健康安全及环境计划Health,SafetyAndEnvironmentPlanA.1施工现场管理制度constructionsitemanagementsystemA.1.1安全防护管理制度securitymanagementsystem各区域必须有安全管理技术措施，分项施工前必须作好分级、分段逐步安全技术交底。无措施、无交底、作业班组有权拒绝施工。Everyregionmusthavesecuritymanagementmeasures,,stages,andtechnicalrequirementsmustbeintroducedindetail.进入施工现场，必须戴安全帽。严禁穿拖鞋、硬底易滑鞋等。Allpersonnelenteringintotheconstructionsitemustwearasafetyhelmet.Forbiddentowearslippersandotherhard-soled.高空作业要按规定栓好安全带、支搭安全网，架子必须按规定搭设、经验收合格后方可使用，搭设时严禁高空投扔。Safetyharness,safetynetsmustbeneededwhenworkersoperatingoverhead.Scaffordmustbesetupandbequalified.各种机械设备必须有安全防护装置，不得带病运行。Eachmachinemustbeprovidedsafetyprotectiveequipment,andmustrunsmoothly.施工现场临时用电必须符合临电施工组织设计。实行三级配电二级保护，机电设备必须重复接地保护，临电架设要符合要求。Temporaryelectricalpowermustmeettemporaryelectricalpowersupplyplan.Mechanicalandelectricalequipmentmusthavedoubleearthingprotection.电焊设备、压力容器必须有安全装置，并且保持其灵敏可靠。Electricalweldingequipments,pressurevesselsmustbeprovidedsafetyprotectiveequipmentsandbekeptsensitiveandreliable.特殊工种必须经过培训，合格发证后才能独立操作。Constructionspecialoperatormustbetrainedandqualifiedbeforeoperating安全隐患要及时整改，脚手架、防护设施、安全标志以及警示牌未经许可，不准擅自拆除。Scaffolding,protectiveequipment,safetysignsandwarningsignscan’tremovewithoutpermission.重要劳动保护用品必须使用指定厂家和指定产品，不得自行采购。Importantlaborprotectiveitemshouldbespecifiedmanufacturedandspecifiedproduction.每周进行安全生产检查，做到安全与生产“五同时”。Implementsafetyinspectionweekly.A.1.2安全检查制度safetyinspectionsystem项目安全部每周组织一至两次安全生产全面检查，每天各区域主管对本区域进行不间断巡检。Organizecomprehensivesafetyinspectiononceortwiceaweek.根据工作需要项目安全部或区域主管组织不定期的安全生产大检查，如巡回检查、专项检查等。AccordingtotheMinistryofwork,theregionaldirectororganizesafetyinspectionsirregularly,suchasmobileinspection,specialinspection.安全检查的主要内容：⑴查区域领导，是否认真贯彻了“安全第一、预防为主”的方针，正确处理了安全和施工生产进度的关系等。⑵查班前教育，在时间、内容、人员上是否落实。⑶查防护，各种现场防护是否达到了标准要求。⑷查制度，各项管理制度是否健全，是否得以真正落实。⑸查隐患，各区域是否存在隐患和“三违现象”以及“三定”工作是否落实。⑹查整改，上级部门或项目安全部检查中所发现的安全隐患是否已经整改完毕。themaincontentofsafetyinspection:⑴checktheregionalleader,ifheisimplementthe"safetyfirst,preventionfirst"principle,handlethesafetyandconstructionrelationsproperly.⑵checkPre-worksafetyactiveties:⑶checkprotection,ifavarietyofprotectionon-siteisuptothestandardrequirements.⑷checksystem,ifthemanagementsystemissound,whetheritbeimplementedtruly.⑸checkhiddenhazards,theexistenceofhiddenhazards.⑹checktherectification,theparentdepartmentorprojectintheSecuritycheckswhetherthesecurityrisksfoundintherectificationiscompleted.每次检查要认真考核、评分和评比，好的经验要总结推广，给予适当的奖励，差的要严肃批评、给予处罚，使得安全检查真正形成为经常化、制度化。