科朗设备（苏州）有限公司Crown Equipment (Suzhou) Co., Ltd 制定/Author: Fran Fu 批准/Approval: 危险作业管理程序 /Hazardous operation management procedure1. 目的/Purpose规范厂区内危险作业申请操作流程与作业规范。In order to enhance the management of hazardous operation, avoid happening accidents, protect people in safety, we formulate this regulation.2. 范围/Scope适用于全公司范围内的危险作业安全管理，包括动火作业、高处作业。This procedure ruled all hazardous operations including hot work; work at height;3. 定义/Definitions3.1 动火作业：进行焊接与切割作业及在易燃易爆场所使用喷灯、电钻、砂轮等进行可能产生火焰、火花和赤热表面的临时性作业；Hot work: Any work involving welding, cutting, grinding or other similar operation producing sparks.3.2 高处作业：利用脚手架、升降平台、便携式木梯和金属梯等登高工具从事超过基准面2米以上，存在坠落风险的作业；Work at height:4. 程序/Procedure序号NO.危险作业内容Hazardous operation4.1动火作业 Hot work4.1.1职责 Responsibility4.1.1.1申请人的职责（必须由我司项目负责人担当）：1.动火作业前负责动火作业申请，并遵守“一天一证一动火点”为原则；2.对外来施工单位的资格合法性审查，提醒外来施工单位带齐动火所需要的相关证件、防护装备；以备我司检查；3.对动火作业现场进行确认，确保现场符合动火要求；4.对动火人员的劳动防护、动火设备的安全状态负责；5.对动火人员进行培训，确保熟悉动火作业要求、消防器材使用方法和应急处理流程（必要时，联系EHS人员助指导）；6.每天上、下午各不少于一次不定时对动火现场进行巡视，发现异常立即予以纠正；7.所有供应商外服人员、外来施工人员等进行的焊接、气割、烘烤等明火作业均需要提前申请；Responsibility of applicant (The applicant should be the project leader):1. A hot work permit needs filled out before hot work is performed EXCEPT if it is normal production operation. For example welding area. One permit is valid for one job site and one day.2. The fire operator must be trained specially, and get the license, the project leader is responsible for checking out the license of fire operator, informing constructor to carrying the license and wear proper personal protective equipment. 3. Check out fire-proof measure including condition of site, preventive measures to make sure its safe to perform hot work.4. Check out fire operators PPE and tools.5. Train fire operators about safety requirements for hot work, and how to use fire facilities and evacuation.(If necessary, contact EHS)6. Check out the job site at least one time irregular in morning and afternoon. Shut down and make corrective action if there are any unsafe conditions or actions regarding cutting, welding and/ or heating operations.7. A hot work permit is required prior to start any hot work.4.1.1.2审核人的职责（动火区域主管，及EHS部门担当）：1.对动火作业现场进行确认，确保现场符合动火要求；Supervisor of job site and EHS department sign and approve the permit:1. Go to job site and check out the fire-proof measures before approving the operation, ensure that its safe to perform hot work.4.1.1.3动火人职责1.查验动火作业申请单审批情况，无动火作业申请单、申请单上相关流程未审批完毕的应拒绝进行动火作业；2.逐项确认相关安全措施的落实情况；3.确认动火地点和时间，是否和动火作业申请单一致；4.发现不具备安全条件时不得进行动火作业；5.随身携带动火作业申请单，以备检查；6.所有金属焊接、气割作业人员必须有“金属焊接切割作业操作证”，并熟悉动火要求及消防器材使用方法；7.动火人员必须佩戴焊工专用绝缘手套、绝缘鞋及其它防护用品；8.在动火作业完成后，清理现场，清除残火，确认无遗留火种后方可离开现场。Responsibility of fire operator:1. Check out hot work got approval. If there is no hot work permit, fire operator has right to refuse fire operation.2. Check out fire-proof measure including condition of site, preventive measures to make sure its safe to perform hot work.3. Check out job site is according with hot work permit.4. Shut down fire operation if there are any unsafe conditions.5. Post up hot work permit in job site6. Fire operator must be trained specially and get the license.7. Wear proper personal protective equipment such as gloves, safety shoes and face shield.8. Clean the job site; make sure no fire in job site when finishing fire operation.4.1.1.4监火人职责1.坚守岗位，不准脱岗若暂时离开需找适合人员替岗，并尽快返回；动火期间，不准兼做其它工作； 2.负责现场动火前的准备工作并监护与检查，发现异常情况应立即通知动火人停止动火作业，及时联系有关人员采取措施； 3.当发现动火人违章作业时应立即制止；4.在动火作业完成后，应会同有关人员清理现场，清除残火，确认无遗留火种后方可离开现场。5. 监火人需在动火作业完成1小时及3小时内，返回现场检查确认无遗留火种。