Transformation Fund - Data Collection for Monitoring and Evaluation We need to collect data about your project to supplement the information that you supplied when you submitted your December milestone report and expenditure report. As you should already have completed the survey for the period September to December 2009 , please now supply data that covers the period from January to March 2010. You will not be able to receive your final payment until you have entered data from your project on this questionnaire. How to complete the data report: Download a copy of the questionnaire so that you can check on the information you need to have available before you try to complete the online questionnaire Collect the information you need, to answer each question (you may want to enter information on the questionnaire you have downloaded, and print that off so you can refer to it when you are completing the online questionnaire). When you are confident you have all the information you need, log on to complete the questionnaire online. You will need to do this at one sitting, and it should not take more than 30 minutes. You will need to enter your project number before you start completing the online questionnaire, so please make sure you have that to hand.
