执行安全生产的文件Documentsregardingsafetyproductiontobefollowed●国家政府颁布的安全生产规定的文件。Documentsissuedbythestateregardingsafetyproduction;●上海石化股份有限公司有关安全生产的规定。RulesmadebyShanghaiPetrochemicalCompanyLimitedregardingsafetyproduction;●公司有关安全生产的规定和文件。Rulesandregulationsmadebyourcompanyregardingsafetyproduction.5.7.2开工前安全交底DetailedsafetypresentationbeforeKick-offoftheproject在项目开工前，项目所有人员应接受业主安全部门的安全交底，以了解施工环境情况和安全注意事项。Everyoneshallattendadetailedsafetypresentationgivenbythesafetydepartmentoftheownerpriortothestartoftheprojectinordertohaveagoodunderstandingofsafetyissuesandworkingconditionsattheconstructionsite.在每项新工作开工前，专职安全员应向施工人员进行安全交底，使每个施工人员明白潜在危险的工作及所要采取的安全措施。Thesafetysupervisorshallgiveconstructionworkersadescriptionregardingsafetybeforeeachnewworktoenableeachworkertobeawareofpotentialrisksatjobsiteaswellasthepreventionmeasurestobetakenaccordingly.保留交底的书面记录。Safetypresentationanddescriptionshallbefiledinwrittenform.5.7.3现场安全检查SitesafetyInspection●建立系统性的现场安全检查制度，由安全员每日巡检并重点监控主要施工部位，正式的安全检查每周至少进行一次。Tosetupasystematicsitesafetychecksystem;thesafetysupervisorshallcheckandcontrolmainconstructionplacedaily;regularsafetycheckingshallbeperformedonceaweekminimum.●每一个现场人员的卫生和安全行为将被严密监督，使其规范化。Aclosesupervisiononeverysiteworkingpersonforhis(her)behaviorwithrespecttohealthandsafetyistobecarriedout.●检查出来的问题，立即进行整改，以消除危险，保证施工安全。Anyproblemthathasbeenfoundoutwillbeimmediatelycorrectedtoeliminatetoensurethesafetyofconstruction.5.7.4作业票制度OperationPermitssystem●现场项目部应遵守业主有关作业票制度，以防止安全事故的发生。Siteprojectdepartmentshallfollowanoperationpermitssystemimplementedbytheownertopreventincurringsafetyproblems.●施工作业证含以下类型：Constructionoperationpermitsincludethefollowing:a）动火证：焊接、切割、及任何明火工作，均需持有允许动火证，动火证必须明确动火地点、时间范围，并指定动火人。HotWorkPermit:welding,cuttingandanyotherworkneedsafireshallhaveaWorkingwithFiringPermit;suchWorkingwithFiringPermitshallindicatethepersonwhomistouseafire,thelocationwherethefiringworkisgoingtotakeplaceandthetimeframe;b）动电证：任何电气作业人员均需具有资格证书，须接线动电，应办理动电许可证。WorkingrequestingElectricityPermit:anyelectricianshallhaveaelectricianqualificationlicense.HeshallapplyforaWorkingrequestingElectricityPermitwhenapowercablewiringand/orpowerenergizingactionistobemade;c）探伤作业证：使用X射线和γ射线进行探伤检测时，必须获得探伤作业证，并采取适当防护措施。Non-destructiveTestingPermit:anypersonwhowilldonon-destructivetestingshallgetalicense.HeshallbearmedwithprotectivefacilitywhendoingtestswithX-rayorγ-raymachines.d）车辆通行证：所有车辆进入施工现场，均应按规定办理车辆通行证，戴好防火罩。VehicleTransportationPermit:anyvehicleneedstogointoconstructionsiteshallapplyforVehicleTransportationPermitandtakeafireprotectivehoodwhenitisreadytogointothesite.5.7.5临时用电的规定RegulationsaboutTemporaryPowerUsing●施工现场的临时用电的电源箱应合理布置。