Assesseachinspection,sumupandpromotegoodexperience,giveappropriateincentives,thepoormustbecriticizedseverelyandpunished.A.1.3安全生产教育管理制度safetyinductionmanagementsystem为了对员工劳动保护，安全生产基本知识的教育和安全技术的培训，不断提高员工的安全意识、法制水平，使他们能自觉遵守企业安全生产的规章制度和安全技术操作规程，减少和消除不安全行为，保证项目实现安全生产。根据上级有关规定及我工地的情况特制定以下规定：Increasingemployee’ssafetyawareness,legallevel,sothattheywillcomplywithcompanysafetyregulationsandsafetytechnicaloperation,toreduceandeliminateunsafebehavior,ensuresafeproduction.Accordingtorelevantregulations,formulatedthefollowingrequirements:认真执行“安全第一，预防为主”的安全生产方针，加强员工的安全生产教育和安全技术培训，使之做到经常化，制度化。Implementthe"safetyfirst,preventionfirst"safetyprinciple,enhancestaffsafetyproductioninductionandtechnicaltraining.对新进场的员工，由其他施工现场转入本现场的员工，在本现场工作的因工作需要转换工种的工人（包括联合力量的管理人员及工人），都必须进行三级安全教育。并经考核合格后才能进入工地上岗操作。Allemployeesmustattendgradethreesafetyinductionandbequalifiedbeforeenteringintothisconstrucionsite.教育内容包括：劳动保护，安全生产方针、政策、法规和安全技术规程，以及季节性施工的方案、措施。Educationinclude:laborprotection,safetyguidelines,policies,regulationsandsafetytechnicalregulations,andseasonalconstructionprograms,measures.培训时间：公司安全教育为16小时，项目经理部安全教育为16小时，班组安全教育为8小时。Trainingtime:thecompanysafetyeducationfor16hours,project-specificsafetyinductionfor16hours,construcionteamsafetyinductionfor8hours.项目安全部对受教育人员考试成绩进行登记造册建立档案。TestresultsshouldbeInventoriedtocreatethefile凡是从事特种作业的员工，必须经由天津市建委指定的培训机构进行培训，取得合格的操作证后方可上岗操作。WorkersdirectlyengagedinspecialoperationsshouldbetrainedandqualifiedbytraininginstitutiondesignatedbyTianjinConstructionCommittee项目各级管理人员（包括分包的管理人员）必须积极参加由公司、项目部组织的各种安全培训教育，提高自身的安全管理工作能力及素质projectmanagersatalllevelsmustparticipateintheproductionsafetyinductionandtrainingactively,improvetheirsafetymanagementcapacityandquality.A.1.4安全值班制度securitywatchsystem项目经理部的行政、安全、生产、技术负责人，均要轮流值班。Theadministrative,safety,production,technicalleaderoftheprojectmanagerdepartmenthavetobeshifts.每日安排2人值班，值班表及联系电话要上报指挥部，遇有特殊任务或日夜多班作业时，要增加值班人员。Arrange2personsonduty,dutytableandtelephonenumbermustbereportedtoheadquarters.Incaseofspecialtasks,increasetheduty.值班人员必须认真履行职责：参加班前安全教育，及时进行现场安全检查，将了解到的情况向领导或负责人汇报，并提出整改意见，负责处理日常安全生产事务和发生事故的现场处理工作。Duties:participateinPre-worksafetyinduction,andimplementsafetyinspectionson-sitetimely,torportthesituationtotheleadership,andsupervisethecorrection.认真填写安全值班记录，搞好交接班，移交时，必须填写本班已经做到的工作和已经解决的问题以及下一班应该注意的事项和需要继续解决的问题，并要明确列项交代清楚。Fillinthesecurityrecord,doagoodjobshift.Whentransfering,hemustwritedowntheworkthathasbeendoneandtheproblemsthathavebeenresolved.值班人员在必要时，有权暂停冒险作业人员的工作，有权决定制止“三违”作业现象和对其处以一定数额的罚款。