and soundness of machinery and equipment to be put into operation ex works certificate maintenance record testing record calibration certificate or annual inspection certificate 6 个人防护用品的配备及其符合性 作业进行过程中安全员落实情况 Assignment of safety persons during works 施工作业前 施工员对参加施工作业人员进行施工作业技术及要求交底 根据作业前对总的 施工步骤中存在的危险所采取的控制措施 安全员应负责进行施工作业 HSE 管理交底 办理相关的工 作许可证 Before starting the works construction controllers shall give detailed instructions of the work technical requirements to construction workers and according to the control measures to be adopted with respect to dangers existing in the general work steps before the works safety persons shall be responsible for instructions on HSE management for the works and obtaining relevant work permits 作业前 HSE 检查分为装置区外和装置区内检查 在开始作业前 由项目部组织 各级专业负 责人参加 如 CTCI 有要求也可以参加 按照相关检查内容进行检查 The HSE inspections before work are divided into inspections within the plant area and inspections out of the plant area They will be organized by the Project Management before starting the works and will be participated by responsible persons for various specialties and also by CTCI when requiring and inspections will be performed for relevant items 施工作业前应对作业环境进行检查 每日应进行针对性的班前讲话 做到 三交一清 即 交待作业内容 交待作业环境 交待作业中存在的危险因素和风险 清楚职工的身体和思想动态 并做好记录 Before the construction works the work environment shall be inspected Every day speech on relevant issues should be made before work to make clear the work to be performed the work environment dangerous factors and risks in the works and the physical and ideological conditions of employees making four aspects clear and all these will be properly recorded 对于在检查时发现的 HSE 隐患 项目部将给施工责任方下发 隐患整改通知单 要求施工 方尽快整改 施工方整改完并经自检合格后 填写 隐患整改情况反馈单 报项目部 由项目部再组 织并按上述检查程序进行检查和确认 For any HSE hidden peril found during inspections the Project Management shall issue the Rectification Notice to the relevant work unit requesting prompt rectification After completion of rectification and qualification by self inspection the work unit shall fill in the Feedback sheet on rectification and submit it to the Project Management which will then organize inspections and confirmation according to the above mentioned inspection procedures 1 2 施工作业中的 HSE 管理 HSE management during construction works 工程施工必须按照施工技术方案和相应施工工艺技术要求组织施工作业 Construction works must be organized according to the construction technical scheme and corresponding construction process requirements 施工中施工人员应严格按相关规定 制度和本工种的安全操作规程进行作业 During works construction personnel must strictly follow relevant rules and regulations and safety operation procedures for the jobs 施工中安全员应严格执行现场监督程序 严格执行隐患治理程序 3 During works safety persons shall strictly implement site supervision procedures and procedures to control and prevent hidden perils 施工中设备设施和使用的劳动保护用品应严格按相关规定配备和管理 During works the corresponding configuration and management rules shall be strictly followed for equipment and facilities and the use of labor protection articles 施工中施工人员应确保作业周围的人员 设施及物品处于安全状态 环境不被污染 During works construction personnel shall ensure that the personnel facilities and articles around the work area are in a safe status and the environment is not 施工作业后应清点检查作业所使用的施工机具 劳动保护用品 After the work the construction machinery and labor protection articles used on the worksite shall be counted 施工作业后应清点所有施工作业人员 After the work all working personnel shall be counted 施工作业后应检查本作业结果是否对下道工序存在隐患 有则改之 After the work inspection shall be made on whether there is any hidden peril from the current work to the subsequent step of work and any such problem shall be corrected 施工作业后应进行总结 为以后工作提供参照和经验 Summary shall be made after completion of works as reference and experience for subsequent work 本项目部施工作业应遵守以下 HSE 管理规章制度及其它相关制度 In the construction works by this 会议制度 HSE meeting system 1 4 1 1 项目部会议制度 Project Management meeting system 4 会议的分类与时间 项目部的 HSE 会议分四类 即 Classification and timing of meetings there are four types of HSE meetings for the Project Management 1 HSE 每周例会 与每周的最后一个工程协调会一并召开 HSE weekly routine meetings held together with the last project coordination meeting of every week 2 HSE 