满足中国政府对工程建设强制性标准条文 施工验 收规范 工程质量验收统一标准以及项目设计文件的要求 This procedure shall be used by EPC contractor or construction contractor to apply to XXX for inspections and acceptances on as received materials structural members and equipment entering the construction site during the construction process so as to satisfy relevant PRC construction standards acceptance codes uniform project quality acceptance standards and project design documents 2 0 SCOPE 范围 2 1本程序适用于 YEZHU石化项目所有工程材料 设备等进入施工现场前的报验申请 This procesure applies to applications and acceptances of all project materials and equipment etc for the YEZHU Petrochemicals Project before entering the construction 3 0 DEFINITIONS 定义 3 1 合格证 Certificate of conformance EPC 承包商和 或施工 C 承包商所有工程材料 设备等进入施工现场前 均应提 供原厂产品出厂合格证 Before entering the construction site all EPC contractor or construction contractor materials and equipment etc shall be provided with ex work certificate of conformance 3 2 质量证明文件 Quality documents 质量证明文件包括但不限于 材料 设备生产厂质量部门出具的质量检验报告原件 技术资料 重要部位原材料还应提供物理 化学性能检测报告等 Quality documents shall include but are not limited to quality inspection reports original technical documents issued by the quality control department of the material and equipment manufacturer and raw materials physical and chemical performance test reports for major components etc 3 3 见证取样 Witness sampling 监理工程师对工程项目使用的材料 半成品 构配件的现场取样活动实施见证 The supervision engineer shall witness the site sampling of materials semi products and structures and fittings used by the project 4 0 RESPONSIBILITIES 职责 监理工程师负责本程序的实施 材料设备进场检验程序 Page 4 of 8 Supervision engineer shall be responsible for the implementation of this procedure 核验原材料 设备出厂合格证 Check the ex work certificate of conformance for raw materials and equipment 核验材料 设备检验或试验报告 Check material and equipment inspection or test reports 组织复验或见证取样试验 Organize for re inspection or witness sampling tests 确认材料 设备合格后签署报审表 Sign the inspection application form after confirming that the materials and equipment are qualified 5 0 REQUIREMENTS 材料检验要求 5 1 凡运到施工现场的材料包括半成品 构配件 进入施工现场前应向XXX区域监 理部提交 工程物资进场报验表 见附表A9 Poject Material As Received Inspection Application Form see attached A4 shall be submitted to XXX area supervision department for all arrived materials including semi products and structural members before they enter the construction site 5 2 提交报审表时应该提供产品出厂合格证及技术说明书原件 按规定要求进行 检验的检验或试验报告原件 这些原件由EPC 承包商或C承包商保存 提交给 XXX的宜是原件或复印件加盖原厂印章 The product ex work certificate of conformance and technical instructions original and inspection or test reports original for materials which are requested to be inspected shall be provided at the same time with the submital of inspection application form These original copies shall be kept by EPC contractor or construction contractor and those submitted to XXX should be originals or copies with aditional stamps by the original manufacturer 5 3 EPC 承包商或C承包商应确保任何进入施工现场的材料 设备满足相应的质量 标准 为此承包商有责任提交能说明其满足质量标准的任何技术文件和检验或 试验报告 EPC contractor or construction contractor shall ensure that all materials and equipment entering the constuction site satisfy relevant quality standards Therefore the contractor shall have the responsibility to provide any technical documents and inspection or test reports which can demonstrate that the materials and equipment satisfy relevant quality standards 5 4 当监理工程师认为EPC 承包商和 或施工 C 承包商提交的有关产品合格证 或检验试验报告仍不足以证明到场产品的质量符合要求时 监理工程师可以要 求进行复核性试验或见证取样试验 这些试验应由第三方试验室承担并初具试 验结果报告 以确认其质量是否合格 谁付钱 材料设备进场检验程序 Page 5 of 8 When the supervision engineer thinks that the product certificate of conformance or inpection test report is still not enough to prove that the arrived product satisfies the relevant quality requirements he can request for conducting re inspection tests or witnessed sampling tests These tests任何进入施工现场的材 