胆源性胰腺炎的诊断和治疗 Diagnosis and treatment of biliary pancreatitis摘要：目的 探讨胆源性胰腺炎的诊断方法和治疗效果.方法 回顾性分析10年来236例胆源性胰腺炎的临床资料.患者年龄1676岁,男108例,女128例.结果 急性间质水肿型胰腺炎211例(89.4%),急性坏死性胰腺炎25例(10.5%),胆囊源性胰腺炎142例(60.1%),胆管源性胰腺炎94例(39.8%).全组开始均先用药物治疗,210例(88.9%)治愈后施行择期手术,11例(4.6%)治疗无效施行早期手术;15例(4.2%)施行急诊手术,其中1例死亡.15例内镜下治疗(6.3%).结论 胆源性胰腺炎大多数可先行药物保守治疗后,根据病变类型施行择期手术.若治疗无效,发生化脓性胆管炎或胰腺坏死感染,则须早期或急诊手术治疗,现今在内镜下治疗,效果良好.关键词:胰腺炎 诊断 治疗作者:兰州市第二人民医院普外二科 张勇 马红旗 Abstract: Objective to explore the gallstone pancreatitis diagnostic methods and therapeutic effect. Methods retrospective analysis 236 cases of 10 years gallstone pancreatitis clinical material. Patients age 16 76 years old, male 108 cases, female 128 cases. The result acute interstitial edema type pancreatitis 211 cases (89.4%), acute necrotizing pancreatitis 25 cases (10.5%), gallbladder source sex pancreatitis 142 cases (60.1%), bile duct source sex pancreatitis 94 cases (39.8%). The whole group began to all use first drug treatment, 210 cases (88.9%) and cured after elective surgery, 11 cases (4.6%) of early surgical treatment is invalid; 15 cases (4.2%) of emergency surgery, including 1 case death. 15 cases of endoscopic treatment (6.3%). Conclusion gallstone pancreatitis most but go ahead of the drug after conservative treatment, according to the disease types of elective surgical procedures. If treatment is invalid, happen suppurative cholangitis or pancreatic necrosis infection, shall be early or emergency surgical treatment, and now in endoscopic treatment, the effect is good.
