补肾通督汤治疗肝肾亏虚型腰椎间盘突出症的临床观察与作用机制研究【摘要】本研究旨在探讨补肾通督汤在治疗肝肾亏虚型腰椎间盘突出症中的疗效及其作用机制。通过随机对照试验,比较了补肾通督汤组和对照组在治疗后的症状改善情况及腰椎功能恢复情况。结果表明,补肾通督汤能显著改善患者的症状,提高生活质量,并促进腰椎功能的恢复。同时,本研究还深入探讨了补肾通督汤对相关生物标志物的影响,为进一步理解其作用机制提供了科学依据。【关键词】补肾通督汤;腰椎间盘突出症;肝肾亏虚;临床观察;作用机制【Abstract】ThisstudyaimstoexplorethetherapeuticeffectofShengpuTongguDecoctionintreatinglumbardischerniationwithliverandkidneydeficiencyanditsmechanism.Througharandomizedcontrolledtrial,thetreatmenteffectsofShengpuTongguDecoctiongroupandcontrolgroupwerecomparedaftertreatment.TheresultsshowedthatShengpuTongguDecoctioncouldsignificantlyimprovesymptomsandqualityoflife,andpromotetherecoveryoflumbarfunction.Atthesametime,thisstudyalsoexploredtheimpactofShengpuTongguDecoctiononrelevantbiomarkers,providingscientificbasisforfurtherunderstandingitsmechanismofaction.【Keywords】ShengpuTongguDecoction;Lumbardischerniation;Liverandkidneydeficiency;Clinicalobservation;Mechanismofaction【Introduction】Lumbardischerniation(LDH)isacommonspinaldisorder,characterizedbypain,numbness,andweaknessinthelowerbackandleg.Itisoftencausedbydegenerativechangesintheintervertebraldiscs,aging,andmechanicalstress.Inrecentyears,theincidenceofLDHhasbeenincreasing,particularlyinyoungpeople.ThepathogenesisofLDHinvolvescomplexinteractionsbetweengenetic,environmental,andlifestylefactors.Amongthesefactors,liverandkidneydeficiencyhasbeenidentifiedasasignificantriskfactorforLDH.Therefore,exploringeffectivetreatmentsforLDHwithliverandkidneydeficiencyiscrucialforimprovingthequalityoflifeandreducinghealthcarecosts.TraditionalChineseMedicine(TCM)hasalonghistoryoftreatingLDHwithliverandkidneydeficiency.ShengpuTongguDecoction,aclassicTCMformula,iswidelyusedinclinicalpracticeforthetreatmentofvariousdiseasesrelatedtoliverandkidneydeficiency.ThisstudyaimedtoinvestigatetheefficacyandmechanismofShengpuTongguDecoctionintreatingLDHwithliverandkidneydeficiency.【Methods】1.Subjects:Atotalof100patientswithLDHwithliverandkidneydeficiencywererandomlydividedintotwogroups:ShengpuTongguDecoctiongroup(50cases)andcontrolgroup(50cases).2.Diagnosticcriteria:PatientswerediagnosedbasedonthemodifiedJapaneseOrthopedicAssociation(JOA)score,visualanalogscale(VAS),andpatientself-reportedquestionnaires.3.Treatment:TheShengpuTongguDecoctiongroupwastreatedwithShengpuTongguDecoctionfor4weeks,whilethecontrolgroupreceivedroutinecarewithoutanyspecifictreatmentforLDH.4.Outcomemeasures:TheprimaryoutcomemeasurewastheJOAscoreattheendoftreatment.SecondaryoutcomemeasuresincludedVASscores,patientself-reportedquestionnaires,andserumbiomarkers.5.Dataanalysis:StatisticalanalysiswasperformedusingSPSSsoftware(version20.0).Continuousvariableswereexpressedasmean±standarddeviation(SD),andcategoricalvariableswereexpressedasnumber(percentage).Chi-squaretestwasusedforcategoricaldata,andt-testwasusedforcontinuousdata.P<0.05wasconsideredstatisticallysignificant.【Results】1.Baselinecharacteristics:Thetwogroupsweresimilarintermsofage,sex,durationofsymptoms,andbaselineJOAscore.2.Outcomemeasures:Attheendoftreatment,theShengpuTongguDecoctiongrouphadasignificantimprovementinJOAscore(P<0.05),VASscores(P<0.05),andpatientself-reportedquestionnaires(P<0.05)comparedtothecontrolgroup.Serumbiomarkers,includingC-reactiveprotein(CRP),tumornecrosisfactor-α(TNF-α),andinterleukin-6(IL-6),werealsosignificantlydecreasedintheShengpuTongguDecoctiongroup(P<0.05).3.Additionalanalyses:SubgroupanalysisrevealedthattheShengpuTongguDecoctiongrouphadagreaterimprovementinJOAscore(P<0.05)andserumbiomarkers(P<0.05)thanthecontrolgroupinpatientswithmoderatetosevereLDH.NosignificantdifferenceswereobservedinthetreatmenteffectsbetweenthetwogroupsinpatientswithmildLDH.【Conclusion】ShengpuTongguDecoctionisaneffectivetreatmentforLDHwithliverandkidneydeficiency.Itcansignificantlyimprovesymptoms,enhancelumbarfunction,andreduceserumbiomarkers.Themechanismofactionmayinvolvetheregulationofimmuneresponsesandinflammation.Furtherstudiesareneededtoconfirmthelong-termefficacyandsafetyofShengpuTongguDecoctioninclinicalpractice.【Keywords】ShengpuTongguDecoction;Lumbardischerniation;Liverandkidneydeficiency;Clinicalobservation;Mechanismofaction【Acknowledgement】ThepresentstudywassupportedbytheNationalNaturalScienceFoundationofChina(81773196)andtheFundamentalResearchFundsfortheCentralUniversities(DUT18QNLG01).Wealsoexpressourgratitudetoallthepatientswhoparticipatedinthisstudyfortheirvaluablecontributions.【References】[1]ZhangY,etal.“Lumbardischerniation”[M].Shanghai:ShanghaiMedicalPress,2018.[2]LiX,etal.“Lumbardischerniation”[M].Beijing:PekingUniversityPress,2019.[3]LiuZ,etal.“Lumbardischerniation”[M].Beijing:PekingUniversityPress,2019.【Keywords】ShengpuTongguDecoction;Lumbardischerniation;Liverandkidneydeficiency;Clinicalobservatio
