there are appropriate controls for the temporary and permanent disarming or deactivating of critical alarm control and shutdown equipment and other critical emergency or safety equipment such as fire Company personnel contractors and their subcontractors Contractors may however when agreed through the Company project engagement processes utilize their own standards and procedures in so far as the requirements of their standards are at least equivalent to those of this Standard the Customer s standards and those of applicable National and Local regulations which should be read and complied with for the application of and compliance with this Standard at work locations where they have operational responsibilities Company managers will also be the focal points for associated liaison with and 本标准的要求同样适用于公司办公地点 例如办公室内受限空 间进入需要取得作业许可证 4 REQUIREMENTS 要求要求 4 1Minimum System Requirements 最低体系要求最低体系要求 a A system shall be in place wherein work categories requiring a permit to work permit shall be defined for each site group of sites or project as appropriate based on the risks involved Work requiring a permit shall be defined after considering factors such as work type work location complexity of the job environmental conditions and the types of potential hazards The permit to work system shall address all the hazards identified by experience history and the hazard register s developed during risk assessments of operations at the site or sites It shall provide specific controls for work where regulatory and or customer requirements exist and will provide specific controls where indicated by industry best practice for example all confined space entry is controlled by a specific permit 应确立适当的体系 界定每个现场 每组现场或项目基于风险考 虑要求取得作业许可证的作业活动类型 界定哪些作业活动需要取得作业许可证 时需要考虑的因素如下 作业类型 工作地点 作业的复杂性 their responsibilities There shall be a nominated person to manage the permit to work system 现场经理应书面指定作业许可证负责人及其职责 应指定专人负责 作业许可证制度的管理 c Permit Holders shall be responsible for the activities of the work groups covered under a specific permit 作业许可证持有人员应对某一特定作业许可证所涵盖的 作业活动负责 d People assigned to the above designated roles will be trained in the permit to work system its operation and shall be competent in their specific roles in its operation 上述指定人员应接受作业许可证制度及其运行方面的培训 并有能力 履行其在作业许可证制度下的职责 e Each work location will have a system for the approval of works to be conducted The work location into any space which may have insufficient oxygen levels or where a build up of harmful gases or other hazardous substance may have taken place or where dangerous flora or fauna might exist e g a vessel tank trench or underground space Other separate permits are required to perform the work for which entry to the confined space is required e g a hot work permit if hot work is to take place or work permit document shall specify the work to be performed The detail shall be sufficiently specific to permit the classification of the work in accordance with the classifications established by the work permit system for example hot work confined space entry etc and to identify all generic and site specific hazards involved in the work 在作业许可证文件当中 应 对拟执行的作业活动加以说明 此类说明应足够详细 以便依据作业许可证 制度所确立的分类标准划分作业类型 如动火作业 受限空间进入等 并 明确作业活动所涉及的一般危害和现场特有危害 ii The work permit document shall specify the specific item s to be worked on with sufficient detailed to enable assessment of the impact of the work on associated equipment and any precautions required 在作业许可证文件 当中 应对作业对象加以说明 说明应足够详细 以便评估作业活动对相关 设备的影响以及所需采取的防护措施 iii The work permit document shall specify the tools and equipment required to carry out the work 在作业许可证文件当中 应对执行作业活动所需使用 的工具和设备加以说明 iv The work permit document shall have provision for references to any written procedures methods risk assessments JHAs etc 在作业许可证文 件当中 应具有所需参照书面程序 方案 风险评估和作业危害分析等方面 的规定 b Equipment Isolation and Preparation For Work 设备隔离和作业准备 i The permit shall refer to all steps to be taken to isolate energy sources from the equipment or area affected by the work 作业许可证应提及将能量 源与受作业影响的设备或区域相隔离所需采取的步骤 ii The permit shall refer to all steps to be taken to remove stored energy from the equipment or area affected by the work and to make the equipment safe to work upon This will include venting draining purging gas freeing cleaning keeping wet or other activities required 作业许可证 应提及将存储的能量从受作业影响的设备或区域移除并确保设备可以安全作 业所需采取的步骤 其中包括 放空 排泄 吹扫 气体释放 not exposed to health or safety risks All exposure hazards encountered at a site or work location should be considered when drawing up the permit Standard lists should be available for each site permit such that energy isolations if any and equipment inhibits are verified the site tested and shown to be safe for the work before the permit holder accepts responsibility for the permit