料 设备只有经监理工程师审查并确认合格后方可进入施工现场 shall be conducted in a third party lab which will issue the test reports to verify whether the quality is in conformance who will pay the costs 5 6 获得合格确认的材料进入施工现后 到其使用或安装时通常要经过一定的时间 间隔 如果对材料的存放 保管疏忽 可能导致材料质量状况的恶化 如果此类事件 发生 监理工程师有权对其再次检验 视检验结果再确定是否准用或降级使用 After the positively verified materials enter the construction site normally there is a time interval up to their installation or services If the materials are not properly stored and maintained it is likely to result in quality degradation When such an incident occurs the supervision engineer shall have the right to conduct an inspection again Whether the materials are allowed to be used or used as lower standard materials shall be dependent on the inspection results 5 7 对于检验不合格的材料不得进入施工现场 并对其进行标记 追踪 防止二次进入现 场的可能 Materials that fail to pass the inspection shall not be allowed to enter the construction site They shall be identified and tracked so as to prevent them from entering the construction site again 6 0 EQUIPMENT INSPECTION REQUIREMENTS 备检验要求 6 1 设备进入现场前 承包商应向XXX工程师提交 工程物资进场报验表 见 附表 A4 Before the equipment entering the construction site the contractor shall submit to the XXX engineer the Project Material As Received Inspection Form see attached A4 6 2提交报审表时应该提供设备出厂合格证及技术说明书 质量检验证明 有关 图纸和技术资料 经XXX工程师审查合格 在报审表上签认后设备方可进入 施工现场 The equipment ex work certificate of conformance and technical instructions and quality inspection documents shall be provided when submitting the inspection application form The equipment cannot enter the construction site until the above documents are reviewed and the inpsction application form is signed off by the XXX engineeer 6 3设备进场后 XXX工程师应组织承包商在规定的时间内检查 供货方应派人 参加 按供货方提供的设备清单及技术说明书 相关质量控制资料检查验收 经检查确认合格则验收人员签署验收单 After the equipment enters the cosntruction site the XXX engineer shall organize the contractor to carry out related inspections within specified timing which shall be attended by supplier representative The inspection and acceptance shall be conducted as per equipment list and technical instructions related quality control documetns provided by the supplier When the inspection shows a positive result the acceptance personnel shall sign the acceptance sheet 材料设备进场检验程序 Page 6 of 8 6 4若供货方质量控制资料有误 或实物与清单不符 或对质量资料的正确性有 怀疑 或设计文件及验收规程规定的必须复验合格后才可安装的设备 应由 有关权威部门进行复验 If there are any mistakes in the quality control documents or the materials are not consistent with the list or there are any doubts about the correctness of the quality control documents or relevant design documents and acceptance regulations require that the equipment must be qualified by a re inspection before installation re inspections shall be conducted by relevant authorities 6 5如经检验发现设备质量不符合要求时 则XXX工程师拒绝签认 由供货方予 以更换或处理 合格后再进行检查验收 If the inspection show that the quipment does not satisfy quality requirements XXX engineer shall reject to sign off and the supplier shall replace it or take proper corrective actions followed by another inspection and acceptance 6 6工地交货的大型设备 一般由供货方运至工地后组装 调试和试验 经自检 合格后再由XXX工程师组织复核合格后予以验收 Large equipment for site delivery is normally shipped to the site by the supplier and then is assembled commissioned and tested The XXX engineer shall organize the inspection on such equipment following a positive self inspection The eqiupment will be accepted after the inspection shows a positive result 6 7进口设备的检查验收应会同国家商检部门进行 Inspection and acceptance on mported equipment shall be conducted together with the state commodity entry and exit inpsection department 6 8设备开箱检查应按下列项目进行并做好记录 Equipment OPI shall be conducted on the following items and records shall be taken 箱号 箱数及包装情况 Case No number of cases and packaging 设备的名称 型号 规格 Name type and specifications of the equipment 装箱清单 设备的技术文件 资料和专用工
