CCDC Dance Card持有人由28/5/2005(星期六)起開始優先接受報名；公開報名由30/5/2005(星期一)起至額滿即止。2. 學員均可穿著舞衣/便服/運動服，輕便波鞋或舞蹈鞋上課。不宜穿牛仔褲。3. 可親身到本中心報名 或4. 郵寄：填妥報名表，連同劃線支票，請以城市當代舞蹈團有限公司抬頭支票背面寫上學員姓名及聯絡電話，寄回九龍黃大仙沙田坳道110號地下CCDC舞蹈中心。請勿郵寄現金，若因郵遞錯誤，本中心恕不負責。5. 傳真：填妥報名表內之信用卡資料 (只接受VISA / MASTER)，傳真至2242 2363或2326 0220。(請勿多次傳送同一份報名表，信用卡付款只適用繳付學費折扣後滿HK$600或以上之課程)6. 本中心有權取消報名人數未乎理想之課程，或於必要時更改導師及上課時間。7. 請仔細選擇合適課程、上課日期及時間，除因課程取消或額滿外，一切已繳費用概不發還。如在特殊情況下須轉班，必須書面申請及每項課程須繳交港幣$100元的行政費用。本中心保留學員轉班之權利。8. 遇有暴雨、颱風或天氣惡劣，本中心主要參考教育署為小學之指引，而決定課程取消與否。請家長留意電台、電視廣播或致電本中心2328 9205查詢。如因惡劣天氣而取消之課程，本中心恕不另行通知，亦不會安排補課或退回費用。9. 學期完結時學員上課出席率達80%或以上者，均可向本中心申請出席証明，而每項課程均須繳付港幣$10元的行政費用。如須申請過去季度或須於由申請日起一星期內領取之出席証明，每項課程均須繳付港幣$20元的行政費用。1. Enrolment of CCDC Dance Card Holders will only be accepted on or after 28 May 2005 (Sat); Enrolment of non-CCDC Dance Card Holders will only be accepted on or after 30 May 2005 (Mon).2. Students are advised to wear leotard, casual wear or sports wear and sports shoes or dancing shoes. Jeans are not advisable. 3. Please fill in and return the enrolment form to CCDC in person or4. By mail: Please fill in the enrolment form with crossed cheque payable to “CITY CONTEMPORARY DANCE CO., LTD” (please write down the name and contact number on the back of the cheque) or with Credit Card information and send it to the CCDC Dance Centre G/F., 110 Shatin Pass Road, Wong Tai Sin, Kowloon. Please do not send in cash. CCDC will not be responsible for any mail undelivered.5. By Fax: Please fill in the enrolment form with Credit Card (only VISA / MASTER) information and fax to 2242 2363 or 2326 0220. (Please do not send duplicated forms. CCDC reserves the right: 1) to cancel courses if the no. of application is below the requisite minimum; 2) to change the time and instructors of the courses should it be necessary. 7. Please choose the most appropriate course, time and date carefully. No refund will be made except in the event of the course being cancelled or full. If students wish to transfer their course to another course under special circumstances, they must fill in the application form and pay HKD$100 administration charge.