Responsibility of fire watcher:1. Fire watcher must constantly roam the work point that he is assigned for, aware for potential hazards and/or changing work conditions2. Fire watch has the authority and responsibility to shut down the hot work when he is aware of any unsafe conditions or actions regarding cutting, welding and/ or heating operations and take corrective action 3. When there are some illegal operations, shut down hot work.4. Clean the job site together with fire operator and make sure there is no fire risk5. The fire watcher shall be presented for at 1 hour and 3 hours after hot work is done.4.1.2动火作业安全要求 Safety requirement for hot work:4.1.2.11.所有动火申请由本公司项目负责人负责，以“一天一证一动火点”为原则，负责现场安全检查和应急处理，熟悉消防器材使用方法；2.动火人、监火人都必须熟悉动火要求，并承担现场监督、防火管理责任。3.五级以上风力或雨雪天气，禁止室外动火作业；4.所有金属焊接、切割作业人员必须有“金属焊接切割作业操作证”，为防止触电事故的发生，动火人员必须佩戴焊工专用绝缘手套、绝缘鞋及其它防护用品，包括外服人员和外来施工人员，并保证焊机绝缘层无破损；5.动火作业前必须将周围易燃易爆物品移至安全位置。一般情况下，在施工区10m范围内，无可燃物，易燃液体，易燃气体，可燃粉尘等均清理干净。如仍不能满足安全要求，若半径10m范围内的易燃、易爆品不能撤离，用防火布或湿木板隔离 ；（特别说明：靠近叉车电瓶区域作业时，必须将电瓶拆除或者切断电瓶电源，并进行充分通风，保证作业区域内无燃爆气体后，方可动火，防止氢气爆炸）6.动火期间距离动火区域10米内不得排放各类可燃气体，可燃液体；在动火点10米范围内及用火点下方不得同时进行可燃溶剂清洗、喷漆或抛丸等作业。7.所有气瓶严禁靠近火源、热源和电气设备,与明火距离不少于5米；氧气瓶和乙炔气瓶距离不少于5米。乙炔瓶禁止使用其它气瓶进行替代，并保证有止回火阀。所有气瓶必须无破损，压力正常；8.动火现场配置两只以上灭火器，必要时将消防水带铺设到作业现场。无火警时禁止使用，否则照价赔偿，使用完成，及时放回指定位置；9.动火结束后，动火人员和监火人须共同进行确认，保证现场无火警后，方可离开作业现场。动火作业申请单应交回EHS留存。1. The project leader will fill out hot work permit prior to start hot work; Check out fire-proof measure including condition of site, preventive measures to make sure its safe to perform hot work; knowing how to deal with emergency is basic requirement.2. Fire watcher and operator must be familiar with safety requirement for hot work.3. Hot work in open air is forbidden when there is bad weather such as rainstorm, and heavy wind above five grades that influence the safety of work.4. Fire operator must be trained specially, and gets the license. They should wear proper personal protective equipment to ensure safety operation. Any damaged or frayed cord in welding machine should remove from service. The constructors are required to follow this requirement.5. Exposed flammable/combustible materials and equipment with a 10m radium must remove or be protected by fire-proof covers or fire blankets before hot work begins.(Remark: Shut off the power and keep good ventilation while fire operation being done near charging area, it will prevent gas explosion)6. All combustible dust ,vapor, gas of liquids must be kept 10m away or properly covered with fire-proof curtains, fire blankets. It means weld/cut within 10m was forbidden while a paint booth or shot blaster was running.7. Oxygen cylinder should keep at least 5m away from acetylene cylinder. Meanwhile gas cylinder should keep 5m away from ignition source and in good condition.8. Two or more fire extinguishers must be kept in job site.9. Clean the job site when finished fire operation and make sure there is no fire risk. The fire watcher shall be presented for at 1 hour and 3 hours after hot work is done. EHS department will archive hot work permit.4.2高空作业 Work at height4.1.1职责 Responsibility4.1.1.1申请人的职责（必须由我司项目负责人担当）：1.高处作业前负责高处作业申请，并遵守“一天一证一作业点”的原则；2. 对外来施工单位的资格合法性审查，提醒外来施工单位带齐高处作业所需要的相关证件、防护装备、签订施工商厂内作业安全协议；以备我司检查；3.对高处作业人员及监护人员进行培训，确保熟悉高处作业要求；4.每天上、下午不少于一次对作业现场进行巡视；5.要求作业方作业结束后，将现场整理整顿。Responsibility of applicant (The applicant should be the project leader):1. A work at height permit needs filled out before work is performed. One permit is valid for one job site and one day.2. The operator must be trained specially, and get the license, the project leader is responsible for checking out the license of the operator, informing constructor to carrying the license and wear proper personal protective equipment. 3. Train operators about safety requirements for work at height.