专题会 根据施工现场 HSE 的实施情况和存在问题 确定会议时间 但每月至少一次 HSE special topic meetings the meeting time will be determined according to the HSE implementation condition and existing problems on work site but at least once a month 3 全体人员每周一次 TBM 每周一上午开始工作前召开 TBM for all personnel once a week held before work on every Monday morning 4 全体人员每天一次安全讲话 每天上午开始工作前召开 Safety speech for all personnel every day before work every morning 在项目部组织的每周工程协调会上 有关 HSE 的内容作为会议的一项议程 主要内容有 由 项目部的 HSE 经理通报上周的 HSE 实施情况 检查结果 存在问题 布置 安排下阶段施工现场 HSE 的工作计划和实施的要求 提出持续改进的要求 传达上级部门 CTCI 的有关 HSE 管理的文 件 指示 通报等 At the weekly project coordination meetings organized by the Project Management the relevant HSE issues will be an agenda of the meeting and they mainly include the HSE manager of Project Management reporting the HSE implementation inspection results and existing problems during the previous week allocating and arranging HSE work plan and implementation requirements on worksite for the subsequent stage proposing requirements on continual improvement conveying relevant documents instructions and circulars on HSE management from the superior authorities and CTCI 在每周工程协调会上 对施工现场的 HSE 实施情况和存在的问题 要进行全面总结 对下一 步的工作要进行详细布置 At the weekly project coordination meetings overall summary will be made for the HSE implementation and existing problems on the worksite and detailed arrangement will be made for subsequent works 项目部全部或以二级施工单位为单位每周一上午 7 30 8 00 之间进行 TBM 项目部领导按要 求参加定点联系的二级施工单位的安全活动 由部门和施工队的专职或兼职安全员分别组织召开并做 好记录 也可以是全体人员集中在一个地点 由 HSE 经理组织召开 其主要内容是 TBM will be held at 7 30 8 00 every Monday morning for all personnel in the Project Management or by units of level II work units and Project Management leaders will participate in the safety activities of the designated contact level II work units as required This meeting will be organized by full time or part time safety persons of the department or work team and records will be made This meeting can also be organized by the HSE manager for all personnel at one location The main contents are 1 上周 HSE 实施情况简单介绍 Brief presentation of HSE implementation during the previous week 2 本周施工时的有关 HSE 要求和注意事项 Relevant HSE requirements and precautions for work during the present week 5 3 工作环境状况 安全防护措施及施工注意事项 Conditions of working environment safety protection measures and precautions in works 4 所使用的设备状况及操作注意事项 Conditions of equipment used and operation precautions 5 个人防护要求和应急准备 Requirements on individual protection and preparation for emergency 项目部 HSE 专题会的组织和内容 这种会议主要是由项目部安全部组织召开 正常情况下 是在每月的最后一周召开 其会议主要内容是 Organization and content of special topic HSE meetings of Project Management such meetings will mainly be organized by the Safety Department of Project Management and will be arranged in the last week of each month under normal conditions The main contents of the meeting are 1 对二级施工单位一个月的 HSE 实施情况进行评比 Assessing the HSE implementation of level II work units during the month 2 指出存在的不足之处并提出持续改进的要求 Identifying deficiencies and proposing requirements on continual improvement 3 布置下一阶段的工作 Arranging the work for the subsequent stage 但如果因 HSE 的实施过程中出现了问题且有可能继续发生的趋势 在这种情况下 项目部也将及 时组织召开专题会 及时解决存在的问题或将要出现的问题 其主要内容是 However if there is any problem in the implementation of HSE and there is the trend of continuous occurrence the Project Management may organize a special topic meeting to solve the existing or foreseen problem in a timely manner The main contents are 1 专题事项的详细情况及存在问题的介绍 Presentation of details of the matter covered by the special topic and existing problems 2 针对专题事项或存在问题的 制定纠正措施 Working out corrective actions for the matter or existing problems 3 为了预防问题的再发生 制定必要的预防措施 Working out necessary preventive actions to prevent recurrence of the problem 1 4 1 2 基层班组会议制度 Meeting system for grass root teams 班组除参加项目部周一安全活动外 班组长每天早上按 三交一清 的要求组织召开班前会 对每天的工作做出分工和安排 对重点部位 环节 工序及岗位进行教育作好交待 并做好班前讲话 记录 In addition to the safety activity arranged by the Project Management every Monday the team leaders shall organize a meeting before work every morning according to the requirements of making four aspects 6 clear to arrange the