6、rein migrantpatientswithhypertension.KEY WORDS hypertension ； migrant population；management改革开放三十多年来，我国经济持续快速发展，上海的流动人口数量逐年增加，大批外来务工人员涌入上海。外来务工人员在为城市发展做出贡献的同时，他们的健康却没得到社会的重视1-2。 2013年，据中国疾病控制中心（CDC）报道，我国15 岁及以上人群高血压患病率为24%，全国共有2.66 亿人患高血压， 每 5 个成人中至少有1 人患高血压 1 。高血压和脑卒中等心血管病已经成为我国突出的公共卫生问题，本文通过调查，评价本社区外来高血压人群的知信行水平，以探索有针对性的慢性病管理方式。1 对象与方法1.1对象选取 2014 年 11 月 2015 年 1 月在豫园街道社区卫生服务中心就诊及居委咨询的3080 岁外来人口高血压患者160例，其中男 72 例，女 88 例；年龄4000 元 15例；有高血压家族史 96 例，无家族史 64 例。1.2方法采用中心自行设计的问卷进行调查，内容包括外来高血压患者的一般情况（年龄、文化、经济收入、医疗费用支出方式）、健康行为（烟酒、饮食、运动）和高血压管理情况
