Nov. 2013 编号：Number:风险评估值Risk level序号S/N工作步骤Job Step可能的危害或损失Potential Danger &Loss高High中Medium低Low对危害的控制方法The Controlling Means of Risk备注Remarks1登高作业人员没有经过高处作业安全知识培训Climbing operator does not receive the safety training in work at height.缺乏安全知识，违章作业，发生安全事故。In lack of safety knowledge, work against job regulation, and result in safety misadventure.充分利用周例会、工具箱会议及其它宣传教育机会，加强高处作业安全知识宣传教育To fully utilize Weekly meeting, TBM, and other publicity and education opportunity to improve safety education on work at height.2高处作业人员身体健康状况差，不适合登高作业Climbing operator is in poor health, and not suitable for work at height.作业时注意力不集中或中暑等发生高处坠落事故High falling occurs as attention is not focused or heatstroke is suffered during work. 加强宣传教育，咨询作业人员，身体状况差的人不允许登高作业。To reinforce safety publicity and education, consult with operator, and not allow person in poor physical condition to work at height. 3登高作业使用的梯子不牢靠或地面不平，无人扶持The ladder for climbing is not firm or not on flat ground, or without support by the hand. 登高时梯子断裂或倒落，人员坠落支撑平台被不慎碰到，导致高空作业人员坠落。During climbing, the ladder is broken or fallen down, operator falls, or the supporting platform is hit down, which result in the fall of high climber. 使用合格爬梯，作业前对梯子、地面状况进行检查，确保安全。高空作业时，警戒标识，确保有人看守。To use qualified ladder, inspect the condition of ladder and ground to ensure safety. Work at height shall be with warning mark and under close guard.4屋顶作业人员没挂安全带The operator on rooftop does not wear safety belt. 脚下打滑，不慎坠落。屋面板安装不劳，面板滑动导致施工人员坠落。高空施工人员，受到惊吓不慎坠落。遇到大风，特别是刮起的灰尘给工人高处行走带来不便甚至容易造成坠落。Operators fall down accidently by:- The rooftop is slippery;- The roof panel is not firmly installed, and the panel shaky;- The high-altitude operators are shocked and disturbed;- The strong wind, especially the raised dust, brings operator with inconvenience. 施工前，召开工具箱会议，对安全带，安全绳进行检查，确保劳保用品完好后方可进行施工。在屋面上方正确设置安全绳，确保屋面作业人员安全带有处可挂。与高处施工人员说话时，要确保其安全站稳后方可与其说话，严禁在高空作业人员违章作业时对其大声斥责。遇到大风天气，停止高空施工。To hold tool box meeting, check the safety belt and safety rope to ensure PPE is in proper condition prior to construction work.To use safety rope correctly on rooftop with operator hung up by safety belt.5屋顶人员随意往下丢东西Operators on rooftop throw down materials at random.砸到地面人员The personnel on the ground are hit by falling objects.禁止随意往下丢东西Do not throw down materials at random.6电机线路破皮 ，接头处没有包扎好，或者施工过程中被破坏。The motor circuit is worn, and the joint not properly wrapped, or damaged during construction. 彩板房材料为金属外壳，且金属片很锋利，如导电，施工人员容易受触电伤害。The color-coated steel house is made of metal in shell, and metal is very sharp, conductive, which let operator liable to electric shock.施工前检查用电线路，确保线路整洁，并确保人多处作好警示标识。To check the electric circuit and keep it clean and tidy before construction; warning mark shall be located at people concentrated area.7雷击Lightning stroke 遇到雷雨天气，施工人员屋面上工作时受雷击伤害The operators are struck by lightning during thunderstorm. 遇到雷雨天气，停止施工。Construction activity shall be stopped in the thunderstorm. 8夜间施工Night construction 夜间施工，由于光线不明导致失足滑落。The operator loses his footing and fall down as the light is dim during night construction. 夜间施工前，召开班前会议，检查照明系统是否正常，填写夜间施工申请，并与相关负责人检查无误许可后，方可施工。To hold meeting before night construction to inspection whether lighting system is working normal; to submit application for night construction and start work only after inspected and approved by relevant person in charge. 施工单位负责人：杨春林，陈本隧The head of Construction Unit: Yang Chunlin, Chen
