Aisles and exits are also clear of obstructions1-2Proper disposal containers are available 1-3Heavy items are stored on lower shelves1-4Experiments are conducted on stable non- moving surfaces1-5Chemicals not is use are stored in cabinets not on open benches1-6Sinks and Sink Traps look clean/free of chemical residue1-7Mercury thermometers are not in use1-8Unattended experiments are accompanied by signs & status2PERSONAL PROTECTIVE EQUIPMENT/PRACTICES2.1Proper personal protective equipment is available for employees and visitors and in use/proper sizes2.2Laboratory workers are dressed properly2.3Personal are aware of MSDS2.4Proper fire extinguishers are within 50 feet of laboratory.2.5First aid kit is available and maintained3MANAGEMENT REQUIREMENTS - Organization3.1Is the laboratory or the organization legally responsible?1) Qualified Lab Management personnel2) Trained lab technicians 3) Clear reporting lines3.2Provide adequate supervision of testing and calibration staff, including trainees, by:1) Persons familiar with methods and procedures?2) Purpose of each test? 3) Assessment of the test results?4) proper documentation of results3.3Is technical management available?1) Are all technical issues relating to testing activities fully covered by the technical management?4DOCUMENT MANAGEMENT4.1Does the laboratory establish and maintain procedures to control all documents that form part of how the lab is managed?1) Method Statements for test2) Clearly defined forms and checklist3) Test reports4.2Are the management system documents generated by the laboratory uniquely identified? Do such identification include:1) Date of issue and/or revision identification? 2) Page numbering?3) The total number of pages or a mark to signify the end of the document?4) The issuing authority (is)?5EQUIPMENT5.1Are all the appropriate equipment and its software (including that outside the laboratorys permanent control) required for all testing and/or calibration activities:1) Available and functioning properly?2) Achieving the accuracy required?3) Comply with specifications relevant to the test and/or calibrations concerned?4) Are calibration programmes established? (where key quantities or values of the instruments have a significant effect on the results)5) Calibrated and checked before being placed in service?5.2Is there a monitoring system in operation to alert the laboratory staff on the due dates of calibration, verification and maintenance for all items of equipment?5.3Is the laboratory able to show that the calibration reports are reviewed after calibration of equipment?5.4Are the following records of each item of equipment and its software maintained :1) Identity of the item of equipment and its software?2) Manufacturers name, type identity, and serial number or other unique identification?3) Check of equipment for compliance with specification4) Manufacturers instructions, if available, or reference to their location?5) Maintenance plan, where appropriate, and maintenance carried out to date?6) Any damage, malfunction, modification or repair to the equipment?5.5Are equipment giving suspected results or defective or outside specified limits due to overloading or mishandling:1) Taken out of service?2) Isolated?3) Clearly labeled?4) Marked as being out of service?5.6When equipment goes outside the direct control of the laboratory, does the laboratory ensure that the function and calibration status of the equipment are checked and shown to be satisfactory before the equipment is returned to service?6SAMPLING6.1Are a sampling plan and procedures for sampling available6.2Does the sampling process address factors that need to be controlled in order to ensure validity of the test and calibration results?6.3Does the laboratory have procedures for recording relevant data and operations relating to sampling that forms part of the testing or calibration that is undertaken?
