2、ction,填寫基本要求Basic Requirement,CMR應使用英文填寫； CMR shall filled in English 所有表格中的“*”項目必須填寫； All items marked with “*” are mandatory fields 產品中含有四種金屬的廠商至少應識別一家以上冶煉商； More than one smelter shall be identified Checker中不允許有紅色背景項目； Item with red background in “Checker” is unacceptable CMR應保存為標準EXCEL格式，否則無法上傳；

5、tion of Identified Smelter,Sheet 3：Declaration （宣告）,Declaration填寫要求Declaration Requirement,公司名稱Company Name,Company Name： 應填寫同富士康交易的法人全稱 Full name of legal body is required 請勿填寫簡稱、縮寫 Do not use abbreviation,宣告範圍Declaration Scope,Declaration Scope: 供應商應選擇適當的範圍，應包含所有交貨富士康的產品 Please select the proper c

6、lass which could cover all the product shipped to Foxconn “Product”選項應在“Product List”內寫出產品編號 Product list is needed if Product level is selected,唯一識別碼Unique Identifier,Company Unique Identifier: 獨特的識別碼可以填寫DUNS或者VAT The unique identifier could be DUNS or VAT VAT Number，Value Added Tax Number，指增值税发票号
