to African Minerals and Contractor personnel in relation to operation of cranes at African Minerals workplaces To ensure that all cranes and lifting equipment is acquired operated inspected maintained and repaired to agreed requirements To provide direction to personnel in the management of lifts that are considered to be critical or non standard type lifts 2 SCOPE This procedure shall be applied whenever cranes are operated at any African Minerals site Persons responsible for ensuring compliance with these requirements include The competent Crane Operator The competent Rigger DoggerDogman working with the Crane Operator The Owner Operator African Minerals or Contractor of the crane The African Minerals Area Manager The African Minerals Site Health and Safety team members The African Minerals Senior Site Official or nominated delegate The Supervisor African Minerals or other responsible for the work involving the crane 3 RELATED PTOCEDURES None 4 TEMPLATES SL03 AML 00 0000 SR FRM DEFINITIONS AND ABREVIATIONS TermDefinition Competent person A person who has acquired through a combination of qualification training or experience the knowledge and skill to perform the task required Crane Operator A competent person with sufficient training and experience who is duly licensed under the regulatory authorities to operate specified types of cranes Dogging The application of slinging techniques including the selection and or inspection of lifting gear and the directing of a crane hoist operator in the movement of a load when the load is out of the operators view Dogman A competent person with sufficient training and experienced and who is duly licensed under the regulatory authoritiesauthorised to carry out the task of dogging SL03 AML 00 00000 SR PRO 00012 00 0000 SR FRM 00056 Crane Lift Plan addressing the load which is to be lifted shall be correctly developed by a competent person Engineer Attach to this document Rated CapacityThe maximum mass in kg or tonnes which may be handled by a crane at a specific working radius in metres and a specific boom or jib length without the strength or stability requirements being exceeded The rated capacity shall comprise the following Mass of the lifted load and lifting attachments Mass of the hook block in use Any accessories fitted to the mast Rigging Work involving the use of mechanical load shifting equipment and associated gear to move place or secure a load including plant equipment or members of a building or structure and to ensure the stability of those members and for the setting up and dismantling of cranes and hoists The definition is not intended to include work performed by operators or drivers of mechanical load shifting equipment unless the work is part of the process of setting up or dismantling cranes and hoists WLL Working Load Limit The maximum gross load which may be applied to the crane hoist or lifting attachment while in a particular working configuration and under a particular condition of use Also called Rated Capacity and SWL 6 DETAILS ACTIONS 6 1 RESPONSIBILITIES AND ACCOUNTABILITIES RoleResponsibilities and Accountabilities Senior Site Official SSO Add Role to determine who Ensures provision is made for sufficient resources e g materials plant equipment personnel supervision and training Ensures there are competent and appointed person as required for operations involving cranes Site Safety Officer Ensures relevant legislation advisory standards codes of practice International Standards Australian Standards applicable Sierra Leonian Standards and African Minerals documentation are available Ensure competent persons are available to provide advice as required regarding crane operations SL03 AML 00 00000 SR PRO 00012 Operational Superintendent Mining Ensure competent persons are available to provide advice as required regarding crane operations Ensures there are competent and appointed person as required for operations involving cranes Authorise operators to use specific cranes Supervisors Authorise operators to use specific cranes Ensure all persons are suitably certified for the work procedures emergency response procedures proposed plant or equipment and are competent to perform those tasks or are provided with suitable training in the above if required Advises all personnel of the work procedure and the hazards and nominated controls in risk management documentation e g JHA Lift plans and Crane study s Provides adequate supervision by competent persons during crane operations Ensures reporting procedures are in place as required for oemergencies incidents o near misses and hazards o corrective and preventive action Rigging Supervisor Maintains a register inclusive of inspection details of lifting gear including all fall arrest equipment i e lanyards harnesses and where practicable static lines and safety ropes SL03 AML 00 00000 SR PRO 00012 Operational Procedure Crane Management confirms the crane is suitable for the lift to be conducted verifies the weight of the load to be lifted the crane is adequately supported on the ground and the crane is level materials placed under