Ifnecessary,thepersonondutyhavetherighttostoptheunsafetyworkandpunishtheoperator.A.1.5班前安全活动制度Pre-worksafetyactivitiessystem为了加强安全管理，使每一个工人提高个人防护意识，让每个工人在工作中时刻牢记安全，特制定以下规定：Inordertostrengthensafetymanagement,sothatimprovepersonalprotectionawareness,leteachworkerkeepinmindsafetyatwork,formulatedthefollowingrequirements:各区域主管必须要求施工的各种施工班组必须坚持周一安全活动。活动时间不少于1小时。TheregionaldirectormustrequiretheconstructionteamtodosecurityactivitiesonMonday.Activitiesfornotlessthan1hour.活动内容：针对当天的施工情况，重点应注意的安全事项，向施工人员进行安全教育，学习安全生产规章制度、安全操作技术规程、宣讲安全技术交底内容、对作业环境进行检查、对个人劳动重要防护用品进行检查。Activities:Fortheconstructionoftheday,introducetheconstructionnotesandtechnicalrequirements.Inspectconstructionenvironmentandcheckindividuallaboritems.各工种、各施工班组每日班前要对自己所管施工工序的安全注意事项向每一个工人进行安全技术交底。交底时间不少于15分钟。Introducetheconstructionnotesandtechnicalrequirements,notlessthan15minites.班组要认真地填写班前讲话活动的内容、时间、人数等记录，对记录要妥善保管不得丢失。Writedownthecontent,timeandnumberandkeeptherecordswell.A.1.5.1文明施工管理制度Civilizationconstructionmanagementsystem（1） 教育制度：深入广泛开展文明施工管理、创建文明工地达标活动的教育，提高全员文明施工积极性、主动性，为创建安全文明工地提高思想认识，使职工养成保护建筑物品、爱护建筑物品、人人遵守施工秩序的良好习惯。theeducationsystem:Implementcivilizationconstructionmanagement,createasafeandcivilizedconstructionsite,sothattheemployeesgetgoodhabitsofprotectingbuildings.（2） 责任区制度：把现场划分为若干责任区进行管理，明确责任单位、总包和劳务队伍责任人，并挂牌明示。responsibilitysystem:thesiteisdividedintoseveralareas,clearresponsible.（3） 定期检查制：定期每周一次、不定期（阶段性抽查）由分公司副经理组织相关部门参加文明施工检查，并评定、汇总、建档，查出的问题立项、整改，落实责任人、整改期限。periodicinspectionsofthesystem:takepartincivilizationconstructioninspect,assess,aggregatetheresults.Identifyproblemsandcorrectethem.（4） 奖罚制度：对文明施工做出贡献的人员、单位给予奖励；对违反文明施工规定，给项目造成损失或损害公司声誉的人员、单位给予处罚；奖励、处罚由项目额定、实施。rewardandpunishmentsystem:Awardtheemployeeswhocontributetocivilizationconstruction,punishtheemployeeswhodamagethereputationofthecompany.A.1.6环境保护管理制度EnvironmentalProtectionmanagementsystem施工现场要做到围挡严密，路面要硬化、洒水，项目部派专人加强对扬尘的管理。Constructionsiteshouldbeenclosedwithfencing.Mainroadofsiteshallbetreatedwithhardeningsurface.Measuresfordusteuppressionshallbeimplemented.运输散料车辆要严密遮盖，防止遗撒、扬尘。卸料时应采取有效措施，减少扬尘，车辆不准夹带泥沙出现场。Enclosedorcoveredvehiclesshallbeemployedfortransportationofexcavatedsoil.Measuresforcleaningthevehiclesinandoutshallbeimplementedattheaccessofsite.施工现场内的水泥和其它飞扬的细颗散体材料应安排在库内存放或严密遮盖。Cementandotherbuildingmaterialsoffine-particlenatureeasytocauseflyashshallbesealedofforcoveredupinstorage.各区域清理施工垃圾时，必须搭设封闭式临时专用垃圾道或采用容器吊运。严禁随意凌空抛、洒。Constructiongarbageinsidebuildingshallbedisposedofinrelatedcontainerorductfortransmission;droppingdebrisoverheadshallbeforbidden.加强控制大气污染、施工现场的厨房全部采用清洁燃料。Strengthenthecontrolofairpollution,thekitchenoftheconstructionsiteusecleanfuel.施工现场的临时食堂，必须设置简易有效的隔油池、定期淘油，防止污染。