腹部 The abdomen,南京大学医学院人体解剖教研室,腹部 The abdomen,1、境界与分区 Boundaries and division上界：胸廓下口 下界：耻骨联合上缘、耻骨嵴、耻骨结节、腹股沟韧带、髂嵴与腰5下缘的连线。以腋后线为界：分为腹前壁、腹外侧壁、腹后壁腹腔the abdomen cavity：以小骨盆为界分为固有腹腔和盆腔，其实际范围要大于腹部的境界分区：九分法、四分法,表 面 解 剖,1、体表标志surface landmarks骨性标志：耻骨联合pubic symphysis、肋弓costal arch、剑突xiphoid process、髂前上棘anterior superior iliacspine、髂嵴iliac crest、耻骨结节pubic tubercle软组织标志：腹白线linea alba半月线linea semilunaris2、体表投影surface projections,腹外侧壁（1）The lateral abdomen wall,浅层结构皮肤 the skin薄而富有弹性，常为临床选择游离皮瓣浅筋膜 the superficial fascia：Camper筋膜（脂肪层）、Scarpa筋膜（膜样层），该层连到会阴浅筋膜（colls筋膜）,浅动脉：腹壁浅动脉the superficial artery、旋髂浅动脉the superficial circumflex iliac artery浅淋巴：脐以上汇入腋淋巴结，以下汇入腹股沟浅淋巴结,腹外侧壁（2） The lateral abdomen wall,浅静脉the cutaneous veins：脐以上-胸腹壁静脉the thoracoepigastric veins 胸外侧静脉the lateral thoracic veins-腋静脉脐以下-腹壁浅静脉the superficial epigastric vein-大隐静脉上述静脉-附脐静脉paraumbilical vein-门静脉,皮神经the cutaneous nerves： 前皮支the anterior cutaneous n. 外侧皮支the lateral cutaneous nerves 髂腹下神经the iliohypogastric n.,腹外侧壁层次结构,1、皮肤 the skin 2、浅筋膜the superficial fascial 3、深筋膜the deep fascia4、腹外斜肌the obliquus externus abdominis5、腹内斜肌the obliquus internus abdominis,6、腹横肌the transversus abdominis 结合肌、联合腱conjioned tendon（腹股沟 镰the inguinal falx）睾提肌the cremaster7、腹横筋膜the transversalis fascial凹 间韧带、 腹环8、腹膜外脂肪extraperitoneal fascial（腹膜下 筋膜）9、腹膜壁层parietal peritoneum,腹外斜肌the obliquus externus abdominis,1、腹股沟浅环the superficial inguinal ring （皮下环、外环）2、内侧脚the medial crus （上脚）3、外侧脚the lateral crus（下脚）,4、脚间纤维the intercrural fibers5、反转韧带the reflected lig.6、腹股沟韧带the inguinal ligament7、腔隙韧带the lacunar lig.（陷窝韧带）8、耻骨梳韧带the pectineal lig.,血管、淋巴和神经,1、血管：腹内斜肌和腹横肌之间有下5对肋间动脉和 4对腰动脉。还有腹壁上、下动脉及旋髂深动脉2、淋巴：腹上部-肋间或胸骨旁淋巴结 腹中部：腰淋巴结 腹下部：髂外淋巴结3、神经：第7-12对胸神经、髂腹下神经 iliohypogastric nerve、髂腹股沟经ilioinguinal n.、 生殖股神经genitofemoral n.,腹前壁the anterior abdomen wall,1、腹直肌the rectus adbominis m.2、腹直肌鞘the sheath of rectus abd. m. 鞘前层anterior layer of sheath 鞘后层posterior layer of sheath 脐下4-5厘米,后层缺如，只有腹横筋膜3、弓状线the linea arcuata4、层次：皮肤、浅筋膜、腹直肌前鞘、腹 直肌、后鞘、腹横筋膜、腹膜壁层,腹股沟区 the inguinal region,1、界线：内侧界-腹直肌外侧缘 下界-腹股沟韧带 上界-髂前上棘水平线2、腹股沟管the inguinal canal 男性：精索seprmatic cord 女性：子宫元韧带the round ligament of the uterus通过,腹股沟区the inguinal region,3、腹股沟的两个口和四个壁 腹股沟浅环the superficial inguinal ring 腹股沟深环the deep inguinal ring,上壁-腹内斜肌the obliquus internus abd.、 与腹横肌the transversus abd.的弓状下缘下壁-腹股沟韧带the inguinal ligament前壁-腹外斜肌腱膜the aponeurosis of the obliquus externus abd.和腹内斜肌部分后壁-腹横筋膜the transverse fascia和联合腱,腹股沟斜疝the indirect inguinal hernias,1、精索the spermatic cord输精管the ductus deferens睾丸动脉testicular artery蔓状静脉丛pampeniform plexus of vein淋巴管lymphatic vessels,神经丛nervous plexus鞘韧带ligament of sheath、睾提肌cremaster精索内筋膜internal spermatic fascia精索外筋膜external spermatic fascia,腹股沟斜疝the indirect inguinal hernias（2）,腹股沟斜疝the indirect inguinal hernias,（胚胎早期）脊柱两侧的睾丸-向下移-（3个月）髂窝-（7个月）腹股沟深环-（出生前一个月）腹股沟管-（出生）阴囊1、隐睾