work for the day give proper instructions on key locations links work steps and posts and make proper record of speeches before work 项目部每天的安全讲话由各分管的安全员组织进行 主要是总结前一天的安全情况 根据风 险评价报告将当天作业中所涉及的危险向所分管单位进行交底 提出作业中的防护重点 The safety speech in the Project Management every day will be organized by the charging safety person mainly for summarizing the safety conditions of the previous day make instructions on possible dangers to be involved in the work of the day to the charged units according to the risk evaluation report and pointing out emphasis of protection in the work 1 4 2 HSE 检查 考核 奖惩管理制度 HSE inspection examination award and punishment regulations 1 4 2 1 HSE 检查管理制度 HSE inspection and management regulations 项目部检查以定期检查 专项检查 日常检查和节假日检查四种形式相结合 每月根据施工 重点与 HSE 动向开展专项检查 The Project Management will conduct inspections in the combination of four types periodical inspection special inspection daily inspection and inspection during holidays and conduct special topic inspections according to emphasis of works and HSE movements 日常检查由现场安全员完成 负责建立健全巡检日记 对隐患 问题 提出整改措施 对违 章提出处罚建议 Daily inspection shall be performed by site safety persons who will establish and keep patrol inspection logs propose correction and rectification measures on hidden perils problems and propose on punishment to violation of rules 定期 专项及节假日检查由 HSE 经理组织 安全员参加 定期每月一次 专项可根据工程特 点安排有针对性的检查 Periodical and special inspections and inspections during holidays shall be organized by HSE manager and participated by safety persons Periodical inspections shall be performed once a month and special inspections can be arranged for specific issued according to characteristics of the works 检查计划在每月安全部的工作计划中体现 每次检查最后分类汇总 形成检查记录 并下发 隐患整改单 隐患整改备忘录或安全工作联系笺到相关单位 The inspection program shall be indicated in the monthly working plan of Safety Department For each inspection summary will be made by categories to form an inspection record and Rectification Notice and Rectification Memorandum will be issued or safety work liaison sheets be sent to relevant units 现场安全员对受检单位的隐患整改情况进行检查监督 受检单位整改完后向安全部提出复查 申请 由安全部负责验收 Site safety persons shall check and supervise the correction and rectification of hidden perils in the inspected units and after completion of correction and rectification the inspected units shall submit a request for re inspection to the Safety Department who will be responsible for acceptance 1 4 2 2 HSE 考核管理制度 7 HSE examination and management regulations 根据项目部对二级施工单位和部门每月考核规定 组成安全考核小组对各单位考核 A safety examination group will be organized to examine various units according to the regulation of Project Management on monthly examination of level II work units and departments 将各项 HSE 检查的结果与考核相结合 即定期考核与日常检查考核结合 定期考核每月进行 两次 分别为中旬与月底考核 The results of various HSE inspections will be combined with examination i e periodical examination will be combined with daily inspections and examinations Periodical examination will be performed twice a month respectively in the middle and at the end of the month 对每月考核结果进行汇总 评比分并与奖金挂钩 安全风险抵押金的返回比例及年终评优考 核应参照月考核结果 The examination results of each month will be summarized scored and be linked with bonus The proportion of return of safety and risk mortgage money and year end excellence examination will also make reference to the monthly examination results 1 4 2 3 HSE 奖惩管理制度 HSE award and punishment management system 项目部内部奖惩执行公司 安全管理规定 Within the Project Management the Regulation on safety management of the company will be implemented 所有违章者被 CTCI 发现而通报的 项目部将按规定对该单位进行双倍的处罚 CTCI 发现 而处罚 所有处罚额全部由该违章单位承担 此外项目部对该单位进行双倍的追加处罚 For any violation of rule found and reported by CTCI the Project Management will double the punishment of the unit concerned according to rules When violation is found and punished by CTCI all penalty month will be borne by the violating unit and in addition the Project Management will give additional double punishment on such a unit 对违章严重的二级施工单位 安全部将要求该单位及个人进行停工处理 直到整改合格为止 情节严重者 安全部将勒令其退出项目部 但以上措施不妨碍且也不替代任何二级施工单位或个人因 违犯施工管理程序或违章作业而导致的财产损失及人员伤害等重大 HSE 事故所受到的法律责任或处罚 For any level II work unit having seriously