(If necessary, contact EHS)4. Check out the job site at least one time irregular in morning and afternoon. Shut down and make corrective action if there are any unsafe conditions.5. Clean job site when finished to ensure job site orderly and clean.4.2.1.2会签人的职责（使用部门主管担当）：1.对作业现场进行确认，确保现场符合高处作业要求；2.协调并避免施工与生产之间的交叉作业；3.确认高处作业下方3米范围内无其他作业。Supervisor of job site is responsible to:1. Check out the precautions before approving the operation; ensure that its safe to perform work.2. Coordinate to prevent daily work conflict with work at height3. Check out there is no work with a 3 meters radium.4.2.1.3审核人的职责（安全部门担当）； 1.高处作业前对作业现场进行确认，确保现场符合作业要求；2.检查作业人员高处作业申请单手续申请是否齐全、作业人员防护是否配齐；施工方针对所有高处作业的管理要求是否了解；3.审核人员每天对施工现场上下午各不少于1次巡查，发现异常立即予以纠正，并记录。Responsibility of auditor (facility-EHS department):1. Check out the condition of job site, preventive measures to make sure its safe to perform work at height2. Check out operators wearing proper personal protective equipment, and constructors understanding safety requirement for work at height totally.3. Conduct spot check at least one time, if there is any unsafe condition, shut down the operation and take corrective actions.4.2.1.4作业人员的职责1.针对身体状况负责，有高处作业禁忌的症状（高血压、心脏病、贫血、癫痫等）视力不佳、疲劳过度等不适宜进行高处作业的情况，严禁高处作业；2.查验高处作业申请单的审批情况，无高处作业申请单、申请单上相关流程未审批完毕的应禁止作业；3.掌握高处作业的安全要求，并逐项确认相关安全措施的落实情况；4.配齐防护设备，对于危险性过高且无法进行安全保障的应禁止作业；5.高于5米的高处作业（3个脚手架高度）必须持证上岗；6.确认高处作业地点和时间，是否和高处作业申请单一致。Responsibility of operators:1. A regular physical for operators engaged in work at height is required: people who has occupational contraindication (such as hypertension, cardiopathy, anemia, epilepsy, and mental illness), oldness, weakness, overtired, poor eyesight or other improper people are not allowed to engage in work at height.2. Check out the work has got approved. Operator has right to refuse perform the work if there is no work permit. 3. Understand safety requirement for work at height, and check out preventive measures one by one according to work permit.4. Wear proper personal protective equipment, operator has right to refuse the work with high risk.5. If operator is working above 5m, he shall receive special technology training, pass relevant test and gain authorization.6. Check out the job site and work time is according with work permit.4.2.1.5监护人员的职责（可以为施工方的人员）1.高处作业监护人必须全程监护，坚守岗位；对使用靠墙梯进行高处作业的，监护人员必须扶住梯梁2.监护高处作业人员正确履行各项安全职责、落实各项安全措施；3.随身携带高处作业申请单，以备检查。Responsibility of watch master:1. Watch master must constantly roam the work point that he is assigned for. When using ladders or stepladders, watch master must assist to hold it.4.2.3作业安全要求 Safety requirement4.2.3.1针对所有高处作业的管理要求 Safety management requirement1.高处作业者无职业禁忌的症（高血压、心脏病、贫血、癫痫等）视力不佳、疲劳过度等不适宜进行高处作业；2.高于5米的高处作业（3个脚手架高度）必须持证上岗； 3.高处作业必须佩戴好安全帽、安全带；安全帽必须扣紧下颚带，锁紧后定锁；安全带必须系在作业处上方的牢固构件上或专为挂安全带的钢架或钢丝绳上，不得系挂在移动或不牢固的物件上；不得系挂在尖锐棱角部位，不得底挂高用；4.高处作业监护人必须全程监护，坚守岗位；对使用靠墙梯进行高处作业的，监护人员必须扶住梯梁；5.高处作业所使用的工具、材料、零件必须装入工具袋；6.高处作业时，工具、材料、零件严禁随意乱置或向下丢弃，使用完毕后，必须及时放入工具袋中；7.雨天、雪天、6级以上强风、浓雾等恶劣气候，严禁室外登高作业；8.夜间登高作业，必须有足够的照明；Safety requirement:1. A regular physical for operators engaged in work at height is required: people who has occupational contraindication (such as hypertension, cardiopathy, anemia, epilepsy, and mental illness), oldness, weakness, overtired, poor eyesight or other improper people are not allowed to engage in work at height.2. If operator is working above 5m, he shall receive special technology training, pass relevant test and gain authorization.3. Operator must wear proper personal protective equipment such as safety helmet and safety harness. An example is putting on a safety harness correctly and connecting it, via an energy-absorbing lanyard, to a suitable anchor point.4.
