布比卡因在麻醉意外死亡犬体内的分布情况与给药途径和剂量有关 硬 膜外麻醉时脊髓和脑脊液中含量较高 而静脉给药时则胆汁 肝 脾等脏器中含量较高 另外 硬膜外腔给药时 颈段和骶段脊髓中布比卡因的含量比胸段和腰段脊髓中高 而蛛 网膜下腔给药的结果则相反 因此 对疑为布比卡因麻醉意外致死的案件的鉴定 应全面 系统的解剖并提取各种检材 仔细分析各种检材中含量比例关系 可望为案件的鉴定提供 一定的毒物动力学依据 4 布比卡因在麻醉意外致死犬体内存在死后再分布现象 尸体储存温度是影响死后再 分布的重要因素 不同的脏器或体液在不同的储存温度下受到的影响大小不同 且其中布 比卡因的含量变化趋势不同 心血 周围血和大脑受到的影响比较大 而肝脏受到的影响 较小 血液受到的影响幅度较大 且变化趋势比较复杂 死后再分布与保存温度有关 保 存温度越高 死后再分布现象越明显 变化趋势越复杂 由研究可见 对麻醉意外案件的 鉴定应及时 全面 并应考虑死后再分布的影响 以便做出正确的鉴定结论 5 本实验研究结合前期利多卡因麻醉意外的研究提示 布比卡因与利多卡因虽同为酰 胺类局麻药 但二者在中毒症状 死后分布及死后再分布方面均存在一定的差异 需进一 步进行动态分布研究 6 实验表明 温度可以影响死后再分布的发生 推测温度是通过影响组织的自溶 崩 解及微生物的繁衍来影响死后再分布的 组织脏器的自溶