电源电缆敷设安全可靠。为防止电缆损坏，应架空或者有效敷设。配电箱的设置安全醒目，使用方便。Eachtemporarypowersourcecabinetatsiteshallbereasonablyallocated.Powercablesshallbesafelyandreliablylocatedhighincabletrayorinothereffectivemethodtoavoidgettingdamaged.Suchpowersourcecabinet,whichshallbeusedeasily,shallbeinstalledinasafepositionwhereiseasilyseenbypeopleatthesite●施工配电箱、盘都应安装断电保护器，有良好的接地装置。Allswitchboxesorpanelsshallbeequippedwithcircuitbreakerandreliableearthingdevice;●所有的施工用电气设备，应定期进行检查。发现问题及时整修或调换。电气设备使用完后，一定要关掉电源。Allelectricfacilitiesforconstructionmustbeperiodicallychecked.Anyproblemsfoundwiththefacilityshallgetrepairedorreplaced;allelectricfacilitiesmustbeswitchedoffwhenworkisfinished;●配电箱盘开关的外壳和闸刀开关均应保持完整无损。熔丝（片）应按设备或线路容量确定。Theserviceofthepowersourcecabinet,panelandswitchshallbekeptcompleteandundamaged.Fusestobeusedshallbechoseninaccordancewiththespecificationoftheequipmentorcapacityoftheline;●非电工严禁擅自装接用电器具,拉接电线。Anypersonsotherthanelectricianarenotpermittedtoinstallelectricalusersortowireapowercable.5.7.6施工脚手架搭设使用规定Rulesforinstallationofscaffoldings●脚手架的搭设应填写申报委托单及审批制度，手续需齐全。Scaffoldinginstallingshallmakeapplicationform.Approvalpermitshallbeavailablebeforehand;●施工脚手架建立脚手架搭设分阶段验收制度和专人维护保养。Sectionalcheckingandacceptancesystemshallbesetupforscaffoldinginstallingandmaintenanceservicebydesignatedpersonsystemshallalsobeestablishedforscaffoldingwhenitisbuiltup;●脚手架的材料及搭设，严格按照规程的要求搭设。具体要求见《脚手架安全搭设技术规定》。Materialschosenandinstallingofscaffoldingshallbemadestrictlyaccordingtorelevantregulation.(Thedetailscanbeseenin<SpecificationsforSafeInstallationofScaffoldings>).5.7.7施工机械使用规定Rulesforusingconstructionmachines●现场需用的各类机械必须经过维护、保养。确认机械的性能完好，方能使用。Alltypesofmachinestobeusedinconstructionsitemusthavebeenwellmaintained.Anymachinemustbecheckedifitstillhasagoodperformancebeforeusingit.●现场使用的各类机械应定机定人，特殊施工的机械的操作人员，应经培训考试合格，持有安全操作证。Adesignatedpersonmustbeappointedforamachinetobeuseatsiteforitsoperationandmaintenance.Anoperatorforrunningaspecialmachinemustgettrained,passanexaminationandhaveacquiredaOperationLicense;●施工机械应有良好的安全保护罩壳，并接地良好。Machineshallbeequippedwithprotectivehoodandwellgrounded;●各类机械的操作人员应了解施工机械的使用性能和操作要求。使用应检查是否完好无损，操作应遵守操作规程，发现故障应立即检修，严禁超负荷使用。Operatorsoperatingmachinesshallhaveagoodunderstandingofperformanceandoperatingrequirementsofthemachineshetakeschargeof.Heshallcheckitandbesurethatitisingoodconditionpriortorunningit.Operationproceduresshallbefollowedduringoperation.Machinesmustbestoppedwheneveramalfunctionisdetectedandgetitrepairedafterwards.Runningwithoverloadingisstrictlyprohibited.5.7.8起重机械（吊车）的使用规定Rulesforhoisting(cranes)machines●使用大型起重机、吊车必须按施工方案操作，明确其所使用的起重机能量和性能。