生活垃圾要集中堆放，及时清理出现场。Sewagepipingconnectedwithcanteen,washingroomandshowerroomshallbeprovidedwithfilterscreentpensurefreeflow.贯彻食品卫生法、加强施工现场卫生防疫工作、杜绝食物中毒及事故发生。Carryoutfoodhygiene,strengthenhealthanddiseasepreventionworkoftheconstructionsite,preventfoodpoisoningaccidents.各区域的强噪声设备，必须封闭使用，并合理安排作业时间,减轻噪声扰民。Machineproducinghigh-pitchednoiseatsiteshallbesealedtoreducethenoise.各区域主管对人为的施工噪声应有降噪措施和管理制度，并严格控制，最大限度地减少噪声扰民事件发生。Theregionaldirectorshalltakemeasurestoreducethenoise.A.1.7机械设备管理制度machineryandequipmentmanagementsystem各区域主管负责人要加强机械设备综合管理，要求分包队设置懂机械设备管理工作的专职（或兼职）管理员，充分发挥机械设备的效能。Theregionaldirectorisinchargeofstrengtheningtheintegratedmanagementofmachineryandequipment,makethemachineryandequipmentmoreefficient.各区域主管要制定机械设备使用的布置方案（符合项目平面布置图要求），并根据施工进度要求合理选配及时进场，满足施工需要。Theregionaldirectortopreparemachineryandequipmentlayoutprogram,andmakethemachineryintothesitewhennecessary.施工项目机械设备必须实行统一管理（包括自有、租赁、分包单位自带的机械设备），由区域管理员按规定要求对进场的机械设备进行验收，合格后向有关人员和操作人员进行安全技术交底，手续要齐全，记录要认真，而后方可使用。constructionmachineryandequipmentmustbeunderunifiedmanagement.Theregionaladministratorisresponsiblefortheapproachacceptanceofthemechanicalequipmentrequired,afterqualified,introduceoperatingmethodtotheoperator.机械设备的操作人员按规定持有效证件上岗操作，在用的机械设备安全防护装置必须齐全有效。Machineryoperatorsrequiredvalidcertificates,machineryandequipmentinusemusthaveeffectivesecuritydevice.要合理使用机械设备，安装要符合规定，特种机械设备要制定专项的安全技术要求，机械设备使用环境要符合文明施工管理要求。Therationaluseofmachineryandequipment,theinstallationofmachineryshouldcomplywiththerequirements.贯彻“安全第一，预防为主”的方针，如若发生机械事故，应保护好现场及时上报有关领导和部门，同时采取一切可行的措施，防止事故进一步的扩大，在事故处理上要坚持“四不放过”的原则，加以整改，杜绝事故重复发生。implementethe"safetyfirst,preventionfirst"principle,ifamechanicalaccidenthappen,shouldreportpromptlytotheleadersandauthorities.Atthesametimetakeallfeasiblemeasurestopreventfurtherexpansionoftheaccident.坚持机械设备使用“十字”作业法，加强维修保养工作，严禁机械设备带病运转作业。Strengthenthemaintenancetothemachine,mechanicalequipmentisprohibitedinbadcondition.各区域要积极组织有关人员，参加项目组织的关于安全使用机械设备的教育和技术培训，使得操作者懂技、会操做、能维修。Organizerelevantpersonneltoparticipateinproductionsafetyinductionandtraining,enabletheoperatortounderstandthetechnology,canrepairthemachine.各区域主管对机械设备使用要进行经济合算，作到资料完整齐全。Keeptheinformationofthemachineswell.A.1.8临时用电安全管理制度Provisionalpowersupplysafetymanagementsystem现场临时供用电设施统一由项目经理部机电部管理。ProvisionalpowersupplyequipmentsaremanagedbyElectricalprojectDepartment.项目经理部设置一名安全用电技术负责人。ProjectDepartmentappointapersoninchargeofElectrical.编制“临时用电施工组织设计”并经公司工程管理部审批。临电工的工作要在施工组织设计的指导下进行。TmeporaryElectricalpowersupplyplanshallbepreparedandbeapproved.临电设施必须经验收合格后方可投入使用。每次大风、雨雪天气后进行一次验收。