Theresultsrevealwideningtensionsacrossthehealthcaresystem:consumerexpectationsarerising

fasterthanexperiencecankeepup,cliniciansarereadytowalkawayfromoverburdenedsystems,andartificialintelligence(AI)capabilitiesareoutpacingorganisationalreadiness.

OurdatareflectsarealityalreadyunderwayandsurfacescriticalchoicestohelpCEOsprioritiseandexecutetheirstrategyforthenext18months.

Persistentpainpoints

Thefrictionconsumersandcliniciansareexperiencingisanexpressionofadeeper,structuralimbalancebetweensurgingdemandandconstrainedsupply.

TheAsia-Pacificregionishometo60%oftheworld’spopulationandbearsadisproportionateshareoftheglobaldiseaseburden,yetitaccountsforonly22%ofglobalhealthcarespending.Healthworkerdensity

remainscriticallylow:Onapopulation-weightedbasis,emergingAsia-Pacificcountriesaverageroughly1.6doctorsper1,000people.ExcludingChina,thisaveragedropsnear0.9doctorsper1,000people.Forreference,theWorldHealthOrganizationrecommendsaminimumof2.5physiciansper1,000people,whilemanyOrganisationforEconomicCo-operationandDevelopmentnationsaverage3.7.

Longwaittimesremainthetopconsumerfrustration,consistentwithourthreepreviousreports

(seeFigure1).Thisissuecutsacrossmatureandemergingsystems,privatelyandpubliclyfundedalike,andmirrorspatternsseenintheUSandUK.

Costandcomplexitycompoundtheproblem.Highout-of-pocketcostsoftendeterordelaycare,evenamonginsuredpopulations,whilebillingandcoveragecomplexityaddfrictionanderodetrust.The

consequencesaretangible:Fewerthan70%ofchronicpatientsreporthavingregularcheckups,citinghighcostastheirprimarybarrier.

Doctorsarefeelingthestraintoo,particularlyinpublicsystems,wheretheyreportedsignificantlylowerjobadvocacycomparedtophysiciansinprivatehospitals.Inbothmature(Australia)andemerging(thePhilippines)markets,doctorsranked“professionaldevelopment”and“technologyandtools”astheir

mostvaluedprofessionaldimensions—aheadofcompensation.Yetonlyabout30%ofdoctorsreportedfeelingverysatisfiedwitheitherdimension.

Ourlongitudinal,cross-regionresearchonthefrontlineofhealthcaresuggestsastrongcorrelation

betweenphysicianinvolvementinstrategicdecision-makingandemployeeNetPromoterScores(eNPS),whichmeasuretheextenttowhichclinicianswouldrecommendtheirorganisationasaplacetoworktoafriendorcolleague.Asia-PacificdoctorswhofeelengagedreportedeNPSupto36pointshigherthan

thosewhodonot.

Approximately30%ofdoctorsbelieverecruitmentandretentionattheirorganisationshavebecomemoredifficultsince2023,and20%saythey’reactivelyconsideringswitchingorganisations.Thesesentiments

Severalforcesarereshapinghowcareissoughtanddeliveredacrosstheregion,includingconsumerism,expandedcaresites,fragmentation,andAI.Thesetrendsarealsochangingwhatittakestosecurea

competitiveadvantage.

Consumerism

Asia-Pacificpatientsarebecomingconsumers.Eighty-fourpercentexpecthealthcaretobemore

convenienttodaythantwoyearsago,and71%expectdoctorstobemoreresponsiveviaphone,WhatsApp,

oremailvs.waitingforanextappointment(seeFigure3).ThisshiftisbeingamplifiedbytherapidglobaladoptionofAI-basedtools.Nearly70%oftheAsia-PacificconsumerswesurveyedreportedusingAItoolstobetterunderstandamedicaldiagnosisortreatmentplan.

Preventivecareincreaseddecisivelycomparedtopriorsurveys.Sixtypercentofconsumersreported

schedulingregularcheckupsandscreeningsin2025,comparedtoonly47%in2023.EverycountryexceptIndiaandIndonesiareportedasteepincreaseinpreventivecare;Chinaledtheregion,with76%of

consumersschedulingregularcheck-ups(seeFigure4).

