All penalties will be deducted from the progress payment described in the contract between APCI and the contractor. 所有罚款都将在工程进度款中由APCI公司扣除。2. Other safety violations not listed within this documentation shall be governed by APCI and APCICI safety requirements. 其它未尽安全违反事宜均以APCI公司的安全要求为准。3. First time violation of the contractor of the following activities noticed or recognized by APCI Construction Engineer will result in oral warning and immediate correction executed on the spot. Otherwise, the exact violator will be dismissed from the APCI site by APCI Construction Engineer. 承包商第一次违犯下列各项安全要求时, APCI公司现场施工员将对其给予口头警告，并要求立即改正。若有不从，违犯者将被请出APCI公司的施工现场。(1) No APCI appropriate hard hat, steel toed safety shoes, safety glasses with rigid side shield while entering into the APCI site. 未配备合适的个人安全防护用品就要进入APCI公司的施工现场。(2) No working badge while entering into the APCI site.未配带工作卡就要进入APCI公司施工现场。(3) No welding helmet while raising up the welding tool. 即将拿起焊把进行焊接作业却忽略了焊接面罩时。(4) No full-face shield while turning on the cutting machine and grinding tool. 即将启动切割机器或打磨砂轮机时却忽略了全保护面罩时。(5) APCI appropriate gloves not being worn when required. 当有要求却未戴手套时。(6) No safety belt or full-body harness worn while intending to climb up the elevated location over 1.8 M from the ground.没有穿戴好安全带或全保护缓冲安全带就打算登上高于地面1.8米作业时。(7) Contractor personnel has bad attitude towards APC safety requirements. 承包商雇员对APCI公司的安全要求持恶劣态度时。(8) Contractor personnel refuse to have safety indoctrination and safety education APCI Construction Engineer while intending to start the relevant construction activities. 承包商雇员想从事某项施工作业却拒绝接受APCI公司提供的相关安全教育和培训时。(9) No item listed on the Work Permit while contractor personnel intend to start the construction work. 没有在工作许可证上列出而承包商雇员却打算开始施工作业时。(10) No permission in the working area while contractor personnel eating and sleeping. 承包商雇员在工作区域未经许可而吃饭和睡眠时。(11) No working badge while telling a lie to excuse. 没有工作卡却撒谎找借口的。(12) No correction acts while facing unsafe conditions and refusing to stop the starting desire. 面对不安全工作条件不采取改正措施并且不接受停止命令时。(13) Sunglasses being worn where clear glasses should be used. 应使用清晰眼镜时戴上太阳眼镜。(14) Welder not protecting bystanders from welding flash. 焊接工未作好旁观者对焊接火星之防护。(15) Not wearing long sleeve shirts when directed. 未依指导穿上长袖衬衫。(16) Wearing hearing protection in areas designated to be worn.在指定要戴听力防护具之区域未戴上听力防护具。4. The Second Time and more violation of the contractor of the above activities, listed into item 1, noticed or recognized by APCI Construction Engineer will result in immediate penalty: RMB50. The penalty sheet will be signed on the spot and the violator will be dismissed from the APCI site.上面所列各项被发现是第二次违犯时，将导致当场50元人民币的罚款，而且违犯者将被立即请出APCI公司的施工现场。5. All of the following safety violation will result in penalty RMB50. 下列违犯安全的行为将被处以人民币50元的罚款。(1) Personal Attire and Protective Equipment 个人服装与防护装备 Not wearing hard hats while working (including under a welding hood).作业时未戴安全帽（包括在抽气防护罩之下）。 Safety glasses with side shields not being worn while working. 作业时未戴有侧边防护之安全镜。 Not using a Matrix face shield when using a grinder/band saw/ electric wood saw, cutter, etc. 当使用研磨机/带锯/电动木锯/切割机等时未戴基本脸部之防护面罩。 The wearing of respirators when procedures dictate to be worn. 当作业程序规定要戴有呼吸防护具却未戴。 Not wearing steel toe and steel mid-sole safety shoes/boots or rubber boots. 未穿有脚趾保护钢片及钢片夹层鞋底之安全鞋/长统靴或橡胶材质之长统靴。 (2) Personal Conduct 个人行为 Horseplay or fighting. 恶作剧或打架。 Alcohol found or being used at site. 在施工现场喝酒。 Urinating in areas other than Port-O-Lets/toilets/rest rooms. 在厕所以外地区小便6. The following safety violation acts will result in penalty RMB100 .下列违犯安全的行为将被处以人民币100元的罚款。(1) Motor Vehicles and Construction Equipment 机动车辆 Proper speed limit not being used by car/truck or other vehicles. 汽车/卡车或其它车辆未使用正确的速度限制。 Improper speed limit of construction equipment.施工设备不正确的速度限制。(2) Fire Protection and Prevention 火灾防护与预防 Motor not shut off during fuelling. 加油时引擎未熄火。 No fire extinguisher allocated at the welding, grinding area. 焊接和打磨区域未配置灭火器。(3) Welding, Cutting, and Grinding 焊接、切割与研磨 Separate grounding cable from welding/cutting machine not being correctly installed. 由焊接/切割机器所分离出来的接地电缆未予接地 No APCI appropriate distance between gas cylinders 钢气瓶之间的放置距离不合适 Valves not in place when not in use or in transit.当不使用或运送时，转阀之防护盖未在适当的位置。 Not wearing safety glasses under welding hoods.在焊接抽气罩之下未戴安全眼镜。 Not wearing hard hats under welding hoods.在焊接抽气罩之下未戴安全帽。 Appropriate gloves not being used for work being performed.