Purpose:To response for emergency incident/accident identified in the SCOPE, and provide a procedure for fire fighting. Meanwhile, under the earthquake/explosion, the evacuation will be started as same as the fire disaster. 2. Scope:This procedure is apply for the following situation:ItemEmergency Case1Fire2Explosion3Earthquake3. Refence:N/A4. Procedure:This plan will be implemented when our company threatened by conflagration or explosion / earthquake. All actions taken will be started in the plan and under the direction of the on-duty supervisor/loss prevention engineer.4.1 Program of ERT set up:4.1.1 The ERTs organization: The ERT constitute one coordinator as the director of EHS Dept. The ERT also constitute four group leaders, who come from MFG Dept. The ERT members mainly come from MFG Dept, HR Dept and FE/EHS Dept. Their names and telephone numbers are list in the attachments of this plan. The ERT is divided into 4 groups according to 4 shift (A, B, C, D)4.1.2 The ERT training includes orientation training when employee was first time assigned to be the ERT member and annual refresh training. If someone cant attend the annually refresh training for some special job reasons as approved by the coordinator he/she must attend at least one training per year for familiarization. For example, FE and security guard may not attend the training due to special job reasons, but he must get approval from coordinator in advance. However, he should strict to the regulation that one refreshing training shall be received in a year.4.1.3 The ERTs responsible for attack the incipient fire and assist the first aid group.4.2 Emergency Response ProcedureThe Emergence Response will be started by two ways: The fire discoverer gives an alarm by pushing manual alarm. The fire discoverer gives an alarm by calling fire line 6333.4.2.1 By fire line 63334.2.1.1 When anyone notice a fire happening, he/she must inform fire control center immediately at extend number: 6333, and also any management at the scene.4.2.1.2 The on-duty supervisor/loss prevention engineer who receives the information should: Evaluate the situation. Inform ERT coordinator and Group leader promptly by telephone or pager. Inform the security officer using the PA system to repeat following call list at twice:“Attention please, there is a fire in (location of fire/Emergency), please stop your work and stand by to evacuation.” Stay in fire control center for deal with the telephone call and acknowledge the fire alarm.4.2.1.3 The coordinator of ERT should: Arrive the scene and direct the action of ERT members to attack the fire by extinguisher. will evaluate the emergency situation and coordinate the activities of the various support groups. is responsible for determining if and when a partial or complete evacuation of the plant is necessary. When the fire situation has been kept under control, the coordinator of ERT will give the order to security officer using PA system to repeat following instruction twice: “Attention please, the fire has been kept under control, please continue with your duties.” In the event the situation becomes uncontrollable, the coordinator must inform the local Fire Brigade by himself or appoint loss prevention engineer to call the public fire line 119 and ensure the Fire Brigade has been informed and known the location of our company. After determined the completed evacuation, the coordinator will command the security officer using the PA system to repeat following call list twice: “ Attention please, this is emergency evacuation, please stop your work and leave your work area immediately.” is responsible for coordinate and keep contact with local fire brigade.4.2.1.4 After determined the total evacuation, the security officer shall: Inform the relevant outside contractor. Exclude and prohibit all outside vehicle and people enter. Preserve the evacuation in good order.4.2.2 By manual alarmThe manual alarm is prohibited to touch in normal condition and is allowed only in urgent situation exactly.4.2.2.1 After fire discover give an alarm by manual alarm, the local ERT members who heard the alarm must arrive the locale immediately and attack the fire by extinguishers or fire hose.4.2.2.2 The on-duty supervisor/loss prevention engineer who received the information should: Evaluated the situation. Acknowledge the fire alarm on fire control panel. Inform the coordinator and group leader by telephone and pager promptly. The follow step is same as forementioned.4.2.2.3 The coordinator of ERT received the information should: The step will be same as 4.2.1.3.4.2.2.4 The security officer should: The step will be same as 4.2.1.3 & 4.2.1.4.When the situation becomes uncontrolled and have to make the important decision, the GM/or on-duty supervisor who has gotten GM/ERT directors permit will be the final commander in the important matter. 4.3 Emergency Evacuation procedure:This procedure will be implemented under the conflagration/explosion/earthquake.4.3.1 Anyone who get the information of emergency evacuation include outside contractor will: Stop all work and shut down all the machine and equipment power or gas supplying. Leave the work area and get out of the building along the designed the Emergency Evacuation Map from the nearest exit rapid. Arrive the pointed place to assemble so as to check the amount the number. Follow the direction and instruction of ERT coordinator and ERT member. 4.3.2 The employees on Emergency evacuation will not: Run Scream or make unnecessary noise. Stop to get material from locker room Stop to visit toilets Return to the work area for valuables. Spread any rumors.4.3.3 Action and Responsibilities during Evacuation:4.3.3.1 Coordinator of ERT: Shall be the director in all matters during the emergency. Evaluate the situation and coordinate the various groups. Keep contact with outside authority. Account number of arriving employees totally and direct ERT members to rescue the employee who is still in peril. After the danger has been eliminated, the coordinator shall give the order to all employees to return their work place and continue their work.4.3.3.2 FE staff: Take the emergency tools to them. Cut off the main electric power. Leave one Mechanical engineer in fire fighting pump room and supervise the fire fighting pumps running situation. 4.3.3.3 The directors of Department: Supervise own department evacuation method. Arrive the pointed place and join own departments group. In charge of account the number of arriving employees and fill in the EVACUATION NAME LIST to hand in coordinator of ERT.4.3.3.4 Security officer and loss prevention Engineer: Ensure that total evacuated and no person enter or re-enter the plant. Provide manpower as available to direct and control traffic.4.3.3.5 Emergency Response team: Assemble at the fire scene under the direction of loss prevention engineer /on duty supervisor. Attack the fire by portable extinguisher and fire hose under the direction of loss prevention engineer in the incipient stage fire only. Leave the fire scene when the fire situation become uncontrolled or after the local fire brigade arrived in the locale. Follow the direction of coordinator to complete other mission.4.3.3.6 Doctor and nurse & first aid team: Arrive scene when heard fire alarm. Lead first aid team to perform to injured person. Keep contact with local first aid center or ambulance.4.4 Investigating:4.4.1 After fire/explosion, the correct action must be taken by the relative dept.4.4.2 The fire /explosion site must be remained until the specialist conducted the investigation and take the evidence/photos.4.4.3 Loss prevention engineer and production supervisor will investigate the fire/explosion cause and give the correct action plan to the relative dept and report it to relevant manager.4.4.4 After the investigation the fire/explosion site must be cleaned at once.4.4.5 Loss prevention engineer will review the result of CA taken and determine to take the further action.4.5 Evacuation Drill:4.5.1 The emergency evacuation drill will be hold at least semi-annually. Each shift will attend one times evacuation drill semi-annually.4.5.2 Evacuation Drill procedure: Give order for the evacuation starting at designed time. Coordinator Record the time when the emergency evacuation is at Loss Prevention beginning. Engineer Using the PA system to inform all employees to go away from their work place immediately. Security officer. The coordinator and ERT members get to scene promptly. ERT Shut down the machine and equipment power or gas OperatorSupply. Set up the first aid center to rescue the maimed person. Doctor/Nurse Check the attendance of department evacuees at the Director of each Assembly Area.