Consumersalsoreallocatedpersonalbudgetstowardhealthandwellness.Netspendingincreasedacrosseveryconsumerhealthcategoryovertheprior12months.Nutritionsupplementssawthebiggestjump(43%),followedbyfitnessandexercise(34%)andoralhealthcare(31%).

Careeverywhere

In2019,roughly50%ofpatientssupportedmovingnonemergencycaretoalternativesites.Thatintenthastranslatedintoaction.Today,nearly60%ofconsumersreceivecareinalternativesettingssuchaswalk-inclinicsandpatienthomes,andviaphonesandwearables.

Uptakevariesbymarket:Walk-inclinicsinretailorpharmacysettingsarethetopalternativecaresitesinMalaysia,Australia,andthePhilippines;walk-inandafter-hoursclinicsleadinIndonesiaandHongKong;homebasedcareleadsinIndiaandVietnam;andtelehealthdominatesinChinaandSingapore(seeFigure5).

Thisshifthasstrongphysiciansupport.Acrossspecialties,surgeonssaidtheywouldideallyperform

30%–42%ofproceduresinambulatorysurgicalcentres(ASCs);only15%–25%areperformedinambulatorysettingstoday(seeFigure6).Eveninmedicaloncology—wherehospital-basedcareremainsdominant—cliniciansindicatedaclearintenttomovecareintolower-acuityenvironments.

Clinicianscited“patientpreference”astheprimarydriverforshiftingcaretoanASC.TheyalsoperceiveASCsasmorewillingtoinvestincutting-edgeequipmentandbelievetheshiftwouldincreasethevolumeofcaredeliveredoverall.

TelehealthadoptiondoubledacrosstheAsia-Pacificregionbetween2019and2021,thenmoderatedeverywhereexceptIndiaandChina.

InIndia,telehealthadoptionplummetedbelowits2019baseline,droppingto10%penetrationin2025.Telehealthcontinuestobeusedfornon-acutecases.However,becausethemarketremainslargelycashpay,payer-drivensteeragetowardtele-consultationshasbeenlimited.

Chinawasanoutlierontheupside.There,telehealthadoptionreached61%in2025,rising37percentagepointsfrom2019(seeFigure7).

Amajorityofdoctors(65%)andconsumers(70%)agreeonanimportantboundary:telehealthisa

complementto,notasubstitutefor,in-personcare.Evenwithstrongdigitaladoption,roughly76%ofconsumersinChinaholdthisview.

Fragmentation

Asthenumberofchannels,providers,andtouchpointsmultiply,fragmentationisbecominganincreasingchallenge.

Formanypatients,thecarejourneyisaseriesofdisconnectedepisodesplaguedbyrepeatedinformationrequests,duplicateddiagnostics,andchallengingnavigation.Halfoftheconsumerswesurveyedwere

senttomultipleprovidersandlocationsbeforereceivingtherightdiagnosisortreatment,andmorethan40%receivedinconsistentadviceacrossclinicians.It’sevenworseforpatientswithchronicconditions;55%ofthoseconsumersreportedhavingtoseemultipledoctorstofulfiltheirhealthcareneeds.

Coordinationgapsalsoburdenclinicians.Inbothmatureandemergingmarkets,aboutoneinthree

doctorshasobservedsignificantwasteandinefficiencyintheirorganisations.Excessiveformsand

paperworkwereachiefcomplaint,androughly40%saidtheyperformlow-value,repetitivetasksthat

couldbeautomatedorsimplified.Physiciansalsoreportedspendingasubstantialamountoftimewaitingforadministratorstocompletenecessarytasks(seeFigure8).

Theconsumerswesurveyedwereclearaboutwhattheywant:95%preferasingletouchpointformanagingtheirhealthcare,upfrom70%in2019.ExcludingChina,approximately60%to75%ofrespondentspreferin-personinteractionastheirprimarycoordinationpoint.