作业执行时未使用适当的手套。 No long-sleeved shirts. 未穿长袖衫。(4) Excavation, Trenching, and Shoring 开凿、挖掘与支撑l Worker working in trench that is not shored or sloped as required (4:1 slope). 劳工进行挖掘作业未依规定加设支撑或使用坡度 (坡度4:1)。l Worker is in trench where the ladder is not accessible within 25 feet of employees. 进行开挖之劳工在距离其位置25英呎之范围内未设置供其攀爬之梯子l Hand probing was not performed for underground utilities that were identified to be in the area prior to digging. 在挖掘之前未对已确认存在于区域内之地下公共设施以手进行探索。l Workers working in excavation when equipment in use next to edge (operator and workers in trench to be dismissed). 当使用之设备靠近开挖区边缘时劳工仍在挖掘区内作业（开挖沟渠内之操作人员与劳工应离开）。l Barricades not being used that are (42 high and 4 back from edge) and of adequate strength and worker is in trench. 未使用适当尺寸（42英吋高、距离边缘4英呎）与适当强度之路路障而劳工已在开挖沟渠中。l Appropriate permit not being displayed for all excavations. 对于所有开挖作业未显示适当的工作许可。l Adequate crossover walkways with handrails (i.e., pit board with handrails) not being used to bridge over trenches/excavations and workers are using. 未使用适当的配置护栏之跨越信道（例如有护栏的凹形平台）以跨越挖掘之沟渠，且劳工又正在使用此跨越信道。(5) Elevated Work and Fall Protection 高架作业与坠落防护a. Ladders 梯子l Ladder being used that has broken rungs or stringers. 使用之梯子梯级或纵梁有损坏。l Ladder being used that is not properly constructed and not properly being used. 所使用之梯子未正确加以组装与使用。l Ladder being used that is not on a proper base. 使用之梯子未置于适当的基础面l Rise/run of ladder is less than 4:1.梯子的高程/水平延伸坡度小于4:1。l All ladders over 15 off ground do not have vertical lifelines installed (100% tie off) and ladder is being used 所有高出地面15英呎之梯子未设置垂直安全母索（100固定）且已经加以使用。b. Scaffolding 脚手架l No inspection of the scaffolds by APCI Construction Engineer prior to using by the contractor. 搭建好的脚手架未经APCI公司现场施工员检查就开始使用。l Scaffold incorrectly anchored (tie back no greater than every 20 feet), and scaffold is being used脚手架不正确之固定（每20英呎之内应加锚定）且已被加以使用。l Hand rail, mid rail, toe boards, and mesh between mid rail and toe board not complete and workers are using the scaffold 脚手架之上栏杆、中栏杆、脚趾板、及中栏杆与脚趾板间之防护网未设置完成而劳工已进行使用。l Proper flooring is not provided and workers are using scaffold 未铺设适当的踏板而劳工已进行使用。l Scaffold inspection/tagging program is not being used 未使用脚手架检查/上挂签计画。l All ladders over 15 off ground do not have vertical lifelines installed (100% tie off) and ladder is being used所有高出地面15英呎之梯子未设置垂直安全母索（100固定）且已经加以使用。c. Safety Harness 安全带l Safety Harness not being utilized for elevated work. 高架作业时未使用安全带。l Dual shock absorbing lanyards are not being used and not secured correctly 未使用双重吸收冲击能量之收紧绳且未正确固定。l Proper tie off connection and of adequate strength is not being implemented (5400 lbs). 未提供适当的固定接合器与适当的接合强度（5400磅）。l Worker not tied off such that he will not fall more than 6ft. This includes portable hand tools and extension cords (double insulated tools are not allowed). 劳工使用中的设备未接地，包括携带式手工具及延伸之线路（不允许双重绝缘之工具）。 Ground fault circuit interrupters or assured equipment grounding is not being done, and worker is using item. (This applies to electrical wiring cords and equipment). 劳工使用之设备未接漏电断路器或采用经确认之设备接地方式（适用于电气金属线路与设备）。 Temporary wiring has not been inspected and there are damaged parts of the cord and the worker is using it. (i.e., electrical cords cannot be repaired or taped up at all, and welding leads cannot be taped up within 10 ft from the end of the lead). 临时性的线路未加以检查与线路有损害之部分而劳工都加以使用（电气线路都不可以修复或以胶带缠绕，焊接的线路在线路末端10英呎范围内不得以胶带缠绕）。(8) Housekeeping and Hygiene 场区整理与卫生 Debris found in the work area. 作业区域发现有垃圾。 Nails found exposed or on ground. 地面上有钉子或钉子外露。 Roll-up cords, hoses, and leads not done on a weekly basis. 未每周进行卷起绳索、软管与线路之工作。 Tools not picked up daily. 没有每天收拾工具。 Walk areas not free of debris, appropriately graded, and have adequate surface. 步行区域未清理垃圾、未设置适当坡度与未铺设适当的地表面。 Disordered storage of construction tools and materials. 施工机具和材料混乱堆放(9) Contractor hasnt gotten safety orientation training but working at the APCI site. 承包允许未经安全培训的员工进入APCI施工现场。7. The following safety violation will result in penalty RMB1000.下列各项违犯安全的行为将被处以人民币1000元的罚款。(1) No Work Permit issued by APCI while the contractor starts construction work at site. 承包商没有获得APCI公司签发的工作许可证就开始现场施工活动。(2) Contractor changed site Safety Supervisor without APCI approval. 承包商更换现场安全主管，未经APCI公司同意。(3) Contractor would not like to submit the Safety Plan to APCI after contract is signed, before moving to the APCI site. 合同签定后进入APCI公司施工现场之前，承包商不愿意提交安全施工计划的。(4) Contractor doesnt arm their personnel with APCI required PPE. 承包商不愿意按照APCI公司要求给雇
