1、无明新科技大学旅馆事业管理系建教合作合约书明新科技大学旅馆事业管理系建教合作合约书明新科技大学 (以下简称甲方)承(以下简称乙方)之协助,合作培育旅馆人力资本,提供学生校外实习机会训练专业技能,从而使学生成为旅馆服务及管理能力的人才,为明确双方之权利义务关系,双方承诺共同遵守下列各项条款:一、甲、乙双方组成实习辅导小组,由明新科技大学旅馆事业管理系实习委员会代表暨乙方代表共同组成,实习辅导小组任务如下:1.召开协调会,解决学生实习问题或讨论双方合作事宜,以利甲方教学与乙方营运的配合。2.实习期间甲方分发学生至乙方各单位实习, 实习学生除接受甲方实习委员会管理外,亦接受乙方实习单位主管之指挥监督

2、,遵守公司政策及工作规则。3.实习期间双方派专人负责指导与考核,藉以督促学生校外实习的能力训练。4.乙方负责监督学生实习训练与生活辅导,如遇情节重大情事,得由双方开会议定提前终止实习,并通知家长协同处理。5.其它有关建教合作校外实习事项。二、实习期间:自民国年月日起至年月日止。三、甲方之职责:1.协助乙方遴选分发实习学生。2.协助乙方实习训练内容或课程,访视辅导实习学生,提供学生考核资料。3.通知实习学生接受乙方半年单位轮调安排。4.负责约束选派之实习学生,切实遵守乙方所安排实习单位工作及作息规定。四、乙方之职责:1.提供甲方实习学生员额与专业训练课程, 唯不使学生担任非相关及危险性工作。2.

3、安排甲方实习学生半年单位或职务轮调, 并提前通知学生与甲方实习委员会访视老师,但实习学生于柜台、订房、与餐厅内场三项职务得免除轮调。3.非约定之学生实习单位异动,须征求本系实习委员会同意。4.实习期间负责实习学生工作时间内之生活管理与实习成绩之考核。5.实习期间给予实习学生每名每月新台币元正之实习津贴,并依劳动基准法及其它相关法令为实习学生办理劳保、健保。6.实习期间应给予实习学生比照一般职员之休假与福利制度。7.其它有关实习事项,依劳动基准法与乙方人事规定办理。五、本合约如有未尽事宜或变更事项,由双方协调修订之。六、本合约自双方签约日起实施。无立约单位甲方:学校名称:明新科技大学代 表 人:

5、ogy (MUST)(hereinafter calledCollege). Both parties hereby agreed to cooperate in the education and training ofhospitality students believing that this co-operation would benefit all parties.It is mutually agreed as follows:1.Sponsor will provide the attachment opportunity & training facilities to p

6、ractical forthe students from the College. List of EWM Industry Training Partners are asfollows:1)The Oriental Hotel, 2) Royal Plaza on Scotts, 3) Le Meridien Singapore, 4) TheRitz-Carlton Millenia, Singapore, 5) The Traders Hotel, 6) Plaza Hotel Parkroyal, 7)Negara on Claymore, 8) Swissotel The Sta

7、mford, 9) Raffles The Plaza.2.The attachment of the students is subject to Sponsor obtaining legal permits for thestudents.3.Period of training attachment is 12 months as agreed between Sponsor & College.4.College undertakes that every applicant is a bona fide registered student of theCollege and sh

8、all provide for each student the following documentation forapplying the legal permit:a) Letter of Information in accordance with Sponsors format.b) Latest Academic Record or Certificate.c) Photocopy of passport.d) Original pre-deployment medical fitness certificate.5.Sponsor will provide the studen

9、ts with the following conditions during attachment:a) Minimum MonthlyAllowance of S$600 per month.b) Free standard accommodation, duty meals, uniform & uniform laundry.c) Medical consultation by companys doctor for work related illness or injuries.Personal medical is students own cost.d) Supervision

10、 and guidance of each student during the training.e) On satisfactory completion, issue a testimonial and certificate to the students.6.The College shall be responsible for the following:a) Recommending selected students based on performance achievement indexand fluent English.b) Repatriate any stude