TemporaryElectricalpowerequipmentsmustbequalifiedbeforebeingused.Theequipmentmustbequalifiedaganafterstormorrain.现场电工人员必须持证作业。Electricianmusthaverelaventcertificatestocarryoutsuchworks.电气技术负责人必须在作业前向电工进行安全技术交底。PersoninchargerofElctricmustintroducetechnicalandsafetyissuebeforework.区域工长必须对所辖区域或分项工程的施工作业人员，针对所用的电气设备以及周围环境中可能发生的触电因素做好安全技术交底。Regionalforemanmustintroducethesafetyissuetotheoperators.临时用电安全器材必须按天津市建委的要求在定点厂家购买。TemporaryElectricalpowerequipmentsmustbeboughtfromtheproducerappointedbuTianjinConstructioncommittee.配电箱的内部设置必须符合国家临时用电安全技术规范及天津市地方标准。TheequipmentsinthedistributionmustaccordtoTechniclcodeoftemporaryelectricalsafety andTianjinlocalstandards.经理部每周组织一次全现场的安全用电检查，发现隐患，要定人、定措施、定时间及时整改。Organizeasafeuseofelectricityinspectiononceaweek,ifhiddenhazards,so严禁无证人员私自接、拆电气设备和线路。现场任何人员有权制止违章乱接线行为和擅自拆除电气设备的行为。Operatorwithoutcertificatesisprohibiteddemolitionelectricalequipmentandwiring.Anypersononsitehastherighttostopillegalbehavior任何人不得使用或向别人提供带有故障的或保护装置不全的电气设备、手持电动工具及灯具。Elctricalequipmentsinbadconditionmustn’tbeusedbyanyone.使用Ⅰ类电动工具和移动式电气设备的人员必须戴好绝缘手套。TheoperatorsusingGrade1Electricalequipmentmustwearprotectionandinsulatedgloves.维修电工必须做好维修工作记录，值班记录和各种设备的摇测记录。maintenanceelectricianmustdothemaintenanceworkrecords.A.1.9工伤事故报告、统计、调查、处理管理制度RegisterandreportingofInguriesmanagementsystem工程项目发生因工伤亡事故后，受伤者或事故现场有关人员要立即报告工程项目经理及指挥部和上级行政主管领导和部门。IFcasualtiesoccurred,theinjuredpersonorotherpeopleonsitereportitimmediatelytoprojectmanager.项目经理及上级领导和部门接到报告后，要立即赶往事故现场，组织指挥抢救受伤者，对受伤者的受伤部位作出判断，有选择地送往专业医院抢救，同时要向公司安全部门报告，分包工程项目经理要同时向总包项目经理报告。Projectmanagersshallrushtothesceneoftheaccident,organizetorescuetheinjuredandsenttheinjuredtospecializedhospital,andreporttosafetydepartmentalofthecompany.事故发生后，项目经理和有关领导及安全管理部门要立即采取措施制止事故蔓延扩大，组织人力和物力保护好事故现场。Aftertheaccident,theprojectmanagerandtheleadershipandsafetymanagementdepartmentshouldtakeimmediatemeasurestostopthespread,protectthescene事故报告的内容：Content3.1.事故发生单位名称、时间、地点。Company,time,place3.2.事故简要经过、伤亡人数、伤情程度、伤亡者姓名。Courseoftheaccident,thenumberofcasualties,extentofinjury,thevictimsname.3.3.事故现场采取的控制措施。Measuresadopted3.4.报告人姓名、工地电话。report’sname,sitephone3.5.同时准备好上级政府有关部门进行事故调查处理所需要的各种资料。Informationsneededbyrelaventdepartments事故发生以后，项目要立即成立主要领导参加的事故调查处理小组，负责对事故发生过程的调查分析与善后处理工作，同时向指挥部和上级行政主管部门提供有关事故的真实情况，配合其对事故的调查与处理。Aftertheaccident,theprojectsetuptheaccidentinvestigationteamimmediately,whichisresponsiblefortheaccidentinvestigationprocessanalysisandrehabilitationwork,andreporttherealsituationoftheaccidenttohigherheadquartersandadministrativedepartment.