Therunningofanytypeoflargesizehoistingmachine,cranesshallfollowaWorkingProgrampreparedforit.Thecapacityandperformanceofthehoistingmachinetobeusedmustbemadewellunderstood;●使用时应有专人负责指挥，并与驾驶员互通情况及统一指挥信号，防止因信号不明确而造成误操作。Adesignatedpersonshallbeappointedtoguideoperationofacrane.Thispersonshallcommunicatewiththecranedriversoastoconsolidateaunifiedoperationsignalstoavoidanymissoperatingincurredbywrongsignals;●起重机行走和停放处，应注意保护地下电缆、管道、地沟或者其他保护措施。Thetrenchesandpowercablesandpipingembeddedundertheroadwhereahoistingmachineoracranewillgobyorstopataboveshallbewellprotectedwithnecessarymeasures;●吊车进行吊装时。应伸足支腿，支腿下放置路基板。车身放置平稳，在进行起吊、行走、回转等多项动作时，指挥人员应视周围情况，认为安全后，方能指挥。Acraneshallgetitssupportswellstretchedontoabase-plateonthegroundbeforehoisting.Thecranemustpositionedstable.Thepersonwhowillguidetheoperationofthecarneshallcheckthesurroundingsandgetconfirmedthatthereisnothingunsafebeforethecranelifts,movesandrotates;●使用的吊索具应有足够的安全系数，并检查钢索的完好无损。Hoistingcablesshallbecheckedfortheirsafetycoefficientaswellastheircompletenessandingoodcondition.5.7.9 安全用火规定RulesforFireUsingonSite●氧气、乙炔与油漆、油类物品须设置专用区，并分别存放，不用时盖上瓶盖，明确标识。Oxygen,acetyleneandpaintandoilproductsshallbeclassifiedandseparatelystoredwithaclearmarking;●对有危险品如易燃/易爆场所，应挂有“严禁烟火”警告牌，并配置相应的消防、灭火器材。A“SmokeandFlameProhibited”signforwarningshallbeputatanareawherehazardousproductssuchasinflammables/explosivesarestored.Necessaryfire-fightingandextinguishersshallbearranged;●凡是用火的地方，应按规定办理动火证，制定防火措施，并设专人看护。工作完毕后，必须将火种彻底熄灭。A“WorkingRequestingFiringPermit”mustbeappliedforaworkneedsit.Necessaryprecautionsshallbetakenandaguardianforsupervisingshallalsobearrangedatthatareawhensuchworkisperforming.Allfiringsubstancemustbeclearedoutwhenthatworkiscompleted;●在高处动火（如电焊、气焊、气割）的下方周围，如有易燃物应予清除或采取用不燃物遮盖，并设专人监护。Whenaworkrequestingafiring(likeelectricwelding,gasweldingangascutting)isperformedatapositionabovetheground,anythingflammableunderitatthegroundshallbeclearedoutcompletelyorplaceanon-flammablesheetatsuchareabeforehand,aguardianforsupervisingshallalsobearrangedatthatarea;●扑灭火灾时，应遵守电气设施严禁用火和泡沫灭火的规定。油漆、油类、沥青、有机溶剂等易燃液体及忌水化学物品，严禁用水灭火。Whenafireisunfortunatelycaused,therulesthatwaterandfoamsareprohibitedforextinguishelectricfacilitiesnorthewaterforpaint,oilproducts,asphalt,organicsolventsaswellaswater-forbiddenchemicals;●消防器材不得挪作它用，周围不准堆物，保持道路畅通。Fire-fightingequipmentordevicestobepositionedinaplacetowhichtheaccessisfreefromanyobstacleandcannotbeusedforanyotherpurposes.有关探伤工作的规定RulesforNDT探伤人员必须持有资格证书。