11、nts who commit during the attachment period anycriminal offence, seriousmisconduct or violates the rules & regulations ofSponsor or Employer or if unfit for training.c) College undertakes to impose severe punitive discipline on student terminatedand repatriated for College shall provide the Sponsor,

12、 without any demand,written information of any disciplinary action.无d) Recover from student & reimburse Sponsor the $5000 immigration SecurityGuarantee, if forfeited by Singapore Government, for any trainee whocommitted and convicted of Immigration offence.e) Inform the students their responsibiliti

13、es during the internship period and theirown expenses for hospitalization & personal insurance and airfares to/fromSingapore.7.The College and Sponsor shall not be responsible for any misconduct, accident,injury or death of anystudent during attachment.8.The Sponsor may terminate the training and re

14、patriate any student at the studentsexpense for any ofthe following reason:a) Commit any Work Permit offences or involved in any conduct or activitiesincompatible or improper with this Training Program.b) Not complying with the rules and regulations of Sponsor, employer or the lawsof Singapore.c) Ce

15、rtified medically unfit by Sponsors appointed doctor or is incapacitated bysickness or diseases.d) Poor performance or conduct.9.The duration of this Agreement is valid for a period of one and a half (1.5) yearsupon signing. During this period, the College undertakes to send students forInternship o

16、r training attachment in Singapore to the Sponsor only and shall notsend any students directly or indirectly to any other party, employer orestablishment in Singapore without the written agreement of the Sponsor.10. The employment of the Students and this Agreement shall be governed solely bythe law

18、签约之建教合作实习代表EWM Consultancy PteLtd 安排第三学年 校外实习 课程之海外专业实习, 为规范 EWM Consultancy PteLtd 与签署人间之权益,签署人与实习生同意下列条款并遵守履行之。1.实习单位部门:EWM Consultancy Pte Ltd 安排之新加坡国际级观光饭店2.月薪:600 新加坡币3.每周工作时数:48 小时4.实习时间:12 个月5.实习绩效保证金:实习生应于抵达新加坡后缴交$500 元新加坡币给 EWMConsultancy Pte Ltd 作为实习绩效保证金,实习生完成一年实习后将全额退回;若未完成一年实习,此实习绩效保证金将

19、作为 EWM Consultancy Pte Ltd名誉受损与实习安排经费之赔偿。6.实习生合约完成之绩效奖金$100 新加坡币每月, 绩效奖金于实习完成后一次发给;未完成 12 个月实习合约,无法领取绩效奖金。7.实习期间提供免费宿舍,每间房间住四位实习生。实习生须负责平均分摊每月水费、电费、瓦斯费及宿舍受损修护等费用;工作时由实习单位提供免费餐食。8.若未获机关补助,则实习生自行负担新加坡来回机票费与每日所需之交通费。9.任何一方若决定终止实习关系, 须于一个月前以书面提出或支付一个月的工作薪资;在实习工作期间与工作之后若有行为不当、表现不佳、触犯法律、违反规定或与顾客交往,实习单位有权终

20、止实习合作关系且无须提前告知并遣送回国,由实习生自行负责回国交通费。10. 上班工作时间完全由实习单位安排,实习生可以在同一个工作单位或不同部门工作。除了实习单位外,实习生不得在新加坡其它单位工作或打工,亦不得吸毒、酗酒、从事不法、不道德、不诚实、犯罪或政治活动等违反新加坡法律之行为。11.除了 EWM Consultancy Pte Ltd 安排的宿舍外,实习生不可以在外过夜,也不允许带任何来宾、朋友、亲戚于宿舍过夜;另外不得于宿舍内吸烟、吸毒、喝酒;实习生应尊重宿舍室友的个人隐私、健康、环境及舒适,并维护宿舍内的清洁与卫生。12. 实习生须自行负责个人疾病与看诊费用,因个人健康因素离开工作