the outrigger feet or crawler tracks are set up to comply with the crane manufacturers specifications the suspended load will remain within the rated capacity of the crane the functions of the crane are operational including all crane motions brakes load cells indicators internal and external the wind is not excessive and within the cranes rated capacity for the load being lifted particularly for loads with a large surface area and when the load is high above the ground Anemometer required to be fitted to boom of crane to comply with above inspect the ground between lifts to ensure continuous operation of the crane has not compressed the ground to increase the risk to operations operate the crane in accordance with the manufacturers recommendations manual and the crane operators skills experience and competency conduct the crane pre start verifies the JHA lift plan and crane study as applicable verifies the competencies of the doggerDogman All movements of the crane are to be instructed by the dogman Ensures crane log book inspection and servicing records are current Ensure the boom of the crane is not left in a raised position when parked up or when lightning is present and Accountabilities Rigger Dogger Dogman Communicate the weight of the load and rigging gear to the crane operator to ensure the WWL of the crane is not exceeded Calculate the WWL of the ropes slings chains and other lifting accessories to be used Take adequate precautions when directing an authorised pick and carry crane across rough surfaces and checking the area for other hazards Safely sling the load and direct the crane operator on load movement considering the load may or may not be in full view of the crane operator Ensure that people and equipment are safely guarded by using the established safe work practices Disconnect and stow any lifting frame and stow rigging gear prior to travelling or tramming a crane 6 2 PRE REQUISITES The pre requisites prior to any crane work being performed are the crane is registered and certified to the legislative requirements Do our cranes meet this requirement the crane has been authorised to operate on site Documentation required for authorisation operators hold the required high risk work licenses suitable to the capacity of the crane the operators have been authorised to use the crane by the relevant supervisor the Training Superintendent or designated person the crane operator and rigger and doggerDogman have completed site specific training and competency assessments relevant planning for the crane lift is completed the crane to be used has been selected as a result of the operational criteria and risk assessments the relevant documentation has been completed The lift has been assessed and the appropriate documentation as per section 6 4 have been completed NOTE In situations where there is a need to operate cranes around or over conveyors due to the specific operational requirements or space limitations a risk assessment must be completed as a pre requisite with extra attention paid to addressing mitigation of risks Such lifts shall be considered and dealt with as Critical Lifts All lifts must be treated as critical regardless of situation 6 3 PLANNING OPERATIONS The person planning the crane Operation will be responsible to ensure a competent person will Ensure the selection of the crane for the operational requirement Ensure there is adequate workplace access Ensuring the supporting structure where applicable is capable of supporting the crane and the load Documented calculations reports shall be submitted for audit to the Senior Site Official or nominated delegate Monitor weather conditions including lightning protection Ensure protection of above and below TONKOLILI IRON ORE PROJECT Crane Management Ensure each item to be lifted is to be assessed as to whether the Lift constitutes a Standard lift Non standard Lift or Critical lift by an appropriately competent persons Where applicable geo technical data shall be obtained to determine the safe bearing capacity under crane outrigger supports Ensure the appropriate number of persons are available to undertake safe crane operations Ensure the siting positioning of the crane considers the risk to all personnel from the crane overturning and any possible collisions between the crane and other plant and structures Ensure communication processes between the crane operator and doggerDogman have been agreed Safe work instructions or procedures are in place for the specified lift 6 4 REQUIREMENTS FOR THE TYPES OF LIFTS Standard lift Standard lifts are those lifts that may range up to 25 tonne but are risk assessed as standard due to the repetitive nature of the work and lifts that are routinely performed Standard lifts will be the responsibility of competent persons a standard lift is a lift where The total load weighs less than 25 tonnes and The crane operates at less than 75 of rated capacity and The lift is routinely performed A JHA is required using the JHA template 100 FR RK 0004 Non Standard Lifts A non standard lift is a lift where the load is More than 25 tonnes in total