Morethan80%ofconsumersbelieveaprimarycarephysicianshouldserveasthefirstpointofcontact—maintainingpatients’longitudinalhealthhistory,guidingprevention,andcoordinatingreferrals.However,accesstoprimarycareisfarfromuniversal.Aboutaquarterofconsumersintheregionlackaprimary

carephysician,withMalaysia(39%),HongKong(38%),Indonesia(33%),andChina(27%)reportingthelargestgaps.

However,patientsareincreasinglyopentoworkingwithalternativecarecoordinators.Consumertrusthaserodedacrosshealthcarestakeholdersoverall,althoughnon-traditionalprovidershavemovedupintheconsumertrustrankings.Indonesiasawthebiggestlift:Onlyonestakeholdertype—hospitals—wastrustedby65%ormoreIndonesianconsumersin2019,comparedtofivetoday.

Traditionalproviders(GPs/clinicsandhospitals)remainthemosttrustedentitiesacrosstheregion,followedbygovernments,healthinsurersandpharmacies.However,stakeholdertrusthasshifteddifferentlyacrossmarkets(seeFigure9):

•Traditionalproviders’relativetrustrankingdeclinedinChina,HongKong,India,Indonesia,Singapore,andVietnam.

•Healthinsurersandhealthmanagementorganisations’trustincreasedinAustralia,Malaysia,thePhilippines,andVietnam.

•DigitalhealthcompaniesbecamethethirdmosttrustedstakeholderinChina,closelytrailing

hospitalsandthegovernment—despitelosingrelativestandinginmostotherAsia-Pacificmarkets.

AI-enabledcare

AI-enabledcareisgaininggroundacrosstheAsia-Pacificregionbutwithanimportantcaveat:consumerswanttechnologytomakehealthcaremoreaccessible,responsive,andreliable—notlesshuman.

ConsumeracceptanceofAIisgenerallyhigherintheAsia-PacificregionthanintheUS,withnearly

three-quartersofAsia-PacificconsumersreportingtheyfeelcomfortablewithatleastoneAI-enabled

healthcareapplication.Consumersaremostcomfortablewithusecasesthatsupportcliniciansand

improveefficiency,suchasAI-assisteddocumentation,clinicaldecisionsupport,andautomatedanalysisofmedicalexamsandtestresults.

SupportisstrongerforAIadoptionthatenhanceshumaninteractionsratherthanreplacingthem.Still,

morethan35%ofAsia-PacificconsumersreportedfeelingcomfortablewithAI-onlycallcentres,medicaladvice,diagnoses,andtreatmentplans(seeFigure10).

In-personappointmentsremainthepreferredchannelwhencaringforaminorillnessornon-acute

symptomlikeacold,sorethroat,orrash(seeFigure11).Consumersareleastlikelytopreferchatbot-leddiagnosesortreatment.

ApproximatelyoneinthreedoctorssaidtheirorganisationsarenotpreparedtodeployAIatscale,heldbackby

unclearstrategies,limitedtraining,andinsufficientclinicianinvolvement.

Doctorsentimentcloselymirrorspatients’.PhysiciansreportedoptimismaboutintegratingAIinto

healthcare,hopingitwillreducetheiradministrativeburdenandworkload,whileremainingconcerneditcouldunderminethevalueofthepatient-clinicianrelationship(seeFigure12).Theseviewswere

consistentacrosstheUS,theUK,andtheAsia-Pacificregion.

WhileconsumersandcliniciansarebecomingmorecomfortablewithAI-enabledcare,theirhealthcareorganisationsaren’tkeepingpace.ApproximatelyoneinthreedoctorssaidtheirorganisationsarenotpreparedtodeployAIatscale,heldbackbyunclearstrategies,limitedtraining,andinsufficient

clinicianinvolvement.

Thisreadinessgapiscompoundedbybroaderdigitalmaturityconstraints.Beyondtelehealth,

foundationaltechnologyadoptionremainslimitedacrosstheregion.EveninAustralia,theadoptionofcaremanagementsystems,workforceandworkflowmanagementtools,andrevenuecyclemanagementsolutionsremainslow(seeFigure13).ApluralityofAustraliandoctorsexpressedinterestinusingthesetechnologiesbutnotedthatprogressisstalledbyalackoffundingandresources.