21、期间不予支付实习薪资。实习单位负责实习生因工作而引起之相关医疗看诊与住院费用,医疗福利须完全依照实习单位之规定办理。13. 实习生于新加坡实习期间, 由实习单位支付移民安全保证金5000 新加坡币予新加坡政府,若实习生因任何犯罪而丧失实习机会,须完全赔偿实习单位无或 EWM Consultancy Pte Ltd 之移民安全保证金5000 新加坡币。另外,实习生护照将由实习单位统一保管以免遗失。14. 实习生保证完全遵守新加坡法律以及实习单位与 EWM Consultancy Pte Ltd之相关规定。15. 实习生须承诺遵守 EWM Consultancy Pte Ltd 的相关规定。EWM

22、 ConsultancyPte Ltd、实习单位或同事等不负责个人的受伤、死亡、所有的花费、遗失、赔偿与支出。16. 前述 15 条款依新加坡法律管辖执行之。除了上开建教合作实习代表约定事项外, 实习生于实习期间愿意配合学校督导, 遵守贵校各项实习规章及生活条约,服从贵系实习委员会及实习单位之指导训练,为规范学校与签署人间之权益,签署人同意下列条款并遵守履行之。1.实习生于校外实习期间,仍为学校学生,优良或不良表现依明新科技大学学生奖惩办法与旅馆事业管理系校外实习学生奖惩办法办理。2.实习生前往实习机构实习,依实习单位请假规定及劳动法办理,若实习单位无明确规定则依学校请假规定办理。 海外实习生

23、应完成一年海外旅行平安险(附约疾病医疗住院险)事宜,并缴交保险单据复印件予贵系后方赴海外实习。3.实习生于校外实习期间,未经告知无故离职、工作懈怠或连续犯错等重大违规事件,经贵系实习委员会派员调查,会同实习单位确认,经本会通过移送本校学务处生活辅导组签请学生奖惩委员会处理,视情节得处以下列处分:(1)勒令退学。(2)记过处分,经贵系实习委员会同意并由签署人具结后,始得继续实习。(3)勒令休学,其已实习时数不予计算。签署人实习生姓名:签章:身份证字号:护照号码:生日:家长姓名:签章:身份证字号:手机号码:住址:中华民国年月日无SPONSORSHIPAGREEMENT HOTEL12 MONTHS

Contract Completion Bonus is SIN$100/- per month or prorate payable o

26、n completion of full contract periodonly.Incomplete contract is not eligible for Completion Bonus.7. Dormitory accommodation will be provided free-of-charge. Each room is shared by 4 persons. Interns shallbe liable equally for the monthly payment of water, electricity, gas, and damages at the accomm

27、odation. Dutymeal is provided free of charge by attachment employer.8. Daily transport and 2 ways air ticket to/from Singapore shall be Interns own cost.9. Either Party may terminate this Sponsorship Agreement by giving one (1) months notice in writing orpayment of 1 months allowance in lieu. The em

28、ployer is entitled to terminate the Sponsorship Agreementwithout advance notice and repatriate the Intern at Interns own cost in case of misconduct, poor performance,breaches laws, regulations or fraternization with guests or companys clients during or after work.10.Working schedule is strictly acco

29、rding to employers schedules. Intern may be attached to same section ordepartment for the complete 12 months at the Employers sole discretion. It is illegal for Intern to work withany other employer whilst in Singapore and illegal to take part in any drugs, alcohol, illegal, immoral,dishonest, crimi

30、nal or political activities while in Singapore.11.Intern shall not stay at any accommodation other than the accommodation provided by the employer or EWMand not allowed to bring any visitors, friends or relatives to visit or stay at the accommodation. Smoking,drugs, and alcohol consumption is prohib

31、ited in the accommodation.Intern shall respect the privacy, health,environment and comfort of others living in the accommodation. Intern is required to undertake cleaning andmaintaining the accommodation in clean & hygienic condition at all times.12.Intern shall be responsible for own illness and ou

32、tpatient medical treatment. Training Allowance is notpayable during medical leave. The employer is responsible for work related medical treatment andhospitalization only if it is caused directly from the employment with them. Medical benefits shall be solely inaccordance with employers policy.13.Intern shall fully reimburse the employer or EWM the Immigration Security Bond of S
