經口服而由胃黏膜吸收 發生中毒 中毒作用機轉 有機磷中毒作用機轉 主要與acetylcholinesterase AchE 產生 可逆結合 抑制其生 作用 因此acetylcholine無法被水解 引起 經過 激而麻痺 氨基甲酸鹽同樣抑制AchE 但其結合為可逆性 人體以acetylcholine為 經傳導物質的構造有 1 副交感 經節後 經元 2 骨骼肌肌肉 經交接處 3 交感及副交感 經節前 經元 4 中樞 經系統 體內有 種AchE 在紅血球上為truecholinesterase 在血清中為pseudocholinesterase 皆可與有機磷結合 床 況之毒性作用 急性中毒的徵候可分成 1 作用 muscariniceffects 包括呼吸系統 氣管痙攣 分 增加 肺水腫 腸胃系統 嘔吐 腹痛 腹瀉 心血管系統 心搏過慢 血壓下 外分 腺 汗 口水 系統 頻 失禁 及眼睛 縮瞳 視 模糊 2 菸鹼樣作用 nicotiniceffects 包括心血管系統 心 過快 血壓上升 和橫紋肌 肌肉顫動 抽搐 麻痺 3 中樞 經性毒性 意 混 失眠 焦燥 安 幻覺 癲癇 呼吸及循環中樞抑制 床 況 SLUDGE S 流口水SalivationL 流眼淚LacrimationU 排尿UrinationD 排便DefecationG 胃部症狀GastricE 嘔吐Emesis 致毒劑 隨農藥種 成分 吸收途徑及個人感受性有關 如巴 松口服致死 約100mg 診斷 綜合下 三項 1 床徵候表現 2 血清中AchE 目減少 3 對於atropine治 有反應 床 據收集 1 一般性 據 無特 性 有時可 短暫高血 或蛋白 2 特殊性物質血中濃 床上常用血清pseudocholinesterase 目減少 診斷 但會受其他因素影響 紅血球cholinesterase活性測定較有具特 性 3 放射線檢查 CXR以評估肺部併發症 4 EKG 心搏過慢或心 整 QTc延長可作為預候 好指標 依 床 況進 之支持性 法 1 呼吸道 airway 保護呼吸道 抽吸分 物 意 清或呼吸衰竭需插氣管內管 2 呼吸 breathing 在治 上應給予氧氣 即給予解毒劑 抽測氣体動脈血 ABG 有必要時則應插氣管內管並使用呼吸器支持 3 循環 Circulation 給予atropine並使用心電圖監視器偵測心 整 可發生torsadesdespointes 4 中樞 經系統 CNS 痙攣可用Valium或Phenobarbital 禁止使用Succinylcholine或Morphine 非特 性 法 特 性 法解毒劑 atropine及pralidoxime PAM 1 解毒劑之劑 及使用方法 1 atropine 0 02 0 05mg kgIVq10
