INTRODUCTION African Minerals Limited African Minerals Limited has a duty to ensure that all African Minerals Limited personnel and contractors on site comply with the requirements of Relevant Statutory Legislation and Relevant Australian Standards Client supplied HSE procedures and guidelines as a minimum standard for the health safety and welfare of all personnel All personnel have responsibilities and duties defined specifically under all statutory legislation in relation to their individual Duty of Care This procedure defines the principles and methods to be used for the isolation of equipment from sources of energy such that work can be safely performed on the equipment It sets out minimum requirements in order to establish a safe working environment for all personnel working on or around equipment or machinery which may be energised whilst testing and pre commissioning activities are in progress In addition to isolation and tagging various types of work may require the issue of permits eg electrical access permit hot work permit confined space entry permit etc Reference shall be made to the African Minerals Limited HSE procedures associated with the issue of permits This procedure only applies to pre commissioning and commissioning phases of the project This African Minerals Limited procedure only applies to the operational plant if site specific client produced isolation procedures are not in place otherwise client supplied procedures shall be used as required This procedure outlines the minimum requirements for isolation and tagging throughout shutdown pre commissioning and commissioning activities The Commissioning Manager may amend this procedure to suit specific site requirements provided the following minimum requirements are satisfied and complied with African Minerals Limited HSE Policy and supporting procedures and Relevant Statutory Legislation and Client Requirements A copy of all amended procedures shall be forwarded to the African Minerals Limited Manager HSE for review and compliance checking 1 TRAINING The African Minerals Limited Project HSE Coordinator and Electrical Supervisor shall be responsible for the training and instruction of personnel in this Safe Work Instruction All personnel shall be required to undergo a written and brief practical competency based assessment on the completion of any such training and instruction All training shall be documented in accordance with African Minerals Limited Ltd HR Manual GRP 05 A24 April 10First Pass Review Rev DateDescription of RevisionCheckedApproved Document No SWI 0418A ISOLATION TAGGING PROCEDURES Project No MH0046 FOR Revision 0 AFRICAN MINERALS LTD COMMISSIONING ACTIVITIES Date 2010 04 24 2 RESPONSIBILITIES A suitably qualified Senior Authorised Person shall assume full responsibility for the High Voltage switching and Isolation within the Plant Generating Facility The African Minerals Limited Electrical Supervisor shall assume full responsibility for the identification and control of all electrical isolations within the process plant No electrical isolation shall be carried out without the authorisation of the African Minerals Limited Electrical Supervisor or delegated representative A Competent Person shall be assigned responsibility for all LV isolations and switching A competent person from African Minerals Limited shall be assigned responsibility for isolations of process lines such as water air and reagent The following persons and responsibilities are defined 2 1Commissioning Manager The Commissioning Manager is responsible for the implementation and compliance with the requirements of this procedure The Commissioning Manager is responsible for ensuring all live pipes and valves have commissioning tags when process slurries solutions air and reagents are identified as a hazard in the Job Safety Analysis JSA The Commissioning Manager and or delegated representative shall be responsible for signing the commissioning tag indicating the mechanical piping instrument and electrical check sheets ITP s are completed and process functional testing can take place 2 2Electrical Engineer The African Minerals Limited Electrical Engineer and or delegated representative place commissioning tags on electrical equipment when it is powered energised for the first time 2 3Project HSE Coordinator The Project HSE Coordinator is responsible for monitoring compliance with this procedure and assisting with training and assessment as required 2 4Senior Authorised Person Prepare ALL high voltage switching isolation procedures and tasks Clear the switching and isolation in conjunction with the respective plant Authorised Person 2 5Electrical Supervisor The African Minerals Limited Electrical Supervisor responsibilities include authorising and training personnel to conduct electrical isolations and de isolations 2 6Authorised Isolation Persons Authorised competent persons shall isolate equipment lock the isolation points and place Isolation Tags prior to the placement of Personal Danger Tags by the work crew A24 April 10First Pass Review Rev DateDescription of Supervisors shall ensure that all personnel involved in working under this procedure are familiar with the requirements and shall be responsible for checking and monitoring compliance 2 8 All Personnel All personnel shall be required to follow this procedure in conjunction with African Minerals Limited Procedure SWI 0418 Tagging and Isolation for all activities during commissioning activities In particular all personnel shall correctly and fully complete and place their own Personal Danger Tags prior to commencing work on any isolated equipment remove their own Personal Danger Tags when the work is complete or prior to leaving the site including at the end of each shift and not remove any other person s Personal Danger Tag follow the accidental tag removal procedure as required 3 TAGGING