事故发生以后项目各级管理人员，要组织有关人员认真讨论，严格执行“四不放过”的原则，吸取教训，搞好整改措施，杜绝事故再次发生。Aftertheaccident,theprojectmanagersatalllevelsorganizerelevantpersonneltodiscussthereason,improvecorrectivemeasurestopreventfromhappeningagain.A.2职业健康与安全OccupationalHealthandSafetyA.2.1场地布置及临时设施建设Sitelayoutandconstructionoftemporaryfacilities（1）施工现场办公区、生活区应与施工区分开设置，并保持安全距离；办公、生活区的选址应当符合安全要求。Constructionsiteofficeandlivingareasseparatetheconstructionareashouldbesetupandmaintainasafedistance;office,livingareaofthesiteshouldmeetthesafetyrequirements.（2）施工现场应设置办公室、宿舍、食堂、厕所、淋浴间、开水房、文体活动室（农民工夜校培训室）、吸烟室、密闭式垃圾站（容器）及盥洗设施等临时设施Constructionsiteshouldbesetupoffices,dormitories,canteens,toilets,showers,drinkingwaterhousing,culturalandsportsactivitiesroom(eveningmigrantworkerstrainingrooms),smokingroom,closedgarbagestation(container)andtoiletfacilitiesandothertemporaryfacilities.（3）施工现场临时搭建的建筑物应符合安全使用要求，施工现场使用的装配式活动房屋应当具有产品合格证书。建设工程竣工一个月内，临建设施应全部拆除。Makeshiftbuildingsconstructionsiteshouldmeetthesafetyrequirements,theconstructionsiteusingPrefabricatedhousesshouldhavetheproductcertificate.Withinonemonthofcompletionofconstructionprojects,constructionoftemporaryfacilitiesshouldbefullyremoved.（4） 严禁在尚未竣工的建筑物内设置员工集体宿舍。Prohibitedinthebuildinghasnotbeencompletedtosetemployeedormitory.OperatingconditionsandenvironmentalsafetyA.2.2作业条件及环境安全Operatingconditionsandenvironmentalsafety（1）施工现场必须采用封闭硬质围挡,高度不低于1.8米.ConstructionsitemustbeclosedhardWeidangaheightofnotlessthan1.8meters.（2） 施工现场应设置标志牌和企业标识,按规定应有现场平面布置图和安全生产、消防保卫、环境保护、文明施工制度板，公示突发事件应急处置流程图。Constructionsitesignsshouldbesetandcorporatelogo,shouldberequiredfloorplanandsitesafety,firepreventionandsecurity,environmentalprotection,civilconstructionandthesystemboard,emergencyhandlingpublicityflow.（3）施工单位应采取保护措施，确保与建设工程毗邻的建筑物构筑物安全和地下管线安全。Constructionunitshouldtakeprotectivemeasurestoensuretheconstructionofadjacentbuildingsandstructuresundergroundpipelinesafetyandsecurity.（4）施工期间应对建设工程周边临街人行道路、车辆出入口采取硬质安全防护措施，夜间应设置照明指示装置。Responseduringtheconstructionofroadconstructionprojectsaroundthestreetpavement,thevehicleentrancetorigidsecuritymeasures,nightlightingshouldbesetindicatingdevice.（5） 定期对从事有毒有害作业人员进行职业健康培训和体检,指导操作人员正确使用职业病防护设备和个人劳动防护用品.Regularlyengagedinhazardousworkonoccupationalhealthpersonneltrainingandmedical,occupationalguidancetotheoperatortheproperuseofpersonalprotectiveequipmentandlaborprotectionarticles.（3）施工单位应为施工人员配备安全帽安全带及与所从事工种相匹配的安全鞋,工作服等个人劳动防护用品。Constructionworkersconstructionunitshouldbeequippedwithseatbeltsandhelmetstomatchthetypeofworkundertakenwithsafetyshoes,workclothesandotherpersonalprotectiveequipment.（4）
