确性 ;改进育种方式 ( IVF 技术 、胚胎操作技术等应用) ; 合理 、安全的保存实验动物资源 ( 胚胎冷冻 、精子冷冻 、卵巢冷冻等技术的应用) 。6) 加强实验动物的信息交流与共享 :建立各种实验动物的标准生物学特性数据库 ;向动物实验人员提供 所使用动物的背景资料和数据 ;向动物实验人员提供所使用动物的实验和处置方法 ;直接提供实验所需的动物器官 、血液 、胚胎等 ,以合理的和尽可能地减少实验动物的使用量 。7) 实验动物运输中的动物福利 :尽量减少动物的运输时间 ,以减轻实验动物的疲劳和痛苦 ,以免给实验 结果带来误差 ;运输中应根据不同实验动物的需要 ,给予适当的饲料和饮水 ; 根据不同实验动物的 ,生理 、生 态 、习性等 ,采取适当的不同的方法进行运输 ;应选用必要的规模和构造的运输车和运输箱 ,以保证实验动物的健康和安全 ,并防治实验动物的外逃 ;对运输箱采取必要的过滤措施 ,以防治外界对实验动物的微生物感 染及污物对环境的污染 。我国加入了世贸组织 ,保护动物 、善待生命不仅是所有善良人的愿望 ,也是社会走向稳定和文明的重要标志 ,更关系到国家在国际上的形象 。实验动物对人类的健康贡献巨大 ,善待实验动物就是善待人类自己 。细菌内毒素检查法替代生物材料和医疗器械热原检查的近况陈丹丹黄清泉王春仁奚廷斐(中国药品生物制品检定所 ,北京 100050)关键词 :细菌内毒素 ; 热原检查 ; 生物材料 ; 医疗器械1 概述生物材料和医疗器械是指用于诊断和治疗的介入和植入人体的材料和器械 。生物材料和医疗器械是近30 年来发展起来的新兴学科 ,目前已经在临床广泛应用 。如何保证生物材料和医疗器械的安全性问题越来 越受到各国政府和学术界的重视 。在生物材料和医疗器械的安全性评价中 ,热原检查无疑是重要的内容之 一 。1942 年热原检查法首次收载于美国药典 12 版 (USP ) 。随后世界各国药典相继收载了这一方法 ,半个多世纪以来该方法一直作为法定的热原检查法为各国药典所采用 。各国药典收载的热原检查方法基本相同 。该法是将一定剂量的供试品 ,静脉注入家兔体内 ,在规定的时 间内 ,观察家兔体温升高的情况 ,以判断供试品中所含热原的限度是否符合规定的一种方法 。直至目前 ,各 国药典确认的热原检查法仍系沿用 60 年前建立起来的家兔升温试验 。由于家兔法本身存在的局限性 ,人们一直试图寻找一些新的方法替代或补充 。因为家兔法虽然能直观地反映各种热原的致热程度 ,但不易标准化 、不能定量 、重现性差 、灵敏度不如鲎试验高 、受某些药品本身的 药理活性及理化性质的影响较大 、费用高 、费时 、结果与家兔的品系和饲养条件有关 、涉及使用动物等 。目前 已经成熟的热原检查的替代方法是细菌内毒素检查法 。细菌内毒素检查法 (Bacterial Endotoxins Test ,B ET) 自 1968 年由美国学者 Bang 和 Levin 正式建立 ( 该法又称作鲎试验 ,即 Limulus Test) 至今 ,经过三十多年的不断完善和发展 ,现巳在医药卫生 、环境保护等方面发挥 着极其重要的作用 。细菌内毒素检查法在药品检验中的应用是从放射性药品和注射用水开始的 。1980 年 USP20 版首次收载细菌内毒素检查法 ,1983 年 USP20 第四增补本规定 5 种水和 40 种放射性药品采用细菌内毒素检查法代替热原检查法 。1991 年 ,USP22 版 第 五 增 补 本 又 规 定 185 种 药 品 采 用 细 菌 内 毒 素 检 查 法 代 替 热 原 检 查 法 。1995 年 ,USP23 规定 471 种药品进行细菌内毒素检查 ,保留热原检查的品种不足 40 种 。美国药典 24 版共收 载 579 种药品进行细菌内毒素检查 。对于那些直接或间接与心血管系统或软体组织接触的标有无热原字样的生物材料和医疗器械来说 ,进行热原检查是必须的 。美国药典中收载的进行细菌内毒素检查的品种如下 : 液体给药设备 ,延伸设备 ,传输 设备 ,血液给药设备 ,静脉内导管 ,植入的体外氧气导管和附件 ,透析器 ,透析管和附件 ,血管移植物 ,心瓣膜 , 肌肉内药物传送管和输液 (血) 器等 。2 现在已经采用细菌内毒素检查法替代热原检查法的品种我们统计了我们工作中已经采用内毒素检查法的品种共有 35 种 ,具体如下 ,这是一个不完全的统计 ,因为还有其它我们没有涉及的品种 。它们是 :眼科用粘弹剂 、人工晶体 、下腔静脉滤器 、必施无菌灌注液 、疝修补网织片 、体内埋植泵 、冠脉支 架系统 、一次性使用无菌穿刺针 、胆道球囊导管 、一次性血液管理套件 、眼科用重水 、硅油 、肾动脉支架 、膜式氧合器 、一次性输液器 、一次性注射器 、J RS 系列自弹式血管内支架及输送系统 、J RZ 系列不锈钢 Z 型支架及 输送系统 、J RN 系列镍钛形状记忆合金支架及输送系统 、CL INI MACS 管道系统 、巴德 LUMINEXX 支架放置系 统 、眼内灌注用平衡盐溶液 、CL INI MACS CD34 细胞分选试剂 、颈动脉转流管 、静脉导管套件盒及附件 、眼用 透明质酸钠 、海藻酸钠微球血管栓塞剂一次性使用麻醉穿刺包 、张力环 、青光眼引流器 、输精管滤过装置 、外 科种植体 、组织固定系统 、血管移植物 、生物陶瓷义眼台 、器官保存液等 。3仍然采用热原检查法的品种根据我们工作中的品种进行统计 ,仍有 18 种采用热原检查法 ,即家兔法 。它们是 : 止血胶原海绵 、医用即溶止血纱布 、一次性活检装置 、一次性使用输血泵 、心脏瓣膜 、尼龙眼科带针缝合线 、一次性使用中心静脉 导管 、PGA 可吸收缝合线 、人工骨粒 、生物衍生骨 、外科修补材料 、水凝胶乳房植入体 、一次性使用创伤手术缝合包 、盐水乳房假体 、硅凝胶乳房假体 、可吸收椎间融合器 、聚丙烯粒料 、离心泵泵头等 。4采用细菌内毒素检查法替代家兔法的优缺点优点 :方便 、快捷 、经济 :缺点 :干扰作用 、回收率低 。5应用生物材料和医疗器械的热原检查用细菌内毒素检查法来替代是大势所趋 ,相信随着细菌内毒素检查法的广泛应用 ,越来越多品种的热原检查将会被鲎试验法所取代 。同时这也是动物保护的一个重要内容 。