and Greater than 75 but less than 85 of rated capacity Supported or balanced using chain blocks or lever hoists Loads which are required to be transferred to chain blocks monorail or other means Where other Loads are assessed as high risk during project risk assessments Is not a critical lift A non Standard Lift will require the preparation of the following a JHA using the JHA template 100 FR RK 0004 a Lift Plan using the Crane Lift Plan template 100 FR SA 0079 completed by the cranege team unless otherwise specified the production of drawings or calculations or both Critical Lifts A critical lift is a lift where the load is Lifts that exceed 40 tonnes in total lifts that exceed 85 of the crane s rated capacity lifts requiring two or more cranes lifts requiring special equipment e g load equalising beams special rigging or non standard crane configuration is to be used lifts to be conducted over operating process piping electrical instrument cable trays live critical or sensitive operating equipment personnel are to be lifted in workboxes a loaded crane is travelled on an incline excludes Franna type cranes working at less than 80 of chart capacity unusual awkward or complicated lifts Lifts when the ability of the ground to support the lifting equipment and suspended load is in question SL03 AML 00 00000 SR PRO 00012 Operational Procedure Crane Management Revision APage 9 Of 12 This document is uncontrolled once printed TONKOLILI IRON ORE PROJECT Crane Management lifts in or over heavily populated work areas Lifts close to overhead power lines i e within 10 metres Lifts which require specific engineering design Any demolition work other loads assessed as high risk 12 or greater by utilising the AML risk matrix A critical lift requires the preparation of the following prior to the lift a JHA using the SL03 AML 00 0000 SR FRM 00014 JHA Template development of a Lift Plan SL03 AML 00 0000 SR FRM 00056 Crane Lift Plan crane study by an Engineer or equivalently qualified competent person using the SL03 AML 00 0000 SR FRM 00057 Critical Non Standard Lift Plan Lift plan components Prior to any crane lift a lift plan of some type is necessary including lifts that are classed as non critical An ill prepared one tonne lift can be more hazardous than a 400 tonne lift if something is left unaccounted for and a mishap occurs The plan can be as simple as the supervisor the crane operator and the dogman rigger discussing the lift prior to it being made Or it can be extremely complex involving site and lift drawings and a team of experienced dogmen riggers crane operators and engineers with a rigging background 6 4 1Lift Plan The lift plan identifies the requirements needed for the primary areas of every lift including Identifies the size shape and weight of the load that is to be lifted where it will be lifted from where it will be placed and where the lifting crane s will be located The lift plan will describe the systematic assessment of important load and site factors These factors are used to determine the size of the crane needed where it will be located and what site preparations are required The size type and set up of the crane s or other lifting device as well as an up to date inspection report The load lift points attachment methods and rigging hardware to be used The step by step movements of the crane s required to ensure a safe lift and work site The plan will include any environmental lifting conditions describe the means of communications on the day of the lift A documented plan will include the names of those preparing the plan A documented plan will designate the lift supervisor supervisor crane operator s dogman riggers and state their qualifications A copy of the plan must go to the designated responsible person for recording and filing As a minimum the Lift Plan SL03 AML 00 0000 SR FRM 00056 Crane Lift Plan will address the following identify the object to be lifted weight and configuration identify the lift location describe the lift identify the parties responsible for the lift identify the Lift Weight from the Weight Control System including rigging and an appropriate contingency describe the structural geo technical assessments undertaken where applicable identify the crane to be used referencing crane studies and associated relevant drawings identify the organisational structure of the parties involved and their various responsibilities in particular who will be the lead rigger dogger SL03 AML 00 00000 SR PRO 00012 Operational Procedure Crane before and on the day of the lift describe the means of communications on the day of the lift describe the various safety issues and what particular steps will be taken on the day of the lift to ensure the safety of personnel and property indicate any limitations on the lift due to inclement weather Critical lift requirements A Weight Control Register of fabricated items and pre assemblies is to be maintained stating lift weight in metric tonnes Lift weights are to be placed on the relevant drawings The Weight Control Register is to state Each item Weight tonnes or kilograms Source of
