Conditions for renewal of this fixed-term appointment are not to be stated in this letter. Do not present this letter to the appointee or make an offer of appointment until the terms and conditions of the offer in this letter have been approved by the Office of Human Resources. If you have questions, please contact the Office of Human Resources.Note: * 9-Month Appointments: 9-month academic year appointment begin and end dates are September 16 June 15. 9-month appointments generally start and end on either an academic year basis or on a term basis. Term begin and end dates are as follows:September 16 December 15December 16 March 15March 16 June 15 * 12-Month Appointments: 12-month fiscal year appointment begin and end dates are generally July 1 June 30. A faculty member, however, may begin employment on or after July 1. For FLSA Exempt employees:This position meets the criteria for exemption from the provisions of the Fair Labor Standards Act, thus, you will not be eligible to receive overtime compensation.For FLSA Non-Exempt employees:This position does not meet the criteria for exemption from the provisions of the Fair Labor Standards Act, thus, you will be eligible to receive overtime compensation, as appropriate. Working overtime requires prior approval from your supervisor.Start-up Package The start-up package paragraph is optional. However, if it is included, it should state WHAT the department/college is firmly committed to providing in a start-up package, not what the department/college will TRY to achieve. This paragraph should not describe HOW the department/college will deliver its commitments as these are matters internal to the department/college. If you are providing a start-up package, insert your paragraph here. We encourage you to review the sample wording at /admin/hr/starter.html.Moving Expenses If the employee will receive reimbursement for moving expenses, please include the following paragraph:In addition to your salary, Oregon State University will reimburse you up to $_ for household moving expenses in accordance with OSU and Oregon University System (OUS) policies. The Office of Human Resources Employee Benefits section will forward a benefit options package in care of your department for your review. Medical, dental, and life & disability insurance enrollment forms must be completed and submitted to Employee Benefits within 60 days of your date of employment in order for your to receive coverage. Your coverage is effective the first day of the calendar month following your date of employment and your submission of the enrollment application. Questions may be directed to Employee Benefits at (541) 737-2805.Select one of the following paragraphs if you are appointing a non-resident alien into this position:1) If the department/unit is paying for visa processing fees, use this paragraph:OSU will pay fees for visa services provided by the International Student and Faculty Services (ISFS) Office. Upon receipt of payment from our department prior to the start of your employment at OSU, ISFS will provide visa services. For more information about ISFS services, please visit /international/atosu/scholars.2) If the appointee will be required to pay for visa processing fees, use this paragraph:You will be required to pay fees for visa services provided by International Student and Faculty Services (ISFS) Office. SOCIAL SECURITY CARD Use if appointee has never been employed by OSU:Please note that you will need to make available to department personnel your original Social Security card for payroll verification purposes when you begin your employment with OSU.
