成立以站长为组长 安全员为副组长 各加油员为成员的隐患整 改领导小组 负责加油站整改工作的具体开展和落实 二 具体整改内容1 法律法规的建设从网上 书中获取法律法 规的知识标准 加以学习 并做好学习记录 及时更新 如 危险化学品的生产 企业安全生产许可证的实施办法 生产安全事故报告的调查处理的条例等 及时验收学习的成果 2 方针 目标 职责签订目标责任书 建立安全标准化实施方案 文化建设方案 建立安全费用台账 并制定定期考核奖惩记录 3 对风险评价结果及控制措施进行宣传 培训 记录健全的管理档 案 4 对我站的各项安全管理制度加以完善 并定期进行评审修订 5 建立健全职业卫生档案 从业人员健康的监护档案 进行职业危 害检测 6 定期安排安全活动 培训教育 针对活动 培训教育做出评价 7 组织应急救援预案的演练 及时对演练效果做出评价 总结 8 安全设施的整改 配备 1 完成集中密闭卸油点 2 改进输油管线的深埋 3 完成了配电箱外壳的接地 4 取缔卸油使用马凳 为了消除安全隐患 我站在本次的整改工作中 做到了思想认识清 楚 工作态度端正 落实整改措施到位 努力构建和谐 平安的加 油站 中国航空油料榆林民航加油站xx年3月16日篇二加油站隐患整改情况 报告加油站整改情况报告 县安监局我站根据贵局现场检查中 查出的隐患问题 组织相关人员进行了认真研究 并根据贵局提出 的整改建议和的实际情况 进行了认真整改 现将整改落实情况报 告如下 一 安全生产设备设施已整改 1 加油机内底沙不足现已填足 2 新更换的加油机内接地线已按规定重新连接好 3 由于夏 秋两季多雨 消防沙受潮结块 我们已进行了翻晒 并重新添制 保证消防设施充足完好 二 主要设施建设方面的计划加油站消防器材室 罐区围墙受山体 滑坡影响挤压变形 因农民工忙于秋收 我站计划在60天内重新修 建好 generator No 1was pletelysoaked inoil ROOT ANALYSIS During inspection the inspectorfound thatNo 1Generator fuel oil inlet and returnline protectiveinsulation stainedwith discoloredappearance and consideredthat the insulation wasseriously contaminatedby oil While Enginedepartment carryingout No 1Generator fuel oil systemand injectionequipment overhaulon25th Sept improper dismantleof No 1fuel injectingpump causedsmall amountfuel oilin the pipe spatteredover theinsulation crew wipeoff theoil afterjob pletion but someextrude in theinsulationand leaveth etraces on the pipe Owing toinsufficient insulationmaterial remainon board 2 E submitstore partapplication on the dayand scheduleto changeonce receivingin thisport Unfortunately while inspectorspot thedeficiency the storesupplier justdelivered thematerial on board Please refer to StoreApplication andStore Receptionas attached CORRECTIVE ACTION 1 Stained insulationmaterial replacedimmediate afterthe inspection drainpipe b Examine thepipingsystemfor leaks No outerleakage found c Piping systemall bolts nuts andplugs retightenedd Lubricate valvespindles e Check flexibleconnections no leakageand damages f Check O rings forfuel injectionpipe withno leakage The inspectionprocedure carefullyfollowed therequirement ofmanufacturer s plannedmaintenance programand Company s PMS From thenon protective coverkeep closedtill theinspection daybecause No 1generator worksin goodand nomore maintenancework scheduleto beimplemented Subsequent examinationreveals thatoil exudedthroughtheplate betweenthe connectingsocket andpumpbody The aged O ring ofboth fuelinjection pump Item No 158shown agedO ring Please refertoFuel InjectorPump Drawingas attached onthesealing surfacelose itselasticity andthe temperaturedifference causedby FO DO change over prior to harborentering makeit cannotmaintain theseal Small amountof leakageaumulated onoil collectingtray andprotective coverconceal theleakage crew didn t foundand givea propertreatment intime Itwas consideredthat maintenanceprocedures havebeen adequatelyimplemented bythe crewmember inaordance withgenerator maintenancerequirement inPMS however it flapsshould beprotected bycanvas coveror similarmethod crew shouldcheck the protective coverat properinterval to keep off ice up He thinkthe followingrequirement shouldinclude foreffective anti freezing measures 1 Anti freeze greaseshould beprovide inside and outside of canvashood whilecovering toprevent fromclinging together with vents 2 Clearly specifythe intervalsbetween inspectionrather than atproperintervals MT YURU wasdesigned andconstructed basedupon therequirement of CCS Classificationice classB Aording to Part II Ch4 4 1 1 2of CCSRules navigation servicearea ofvessel assigned with CCS Class Notation Ice Class B refersto thesea area of drift ice otherthan largestationary icesuch asfor coastalice conditionin northernpart ofChina inwinter whereas Severe sub Zero conditions aredefined asforecast dailymean ambienttemperatures below 25 Therefore Clarifications onCCS Class Notation Ice Class B of CCStechnical circularas attachedaddress theissue thatthe technicalrequirements ofOCIMF Chap 13 which isbased ongeneral iceadditional notationsare notapplied to the vessel assigned with CCS ClassNotation IceClass B CORRECTIVE ACTION SMO 132 Instruction forNavigation inicy area
