摘要:本研究旨在探讨中药通督活血汤对老年脊髓型颈椎病术后残余麻木的临床疗效以及可能的作用机制。选取2019年1月至2020年12月于本医院手术并术后仍存在颈部麻木症状的58例老年脊髓型颈椎病患者为研究对象,随机分为治疗组和对照组,对照组接受对症处理,治疗组在对症处理的基础上口服通督活血汤。结果显示,治疗组总有效率为89.7%,明显高于对照组的70.7%(P<0.05)。同时,治疗组中颈部感觉恢复时间显著短于对照组(P<0.01),术后疼痛缓解时间也显著短于对照组(P<0.05)。病例对照研究发现,通督活血汤能够有效提高手术切口处血液灌注量,降低术后组织水肿程度,增加颈部组织代谢,促进局部循环及氧气供应,进而促进术后神经再生。总之,通督活血汤可作为老年脊髓型颈椎病患者术后残余麻木的有效治疗方法。

Abstract:ThisstudyaimedtoinvestigatetheclinicalefficacyandpossiblemechanismsofChinesemedicineTongduHuoxueSoupforpostoperativeresidualnumbnessinelderlypatientswithcervicalspondyloticmyelopathy.Fifty-eightelderlypatientswithcervicalspondyloticmyelopathywhounderwentsurgeryatourhospitalbetweenJanuary2019andDecember2020andstillhadnecknumbnesssymptomsaftersurgerywereselectedastheresearchsubjectsandrandomlydividedintotreatmentgroupandcontrolgroup.Thecontrolgroupreceivedsymptomatictreatment,andthetreatmentgroupwasgivenTongduHuoxueSouporallyonthebasisofsymptomatictreatment.Theresultsshowedthatthetotaleffectiverateofthetreatmentgroupwas89.7%,significantlyhigherthanthatofthecontrolgroup's70.7%(P<0.05).Atthesametime,therecoverytimeofnecksensationinthetreatmentgroupwassignificantlyshorterthanthatinthecontrolgroup(P<0.01),andthetimeofpainreliefaftersurgerywasalsosignificantlyshorterthanthatinthecontrolgroup(P<0.05).Acase-controlstudyfoundthatTongduHuoxueSoupcouldeffectivelyincreasethebloodperfusionofthesurgicalincision,reducethedegreeoftissueswellingaftersurgery,increasethetissuemetabolismoftheneck,promotelocalcirculationandoxygensupply,andtherebypromotepostoperativenerveregeneration.Insummary,TongduHuoxueSoupcanbeusedasaneffectivetreatmentforpostoperativeresidualnumbnessinelderlypatientswithcervicalspondyloticmyelopathy.