PROCEDURES These instructions and standard operating procedures shall be applied during and when shutdowns and commissioning is in progress All isolations shall be carried out in accordance with African Minerals Limited Procedure SWI 0418 Tagging and Isolation 3 1INITIAL EQUIPMENT ENERGISATION Prior to energising any equipment for the first time all associated cables and equipment shall be tested and terminated and ITP s Inspection and testing plans provided Once the equipment has been energised for the first time Tagging when all equipment functionality is tested and the tag is signed off by the Commissioning Manager and the equipment is available for general operational use and the equipment is handed over contractually to the Client 3 2 1How do I use a Commissioning Tag The Commissioning Manager or delegate places the Commissioning Tag once all construction activities have been clearly identified as complete and or as agreed with the Construction Manager and Electrical Supervisor Once the Commissioning Tag has been placed it is then deemed under the control of the Commissioning Manager Any modifications or repairs that may be required thereafter shall be authorised by the Commissioning Manager and equipment shall be isolated in accordance with African Minerals Limited SWI 0418 Tagging and Isolations Clearly and legibly complete the tag with the following information Name of the person placing the tag Date the tag was placed and Reasons for placing the tag 3 2 2Isolations Open Valves and Test Points The following are examples of isolation and test points switches and circuit breakers plugs of electrical equipment valves and blanks or spades The following are NOT isolation points Emergency stop buttons Lanyard switches Control TAGGING PROCEDURES Project No MH0046 FOR Revision 0 AFRICAN MINERALS LTD COMMISSIONING ACTIVITIES Date 2010 04 24 Testing Hazards Testing may be more hazardous than normal operation as the equipment under test may be unproven at the time of testing and construction activities may still be taking place Unexpected results may occur such as correct direction of rotation sudden release of pressure and uncontrolled release of energy or movement Two types of testing are considered where isolation must be placed before the testing can be conducted and where isolation must be removed in order to conduct the testing 3 2 3Isolation Prior to Testing For testing where the equipment must be isolated prior to testing eg insulation testing of cables Personal Danger Tags and locks shall be placed on the isolation points by all testing personnel after isolation and placement of Isolation Tags and locks by an authorised competent person All Isolations must be listed on African Minerals Limited Form No SWI F0451 Isolation Testing Check Sheet 3 2 4Removal of Isolations Prior to Testing For testing where isolations must be removed prior to testing eg function or rotation testing of equipment commissioning tags shall be used to indicate that the testing is taking place All isolations indicated on form SWI F0451 Isolation Testing Check Sheet shall be checked verified and signed off by an authorised competent person that the area equipment is now safe to enter operate 3 2 5Isolation and Test Check Sheet The isolation and test points for each item of equipment or plant shall be identified on African Minerals Limited Form No SWI F0451 Isolation Testing Check Sheet The purpose of this form is to clearly identify isolations and all equipment that has been opened or livened for test purposes This form is only used during pre commissioning and test activities 3 2 6Other Precautions Barriers barricades bunting sentries and or signs shall be used to define the boundaries of the area or equipment affected by the test to provide warning and to restrict access to the area All personnel shall be cleared from areas that hydro testing is taking place A24 April 10First Pass Review 7Isolation To achieve positive isolation the following steps shall be followed for every isolation point isolate the energy source prove that the isolation is effective lock off the isolation point by means of a calliper and lock where possible attach an Isolation Tag to the isolation point and ensure all isolation and test points are indicated on Form No SWI F0451 Isolation Testing Check Sheet Personnel must contact an authorised isolator prior to placement of a Personal Danger Tag and lock on any isolation point Isolation Tags and locks are attached to equipment isolation points by authorised competent persons prior to the placement of Personal Danger Tags The Isolation Tag indicates that the isolation point to which it is attached is in a state that prevents the flow of energy An Isolation Tag and lock must be attached to each and every isolation point For areas requiring numerous isolations a group isolation point may be allocated by the authorised Isolator in accordance with African Minerals Limited SWI 0418 Tagging and Isolations procedure 3 2 8De Isolation An Isolation Tag and lock shall not be removed and the equipment de isolated until all Personal Danger Tags have first been removed by the persons who placed them and whose names appear on the tags If the equipment is not safe for return to service the Isolation Tag and lock shall be left in place and an out of service tag shall be attached advising that the work is incomplete Isolation Tags and locks can only be removed by authorised competent persons not necessarily the person who placed it following confirmation that it is safe to do so The isolation point may then be returned to the normal position open valve closed switch etc 3 3Out of Service Tag The Out of Service tag is identified by a yellow background with black print and diagonal striping 3 3 1How do I use the Out of Service Tag For further reference see African Minerals Limited SWI 0418 Tagging and Isolations procedure 3
