3、l be filled from this panel.There are immediate vacancies for Clinical Nurse Specialist Palliative Care in the following sites: HSE DNE: Specialist Palliative Care Services Louth Palliative Care Services, Dochas Centre, Drogheda: The Louth Community Care Area (which includes South Monaghan including
4、 Carrickmacross). The Clinical Nurse Specialist shall review patients as required in all settings in the Louth (1 x 1.0 WTE- full time post) HSE South: Waterford Hospice Homecare (1 x 0.8 WTE- 80% part time post)Details of ServiceDochas Centre Specialist Palliative Care HSE Dublin North East is gove
5、rned through a Directorate Model and provides Specialist Palliative Care by means of three Consultant led interdisciplinary teams to the people of Louth (including South Monaghan), Meath and Cavan/Monaghan.The service has developed in line with the NACPC Integrated Programme of Specialist Palliative
6、 Care and provides outpatient, and specialist palliative consultant services in the acute hospitals and community.The regional resource of education, research, support and advice for all palliative care activities is resourced from the Dochas Centre, Our Lady of Lourdes Hospital, Drogheda.The nursin
7、g structure comprises of a Director of Nursing, Practice Development Coordinator, Team Managers, CNS and CNM1.Waterford Hospice HomecareWaterford Hospice Movement Ltd, works with the Health Service Executive in providing a community based Specialist Palliative Care service in the Waterford City and
8、County geographical area. The service is Consultant led.This post is based in Waterford Hospice Home Care and is part of the Waterford Specialist Palliative Care Service providing a 7 day week service, 9am 5pm.Reporting RelationshipDochas CentreOperationally to The Team Manager CNM 3 Palliative Care

12、a.Principal Duties and Responsibilities Clinical FocusThe Clinical Nurse Specialist Palliative Care will: Articulate and demonstrate the concept of nursing and midwifery specialist practice within the framework of relevant legislation, the Scope of Nursing & Midwifery Practice Framework (ABS) 2000,
13、the Code of Professional conduct (ABA 2000) Perform nursing assessments and plan and initiate care and treatment modalities within agreed interdisciplinary protocols to achieve patient/client centred outcomes: evaluates their effectiveness. Attend and contribute to weekly Multidisciplinary meetings
14、Contribute information and support on all aspects of care to patients and their families/carers Identify health promotion priorities in the area of specialist practice Implement health promotion strategies for patient/client groups in accordance with the health agendaPatient AdvocateThe Clinical Nur
15、se Specialist Palliative Care will: Enable patients/clients, families and communities to participate in decisions about their health needs Articulate and represent patient/client interests in collaboration with the multidisciplinary team Act as a resource contact person for patients, families/carers
16、 ensuring that instructions are regarding investigation and follow up are clearly understood. Implement changes in healthcare service in response to patient/client need and service demandEducation and TrainingThe Clinical Nurse Specialist Palliative Care will: Promote a progressive learning environm
17、ent based on high quality, research based nursing care Provide mentorship, preceptorship, teaching, facilitation and professional supervisory skills for nurses and other healthcare workers. Educate patients/clients, families and communities in relation to their healthcare needs in the specialist are
18、a of practice Identify own continuing professional development needs and engage accordinglyAudit and ResearchThe Clinical Nurse Specialist Palliative Care will: Keep up to date with current research and practice Identify, critically analyse, disseminate and integrate nursing and other evidence into
19、the area of specialist practice Initiate and participate in evaluations and audits Use the outcomes of audit to improve service provision Contribute to service planning and budgetary processed through use of audit data and specialist knowledgeConsultantThe Clinical Nurse Specialist Palliative Care w

22、h statutory obligations, in conjunction with relevant staff e.g. health and safety procedures. Observe, report and take appropriate action on any matter which may be detrimental to staff and or service user care or well being may be inhibiting the efficient provision of care. Assist in observing and
23、 ensuring implementation and adherence to established policies and procedures e.g. health and safety, infection control. Ensure completion of incident/near miss forms, clinical risk reporting Adhere to department policies in relation to the care and safety of any equipment supplied for the fulfilmen
24、t of duty. ManagementThe Clinical Nurse Specialist Palliative Care will: Provide an efficient, effective, and high quality service, respecting the needs of each patient/client Effectively manage time and caseload in order to meet the needs of a changing and developing service Be aware of budgetary i

28、te to the development of the post while in office.Eligibility CriteriaQualifications and/ or experienceCandidates must, on the latest date for receipt of completed application forms for the post possess: Be registered in the General Division of Register of Nurses & Midwives maintained by Bord Altran
29、ais agus Cnimhseachais na hireann (Nursing and Midwifery Board of Ireland) or be entitled to be so registered.and Have at least 5 years post registration experience of which 2 must be in the specialty or related area (Palliative Care)and Have the ability to practice safely and effectively fulfilling
30、 his/her professional responsibility within his / her scope of practiceand Must demonstrate evidence of continuing professional developmentand Must have a level 8 (or higher) post-registration National Qualifications Authority of Ireland major academic award relevant to the specialist area of Pallia
31、tive Care. AgeAge restrictions shall only apply to a candidate where he/she is not classified as a new entrant (within the meaning of the Public Service Superannuation Act, 2004). 50、on of interest” stage.Appointment as an employee of the Health Service Executive is governed by the Health Act 2004 and the Public Service Management (Recruitment and Appointment) Act 2004.Remuneration The Salary Scale (as at 01/01/10) for the post is:Euro 47,089 47,886 48,559 49,659 50,874 52,067 5
51、3,260 54,604 55,852 (pro rata)Working WeekThe standard working week applying to the post is 39 (pro rata)HSE Circular 003-2009 “Matching Working Patterns to Service Needs (Extended Working Day / Week Arrangements); Framework for Implementation of Clause 30.4 of Towards 2016” applies. Under the terms
52、 of this circular, all new entrants and staff appointed to promotional posts from Dec 16th 2008 will be required to work agreed roster / on call arrangements as advised by their line manager.
