 TDF 仍表现有良好的疗效 但较野生株略有降低 Van B mmel 等 16 报道了 20 例 LAM 耐药换用 ADV 治疗仍疗效 欠佳者 采用 TDF 平均治疗 15 个月后 仅 1 例病毒抑制欠 佳 考虑与肾功能不全 TDF 剂量减少有关 19 例均达到 HBV DNA 不可测 治疗 48 周后 4 例 HBeAg 血清学转换 1 例 HBsAg 转阴 Tan 等 17 报道了 9 名患者 其中 5 人在 ADV 治疗期间出现了病毒学突破但并未检测到 ADV 耐药突 变 4 人 ADV 治疗病毒学应答欠佳 采用 TDF 治疗 3 15 个 月后 7 人达到 HBV DNA 不可测 Berg 等 18 报道了 TDF 治疗起始存在 ADV 基因耐药 经 TDF 治疗后其中 7 人实 现 HBV DNA 不可测 Manns 等 19 报道了 160 名患者 其中 23 LAM 经治 ADV 治疗 53 周后 HBV DNA 仍可测 采用 TDF 治疗 24 周后 59 ALT 正常 77 HBV DNA 不可测 至 96 周后 又有 10 患者 HBeAg 血清学转换 Qi 等 20 报 道了 ADV 耐药突变如 rtN236T rtA194T rtA181V 并未引起 TDF 敏感性明显降低 上述研究显示 对 ADV 治疗应答欠佳者 TDF 仍有良好 的抗病毒活性 但也有研究认为 ADV 耐药者对 TDF 也出现 应答迟钝 Tan 等 21 报道了 16 名患者用 TDF 治疗期间已达 HBV DNA 已经不可测 换用 ADV 出现病毒学反弹 在不存 在 ADV 耐药的患者 换回 TDF 后 病毒抑制得以重建 然而 在 ADV 耐药者换回 TDF 后应答欠佳 这提示 ADV 与 TDF 可 能存在交叉耐药 同时检测到包括 ADV 耐药突变的 HBV 突变 体在 TDF 治疗期间仍持续存在 Van B mmel F 等 22 报道了 3 名 ADV 耐药者换用 TDF 后 虽显示了一定的病毒抑制 但均 未获得病毒学完全应答 亦未阻止进一步的 ADV 相关性突变 由于肾毒性是 ADV 高剂量的主要不良反应 所以 ADV 的推荐剂量 10 mg d 比 TDF 300 mg d 低 30 倍 多数学 者认为 23 两者治疗剂量的不同 是 ADV 应答不佳和 TDF 呈现持续应答的主要原因 Del Poggio 等 24 报道了低剂量 TDF 75 mg d 可能的疗效 11 例患者 其中 6 人病毒株为野生型 5 人存在 YMDD 病毒变异 采用 TDF 75 mg d 治疗 80 周 10 人 HBV DNA 不可测 然后 7 人转入 ADV 10 mg d 治疗 其中 3 人出现病毒学反弹 TDF 低剂量使用有着实际的意义 如 降低治疗成本和肾功能不全患者的用药安全等 3 不良反应 胃肠道症状是 TDF 最常见的副作用 发生率为 11 可出现恶心 呕吐 腹痛 腹泻 25 等 在长期应用 TDF 的 HIV 感染者 曾有骨密度减少和骨软化症的报道 但在 HBV 感染应用 TDF 治疗者 尚不明确骨密度检测的必要性 26 TDF 经肾排泄 对肾脏功能损伤的病例也频见报道 对 TDF 肾毒性的关注最初起源于 TDF 与阿德福韦 西多夫定结构 的相似性 这两种药物引起肾功能损伤 原因与抑制线粒体 DNA 聚合酶 而减少线粒体 DNA 复制有关 但在用 TDF 培 养的近端肾小管细胞中 只观察到较少的线粒体 DNA 减少 此外 早期 HIV 感染者中在应用 TDF 的随机试验并未显示 源于 TDF 的肾损伤 然而 病例报道 观察研究 动物模 型及细胞培养资料 都支持 TDF 存在近端肾小管损伤这一 观点 27 两份研究表明 在 17 22 TDF 治疗者存在近端 肾小管功能障碍 28 TDF 相关性肾损伤如导致肾小管功能 障碍 间质性肾炎 急性肾小管坏死 范尼可综合征等 29 其尿液特点包括 糖尿 低分子蛋白如 2 微球蛋白增加 蛋白尿和尿酸 尿磷异常 伴磷酸盐重吸收异常 此外 还 常见伴有血尿酸和血磷降低 血清碳酸氢盐减少 27 持续 肾损伤可加重肾小管细胞耗损 并最终导致肾小球滤过率降 低和肾衰竭 TDF 肾毒性的特点通常在停药后改善 早期停 药 肾毒性是可逆的 Herlitz 等随访研究 30 急性肾损伤患 者 TDF 停药 20 26 个月后 大约 50 患者肾功能完全 恢复至基线水平 其他病人肾功能部分恢复 sCr 平均峰值 从 5 6 3 8 mg dl 下降到 1 5 0 3 mg dl 一人需透析 4 个月 肾毒性的早期发现和 TDF 及时停药 是避免肾小 管不可逆性损伤的关键 因此建议 TDF 使用期间定期监测 并根据内生肌酐清除率情况来调整药物剂量 丙磺舒 阿昔 洛韦等药物与 TDF 转运相关的 MRP 4 MRP 2 等转运蛋白 相互作用 导致 TDF 内流增加 外流减少 引起细胞内积聚 增加了肾毒性 应避免同时使用 7 4 结论 包括 LAM ADV 在内的核苷 酸 类似物使乙肝病毒感 染者获益的同时 也存在着日益严峻的耐药问题 因此临床 用药迫切需要研制和推广生物利用度高 抗病毒效力强且诱导 变异率低的药物 TDF 作为一种新型的抗病毒药物 2008 年被 美国食品与药品管理局推荐为 CHB 治疗的一线药物之一 TDF 体内过程的研究建议可通过高脂饮食等方式提高其 生物利用度 TDF 体外实验表明可有效地抑制 LAM 耐药病 毒 临床应用也证实了一致的结论 TDF 在 LAM 耐药者体 现了良好的抗病毒活性 其效力与初治者无显著差异 可应 用于 LAM 经治者的补救治疗 而对于 ADV 耐药者临床应用 TDF 的结论尚不完全一致 体内与体外实验均表明 TDF 在 ADV 耐药株仍敏感性良好 对 ADV 应答欠佳者 有学者认 为 TDF 应答良好 但也有学者认为 TDF 也出现应答欠佳 普遍的观点是 对 ADV 耐药者 TDF 仍有良好的抗病毒活性 是 ADV 耐药者补救治疗的理想选择 而是否达到与 HBV 野 生株相似的抑制效果 仍存在争议 需临床进一步研究 TDF 的出现给 CHB 患者尤其是 LAM 耐药者及 ADV 耐 药者提供了更多的选择 大多数患者长期用药耐受性良好 虽然早期 HIV 感染者应用并未显示明显的肾毒性 但越来 越多的资料支持其对肾脏功能的影响 损伤部位集中在肾近 端小管 可导致急慢性肾功能衰竭 早期发现肾功能受损并 停用 TDF 肾功能多可完全或部分恢复 也有不可逆性肾损 伤的报道 因此建议用药期间定期监测肾功能 并避免合用 丙磺舒
