华中科技大学硕士学位论文基于区域卫生信息平台的医疗卫生监管系统研究姓名：曾宇平申请学位级别：硕士专业：情报学指导教师：陈敏2010-05中文摘要医 疗 卫 生 监 管 是 保 障 医 疗 卫 生 事 业 健 康 发 展 的 重 要 基 础 ， 是 国 家 进 行 医 疗 卫 生 事 业 管 理 的 重 要 手 段 。 中 共 中 央 、 国 务 院 关 于 深 化 医 药 卫 生 体 制 改 革 的 意 见 明 确 指 出 ： “建 立 严 格 有 效 的 医 药 卫 生 监 管 体 制 ， 强 化 医 疗 卫 生 监 管 ， 健 全 卫 生 监 督 执 法 体 系 ， 强 化 医 疗 卫 生 服 务 行 为 和 质 量 监 管 ， 加 强 医 疗 卫 生 机 构 的 准 入 和 运 行 监 管 ， 加 强 社 会 公 共 卫 生 的 监 管 ， 完 善 医 疗 保 障 监 管 ， 加 强 药 品 监 管 ， 建 立 信 息 公 开 、 社 会 多 方 参 与 的 监 管 制 度 ”。 医 疗 卫 生 监 管 体 系 的 建 立 已 成 为 医 疗 卫 生 体 制 改 革 必 须 解 决 的 重 要问题。区 域 卫 生 信 息 平 台 是 医 药 卫 生 体 制 改 革 的 重 要 支 柱 。 利 用 现 代 卫 生 信 息 技 术 和 区 域 卫 生 信 息 平 台 整 合 医 疗 卫 生 资 源 ， 更 好 地 为 公 众 提 供 医 疗 卫 生 服 务 和 为 卫 生 管 理 部 门 提 供 决 策 支 持 的 同 时 ， 能 更 好 地 对 医 疗 卫 生 各 领 域 进 行 监 管 。 基 于 区 域 卫 生 信 息 平 台 为 基 础 构 建 医 疗 卫 生 监 管 系 统 是 医 疗 卫 生 监 管 体 制 改 革 建 设 中 的 一 种 全 新 模 式 ， 对 提 高 监 管 工 作 效 率 ， 保 证 监 管 客 观 、 公 正 ， 增 加 监 管 透 明 度 ， 加 快 医 疗 卫 生 监 管 信 息 化建设，实现医疗卫生监管的科学化有着重要意义。目的 本 课 题 的 研 究 以 区 域 卫 生 信 息 平 台 为 依 托 ， 在 对 医 疗 卫 生 监 管 各 领 域 信 息 资 源 整 合 的 基 础 上 ， 构 建 医 疗 卫 生 各 业 务 领 域 统 一 的 、 多 方 参 与 的 、 实 时 协 同 的 医 疗 卫 生 监 管 系 统 。 主 要 研 究 目 标 包 括 ： 统 一 医 疗 卫 生 监 管 资 源 ， 构 建 协 同 高 效 的 医 疗 卫 生 监 管 应 用 平 台 ； 实 现 医 疗 卫 生 网 络 化 实 时 监 管 ； 构 建 信 息 公 开 、 社 会 多 方 参 与的监管机制。方法 本 课 题 采 用 的 研 究 方 法 有 文 献 研 究 法 ， 现 场 调 查 法 ， 系 统 分 析 法 ， 信 息 资 源 规 划 以 及 软 件 工 程 与 数 据 库 设 计 方 法 等 方 法 。 利 用 文 献 研 究 法 归 纳 分 析 国 内 外 现 有 卫 生 监 管 相 关 系 统 平 台 发 展 现 状 ， 总 结 已 有 的 相 关 经 验 ， 为 课 题 的 研 究 提 供 理 论 依 据 和 借 鉴 内 容 ； 利 用 现 场 调 查 法 进 行 个 人 深 入 访 谈 ， 初 步 了 解 医 疗 卫 生 监 管 系 统 功 能 需 求 和 信 息 需 求 ； 利 用 系 统 分 析 和 信 息 资 源 规 划 的 方 法 对 医 疗 卫 生 监 管 系 统 进 行 全 面 分 析；利用软件工程与数据库设计方法进行系统整体框架、功能体系和数据库设计。结果 详 细 分 析 了 医 疗 卫 生 监 管 系 统 的 目 标 ， 层 次 和 地 位 ， 组 织 结 构 和 工 作 职能 ， 功 能 需 求 和 信 息 需 求 ； 对 系 统 的 整 体 框 架 、 功 能 体 系 、 数 据 中 心 和 数 据 交 换 体 系进行了设计；对医疗卫生监管资源分布 Web GIS 系统进行了设计。结论 医 疗 卫 生 监 管 系 统 整 体 框 架 包 括 基 础 层 、 支 撑 层 、 数 据 层 、 应 用 层 和 访 问层五个层次以及标准规范体系与安全标准体系两大支柱；医疗卫生监管系统功能 包括医疗卫生服务监管、公共卫生监管、卫生资源监管和医疗保障监管四部分。数 据 中 心 以 监 管 数 据 分 布 模 式 为 依 据 进 行 设 计 ， 主 要 分 为 数 据 采 集 平 台 、 数 据 交 换 平 台 、 决策支持数据平台、数据利用等部分；医疗卫生监管系统交换数据库包括空间专题 数据库和医疗卫生监管系统业务数据库两部分。其中医疗卫生监管业务数据库包括医 疗卫生服务类数据库、公共卫生类数据库、卫生资源类数据库和医疗保障类数据库四 大类数据库；对医疗卫生监管资源分布 Web GIS 逻辑架构以及界面功能进行了设 计。关键词： 区域卫生信息平台 医疗卫生监管 监管信息系统 系统 分析 系统设计ABSTRACTMedical and health regulation is an important basis for guaranting healthy development of medical and health services, and is an important form for Country to manage medical and health services. “CPC Central Committee and State Council on Deepening the views of medical and health system“ clearly proposes to establish strict and effective system of medical and health regulation, to strengthen medical and health regulation, to improve the health supervision and law enforcement system, to strengthen regulation of behavior and quality of medical and health, to strengthen regulation of access and run of medical and health, to strengthen regulation of social health, to improve regulation of medical care, to strengthen drug regulation, to establish information accessible and participatory system of regulation and so on. The system of medical and health regulation has become an important problem of medical and health reform which must be solved.Regional health information platform is an important pillar of medical and health reform. Using modern health information technology and regional health information platform to integrate medical and health resource, not only provide better medical and health services for public and provide better decision support for department