Obstet Gynecol Surv,2004, 59 (2): 118-127.       【摘要】背景有关腹主动脉瘤血管内修复与开放修复对照研究报道很少。方法本研究将英国37家医院19992004年1 252例大腹主动脉瘤(直径5·5 cm)患者随机分组:血管内修复组和开放修复组,每组626例患者。跟踪至2009年末,观察患者的死亡、移植相关并发症、再干预发生率和资源使用情况,采用逻辑回归和COX回归法比较两组结果。结果30 d手术死亡率:血管内修复组为1·8%,开放修复组为4·3%;与开放修复组比较,血管内修复组校正优势比为0·39, 95%CI (0·18, 0·87),P=0·02。关于动脉瘤相关死亡率,血管内修复组有早期获益,但研究末期获益丢失,部分原因为致命性腔内移植物破裂校正危害比=0·92, 95% CI (0·57, 1·49),P=0·73。跟踪末期,两组全因死亡率比较差
