ThegreaterHornofAfricaisfacingadirefoodinsecuritycrisisresultingfromextremeweatherevents,alongwithconflict,thefalloutfromtheCOVID-19pandemic,andhighfoodandfuelprices.Mostpartsoftheregionhavejustcomeoutofadrought,theworstonrecord,whileotherareasfacedsubstantialfloodingimmediatelyafter.TheonsetofElNinowasconfirmedinearlyJuly2023,amidexpectationsthatthisclimatephenomenonwillbringanotherperiodofextremeweatherevents,alongwithmorediseaseoutbreaks.Almost60millionpeopleareestimatedtobefoodinsecureintheregion,anincreasefromabout37millioninmid-2022whentheemergencywasdeclared.On-goingoutbreaksofmeaslesandcholeraareamajorpublichealthconcern,notleastbecausethecombinationofmalnutritionandthesediseasesoftenprovesfatal.

ThesituationinSudanhasbeenworsenedapre-existingclimate-inducedfoodandhealthcrisis.Priorto15April2023,over3millionpeopleneededshorttolong-termfoodassistanceduetopoorharvests.Thecountrywasalsointhegripofdiseaseoutbreaks,includingmeasles,hepatitisE,dengueandmalaria.However,nowover3millionpeoplehavebeendisplacedandatotalof24.7millionneedhumanitarianaid.4millionchildren,andpregnantandbreastfeedingwomenareacutelymalnourished.Inaddition,over100,000childrenundertheageoffivewhoandareseverelymalnourishedandhavemedicalcomplicationsneedspecializedcareatstabilizationcenters.Astaggering67%ofallmainhospitalswereoutofserviceasof31May.

Karamojaregion,locatedinthenortheastpartofUganda,isoneofthepoorestregionsinthecountryandcontinuestohavethehighestfoodinsecurityandmalnutritionlevels.Thisisbecauseofextremeweather,includingprolongeddrought,poorfeedingpractices,structuralpoverty,aswellaspoorhygieneandsanitation.Malariaanddiarrheacasesarestillhighintheregion,whichplacesahighdiseaseburdenonchildren,leadingtomalnutrition.Thisisexacerbatedbyhighfoodprices.Over580,000peopleareincrisisleveloffoodinsecurity(IPC3+)representing45%ofassessedpopulation,withover100,000intheemergencystage(IPC4).

SouthSudancontinuestofacemultipleconcurrentcrises,includinghighlevelsoffoodinsecurity,inter-communalviolence,conflict,extensivefloodinganddiseaseoutbreaks.Thefoodinsecuritysituationisexpectedtodeterioratein2023,with7.8millionpeople(abouttwo-thirdsofthepopulation)projectedtobecomehighlyfoodinsecure.Acriticalnutritionsituationexistsintheconflict-affectedareas.Overall,43,000peopleareonthebrinkofdyingfromstarvationunlesstheyreceiveemergencysupport.Accesstohealthservicesisamajorchallenge,particularlyamongdisplacedpopulations.SouthSudanisalsoamongthecountriesreceivingthehighestnumberofrefugeesfromSudan,with146,853havingcrossedintothecountryasof8July2023.

Somaliaisbracingtorecoverfromthreeyearsofdroughtthatbroughtnearfamineconditionsandthedeathof43,000peoplein2022.TheMarch-Mayrainyseasonusheredinsomerespitetofarmersandherders.Butfloodingdisplacedover250,000peopleanddamagedinfrastructure,includinghealthfacilities.Decadesofconflict,frequentepidemicsofcholeraandmeasles,widespreadpoverty,continuetoweakenandcausewidespreadfragilitytothecountry’shealthsystems.Nearly6.6millionpeople–overhalfthecountry’spopulation-werelikelytoexperiencehighlevelsofacutefoodinsecuritybetweenAprilandJune,with1.8millionchildrenexpectedtobeacutelymalnourished.Over40,000people(IPC5)arefacingdeathfromstarvationunlesstheygetimmediate

humanitarianassistance.

TheMarch-MayrainyseasonbroughtimprovementsintheconditionofcropsandlivestockinEthiopia.Yettheeffectofdelayedandsub-optimalrainsoverfiveconsecutiveseasonspersiststotheextentthatEthiopiafacessignificantdeteriorationinfoodsecurityalongwithpersistentlevelsofacutemalnutritionworsenedbyconflictanddisplacement.Over20millionpeoplearefacingcrisislevelsoffoodinsecurity(IPC3+).Otherchallengesincludethedisruptionofessentialhealthservices,routineimmunizationservices,coupledwithalackofcleanwaterandsanitationservices,andmentalhealthdisorders.Thecountryalsocontinuestograpplewiththeriskofinfectiousdiseases,suchasCholera,Diarrhea,Meningitis,HepatitisEandMalaria.