of health management, but also regulate medical and health services in all fields better. As a new model of construction of medical and health regulation reform, establishing the system of health regulation base on regional health information platform is of great significance to speed up the construction of medical and health regulation information, guarantee fair regulation, increase regulatory transparency, improve regulatory efficiency and achieve the scientific management of medical and health regulation.Purpose This subject research construct all business unified, multi-stakeholder involvement, and real-time collaboration system of medical and health regulation, relying on regional health information platform and the integration of information resources of medical and health regulation in all areas. Main research objectives include: Unifiedresources of medical and health regulation, establish the synergy and efficient application platform of medical and health regulation; Achieve real-time regulation of medical andhealth based on network; construct information accessible and multi-party mechanism of regulation.Method The research methods used by this subject include document retrieve, locale investigation, system analysis, information resource planning, software engineering and database design. Summarize and analyze domestic and abroad development status of system or platform of health regulation, using document retrieve method, and summarize the existing relevant experience to provide the theoretical basis and reference content for research projects; Adopt locale investigation to conduct depth interview, in order to preliminary understand functional requirements and information needs of system of medical and health regulation; Adopt the methods of system analysis and information resource planning to comprehensively analyze the system of medical and health regulation; Adopt the methods of software engineering and database design to design the overall framework, functional system and databases of system.Result Exhaustively analyze the objects, level and status, organizational structureand job functions, functional requirements and information needs of system of medical and health regulation; Design the overall framework, functional system, data center and data exchange system of system of medical and health regulation; Design the Web GIS system of resource distribution of medical and health regulation system.Conclusion The overall framework of system of medical and health regulation include five levels such as base layer, support layer, data layer, application layer and access layer and two pillars such as standard system and safety standard system; The functionsof system of medical and health regulation include four parts such as regulation of medical and health services, regulation of public health, regulation of health resources and regulation of health care. The design of data center base on distribution model of regulation data, data center include four parts such as data collection platform, data exchange platform, decision support data platform and data utilization. The exchange databases of system of medical and health regulation include space feature database and business database of medical and health regulation, which include four major parts such as databases of medical and health services, databases of public health, databases of healthresource and database of health care.