violated rules the Safety Department shall require that unit and individuals to stop work for disposition until qualification in correction and rectification Those with serious cases shall be ordered by the Safety Department to leave the Project Management However these actions will not hinder nor substitute any legal liability or penalty against any level II work unit or individual on property damage and personal injury or other major HSE accidents resulted from violation of construction management procedure or from working in violation of rules 1 4 3 HSE 事故报告和信息交流制度 HSE accident report and information exchange system 发生事故应按 CTCI 要求向有关单位报告事故 8 Any accident shall be reported to relevant organizations as required by CTCI 信息主要来自公司 CTCI 和兄弟单位及现场信息综合 Information shall mainly be sources from the company CTCI and peer organizations and site information summary 1 4 4 环境保护 Environment protection 1 4 4 1 环境影响评价 Evaluation on environmental impact 项目开工前项目部应组织相关人员对施工现场和生活区进行可行性评估 做出评估报告 Before starting work for the project Project Management shall organize relevant personnel to conduct a feasibility evaluation of the construction site and living quarter and prepare an evaluation report 在施工过程项目部相关职能部门中对施工作业环境进行相关内容的检测 以便对下一步改善 作业环境提供基本的数据 During the work the relevant functional departments of Project Management shall make testing on relevant items on the work environment so as to provide basic data for further improving the work environment 在发生环境污染事故时 应按应急程序进行对环境的评价 In case of any accident with pollution to the environment environmental evaluation shall be conducted according to emergency procedures 1 4 4 2 环境保护措施 Environment protection measures 职工要注意个人卫生和饮食卫生 预防疾病发生 Employees shall take care of individual sanitation and food sanitation to prevent disease 施工现场道路在指定专人清扫的同时 还应定期洒水 防止尘土飞扬 Roads on work site will be sprayed with water periodically to prevent dust from rising in addition to cleaning by designated personnel 水泥 白灰等易飞扬的细颗粒材料 应在库内存放 室外露天存放时必须下垫上盖防止扬尘 Materials with fine particles likely to rise such as cement and white ash shall be stored indoor or covered when stored outdoor to prevent dust rising 下雨时 在工地大门处铺垫草袋 在雨后泥泞的情况下及时清理车辆轮胎上上的泥土 During rain straw bags shall be laid at the site gate and mud on vehicle wheels will be cleaned promptly when it is muddy after rain 禁止在施工现场焚烧杂草和各种垃圾 It is forbidden to burn weeds and rubbish at worksite 9 现场喷砂除锈施工时应采取相应措施减少粉砂和铁锈的飞扬 During rust removal by blasting in field relevant provisions shall be made to reduce flying dust and rust 施工现场对 CTCI 的绿化要爱护 花草 树木确因施工需要迁移的 要与 CTCI 协商并采 取措施后方可施工 Care shall be taken for the plantation of CTCI on site and when it is really necessary to relocate flowers and trees due to construction consultation shall be made with CTCI and measures be taken before proceeding 为防止水质污染 施工中禁止将施工用的酸洗液 钝化液未经中和处理就直接排放 To prevent pollution to water it is forbidden to directly discharge pickling liquid and passivation liquid without neutralization during works 施工中禁止将有毒有害废弃物作土方回填 It is forbidden to use toxic or harmful wastes as earth for backfilling 理化用的化学药品应集中库内存放 要加盖保管 防止洒落污染水质 Chemicals for physical and chemical tests shall be stored in warehouses in a concentrated manner and be stored with covers to prevent spreading to pollute water 施工用现场砼搅拌站的废水须经专门的沉淀后再排放 现场汽车修理 动设备拆解用的废油 应集中收集 统一处理 严禁乱排放 职工食堂污水排放时应设置隔油池定期淘油和清除杂物 防止 直接排放污染水质 Wastewater from concrete batching plant on worksite must be allowed to settle before discharge waste oil for vehicle repair and dismantling of equipment on site will be collected and treated on a unified basis and it is strictly forbidden to discharge it without permission sewage water from canteen will be drained with an oil separation tank and oil and foreign matter be removed periodically to prevent direct discharge that will pollute water 职工宿舍 厕所应设化粪池 工地临时厕所也应设化粪池处理后排入污水总管 Septic tanks shall be provided for staff dormitory and toilets and temporary toilets on site shall also be provided with septic tanks and sewage will be discharged into the mains after treatment 建筑垃圾应运放到指定的堆放点 Construction rubbish shall