自1959 年 W. M. S. Russell (动物学家) 和 R. L . The questions that how to guarantee the use safety of biological materials and medical instruments are getting more attentions from governments of many countries and academia circles. Among all the activities relating to evaluation of the safety of biological materials and medical instruments , the pyrogen test is , undoubtedly , one of important issues.The 19422year edition of the pyrogen test was firstly recorded in the 12 edition of USA pharmacopoeia (USP )which gradually printed in other countrys pharmacopoeias. For more than half a century , the pyrogen test is regarded as an all2accepted method printed in almost every pharmacopoeia and applied around the world.All pyrogen tests printed in these entire pharmacopoeias are basically the same . The pyrogen test means : infusing a certain amount of provided substance into domestic rabbits body by vein ; observing the rise of the rabbits temperaturewithin the set period ; purposely judging whether pyrogen containing in the provided substance meets the demands or not . Nowadays , the pyrogen test confirmed by the pharmacopoeia of every country is still used with the evolution of domestic rabbits temperature test set up 60 years ago .People have always been looking for some replacement and complements because of shortcomings of the domestic rabbit method itself . Although the domestic rabbit can directly reflect the various temperature changes of test , on the oth2 er hand , rabbit test which easily influenced by some drugs is not as good as Limulus test in the standardization , fixed quantifying , capabilities of recurrence and sensitivity , expenses and time . The outcome of rabbit test is too much relating to the animals feeding condition and factors of itself . At present ,the well2accepted substitute of pyrogen test is Bacterial Endotoxins Test ,B ET.Bacterial Endotoxins Test (B ET2also called Limulus Test) was formally set up by American scholar Bang and Levin. Up to the present , after more than thirty yearsdevelopment and improvement , it has been playing an important role inthe fields of health , medicines and environment protection.The applications of Bacterial Endotoxins Test in pharmacy test experiment began from radioactive medicine and water used for injection. The 19802year edition of USP20 firstly carried the Bacterial Endotoxins Test , and 19832years fourth supplements regulated that five kinds of water and forty kinds of radioactive drugs should introduce Bacterial EndotoxinsTest rather than pyrogen test and then 185 kinds of drugs in 19912years edition of USP22 , fifth supplements. The 19952 year edition of USP23 regulated that 471 kinds of drugs should introduce Bacterial Endotoxins Test , and less 40 kinds re2 maining with pyrogen test .