ANon-destructivetestingtechnicianshallhaveaspecialpermitforhisjob;根据施工进度做好射线探伤工作计划，严格控制施工程序和施工范围。Aworkplanshallbeworkedoutaccordingtoconstructionschedule.Theworkingareaandworkingproceduresshallbestrictlycontrolled;探伤区域应有明显标志，并树立“探伤危险”的警告牌，以防止其他人员进入。Signsfwith“DangerforNDT”orwarningshallbeputatanareawherepeopleatthatareacaneasilyseetoavoidanyothersenterintowhenNDTiscarryingout;探伤工作至少要由两人进行，探伤工作人员应有标志。NDTshallbeundertakenbyatleasttwoNDTpersonswhoshallwearspecialjobmarks.5.7.11安全生产“六大”纪律：“SixDisciplines”forSafetyProduction进入现场必须戴好安全帽，扣好安全带，并正确使用个人劳保防护用品。“Wearingsafetyhelmet”,“fastenone’ssafetybelt”,“correctlyuseone’sprotectiveproducts”mustbewelldonebeforeonegoesintoasite;二米以上的高处，悬空作业，无安全设施的，必须戴好安全带，扣好安全钩。Onemustwearhelmetandfastenhissafetyhookwhenheisworkinginaplacewherethereisnosafetyprotectionsandis2metershighabovetheground;高处作业时，不准往下或往上乱抛材料和工具等物体。Donottossorthrowdownanykindsofmaterialsandtoolswhenworkiscarryingoutatahighposition;各种电动机械设备，必须装置漏电开关，有可靠有效的安全接地和防雷装置，方能开动使用。Noelectricdrivenmachinecanstartworkingifitisnotequippedwithpowerleakageprotectiveswitchandreliableearthingaswellaslightningprotectiondevices;不懂电气和机械的人员，严禁使用和玩弄机电设备。Anyoneotherthananelectricianisnotallowedtouseorplaywithaelectricdrivenmachineorfacility;吊装区域非操作人员，严禁入内，吊装机械必须完好，扒杆垂直下方不准站人。Anyoneotherthanapersonwhoisoperatingahoistingmachineoracraneisnotallowedtoenterintotheareawherehoistingiscarryingout.Hoistingmachinesmustbewellmaintainedandingoodcondition.Nooneispermittedtostandunderthecraneboom.5.8文明施工Civilizedconstruction文明施工是防止安全事故发生的重要组成部分，垃圾杂物和其它易燃材料堆积在施工现场，必然增加火灾的危险性和人员受伤的可能性。因此要求项目部管理人员和施工人员保证有一个清洁的工作环境，项目部管理人员应定期进行检查，以保证文明施工条例的执行。必须做到：Civilconstructionisaveryimportantparttoprotectincurringasafetyaccident.Garbageandotherinflammablematerialsthatarescatteredatsitewillincreasethepossibilityofincurringfireand/orcausingaccidentstoworkersatsite.Somanagepersonnelandconstructionworkersundertheprojectdepartmentshallpayattentiontothatandensuremakingtheconstructionsiteclean,theprojectmanagepersonnelmustcheckitatregularintervalsandmustensurethefollowing:5.8.1所用的材料、工具和设备适当放置。过量的材料、工具和设备尽量不要放高处，搁置在高处的材料、工具和设备应将其绑扎牢固，以防坠落伤人。Allmaterials,toolsandequipmentshallbeproperlyallocatedandplaced.Anyexcessofmaterials,toolsandequipmentshallnotbepositionedatahighplace.Thosealreadyplacedatahighplaceshallbefastenedfirmlytoavoidgettingthemdroppeddownandhurtpeopleconsequently; 随时清理不用的材料和垃圾，并撤离现场。Anymaterialsthatarenolongerusedshallberemovedandanygarbageshallbeclearedout;5.8.3 滴漏的油料及溢出的流体应立即清除，并经合适的处理。Anyliquidthatisoverflowedshallbeimmediatelyremovedanddisposeit;5.8.4
