工伤、意外事故调查及统计程序Work injury, accident investigation and statistic procedure文件编号Document No.CSL-IMP-34文件版本Document Rev.A/0文件编号Document No.:CSL-IMP-34发行版次Document Rev.:A/0生效日期Effective Date: 2011年12月1日总页数Total page: 6(包括本页) 制定Prepared by:_日期Date:_确认Reviewed by:_ 日 期Date:_审批Approved by:_日期Date:_印有受控印章为受控文件,请参阅受控版本The document with this controlled stamp is identified as a control document; please refer to the controlled revision of the document.受控印章 受控印章Controlled stamp1.目的Purpose:为了维护员工权益使工伤者得到妥善及时的治疗同时维护公司与职工双方的利益In order to safeguard the interests of employees , so that injured workers receive proper and timely treatment, while maintaining the interests of both companies and employees2.适用范围Application scope: 本公司within the company。3.定义Definition:3.1 工伤事故:指企业职工在生产劳动过程中,发生的人身伤害(以下简称伤害)、急性中毒(以下简称中毒)。3.1 Work injury: refers to the companys employees suffer the body injury(hereinafter referred to as injury),acute poisoning(hereinafter referred to as poisoning) in the production process. 3.2 损失工作日:指被伤害者失能的工作时间。3.2 days lost : that the injured person can lose hours of work .3.3 暂时性失能伤害:指伤害及中毒者暂时不能从事原岗位工作的伤害。3.3 Temporary disability injury : the injury and poisoning person is temporarily unable to operate in their original positions.3.4 永久性部分失能伤害:指伤害及中毒者肢体或某些器官部分功能不可逆的丧失的伤害。3.4 permanent part disability injury: refer to the injury and poisoning person has irreversible loss and damage for the function of limbs or part of certain organs.3.5 永久性全失能伤害:指除死亡外,一次事故中,受伤者造成完全残废的伤害。3.5 Permanent total loss injury: defined, excepted for deaths, the injury causes total disability in an accident.3.6 意外事故:指企业职工在非生产劳动过程中,发生的人身伤害、急性中毒。3.6 Accident: refers to the companys employees suffer the body injury or acute poisoning during the non-production process.4.职责Responsibility:4.1 安全主任:负责事故的调查、分析、改善;4.1 Safety Officer : Responsible for accident investigation , analysis and improvement;4.2 人力资源部:负责工伤事故申报理赔;4.2 Human Resources: responsible for industrial accidents reporting , claims ;4.3 员工代表:参与事故的调查。4.3 Staff Representative: participate in the investigation of incidents .4.4 急救员:负责外伤的处理;4.4 paramedic : Responsible for the handling of trauma ;4.5 各部门:配合安全主任对事故调查,隐患整改.4.5 each Department: cooperate the Safety Officer to make the accident investigation and hazard correction.5.程序Operational procedure:5.1 工伤事故Work injury5.1.1 工伤事故伤害程度分类Injury degree classification of work injuries 5.1.1.1 轻伤:指损失工作日低于105日的失能伤害。5.1.1.1 minor injury : causes loss working days of less than 105 days.5.1.1.2 重伤:指相当于表定损失工作日等于和超过105日的失能伤害。5.1.1.2 serious injury: equivalent to the defined loss working days equal to or more than 105 days; 5.1.1.3 死亡death。注:以上分类标准参照国家标准局1986年5月31日发布,1987年2月1日起实施的法规企业职工伤亡事故分类标准。Note : The above classification criteria according to the national standards Bureaus regulatory the “enterprise workers casualty classification standards”, released on May 31, 1986 and implemented since February 1, 1987.5.1.2 工伤事故严重程度分类serious degree classification of work injuries 5.1.2.1 轻伤事故:指只有轻伤的事故。5.1.2.1 minor injuries accident : the only minor injuries in the accident .5.1.2.2 重伤事故:指有重伤无死亡的事故。5.1.2.2 serious injuries : serious injury accidents without death .5.1.2.3 死亡事故Fatal accidentA. 重大工伤事故:指一次事故死亡12人的事故。A. Major death accidents: the accident with the death of 1 or 2 people .B. 特大工伤事故:指一次事故死亡3人以上的事故(含3人)。B. serious work injury accidents: the accident with the death of equal to or more than 3 people.5.1.3 工伤事故报告Work injury report:5.1.3.1 工伤事故发生后,负伤者或者事故现场有关人员应当立即直接或者逐级报告部门负责人和人力资源部。5.1.3.1 After the accidents , the injured person or the related persons in the accident site should immediately report directly or step by step to the responsible department and Human Resources department .5.1.3.2 人力资源部负责人接到重伤、死亡、重大死亡工伤事故报告后,立即报告企业主管部门和企业所在地劳动部门、公安部门、人民检察院、工会。5.1.3.2 After HR department responsible staff receive serious injuries , death and major death accidents, should report immediately to the company responsible department and the labor department , public security departments, Peoples Procuratorate and work unions of the company place. 5.1.3.3 发生死亡、重大死亡工伤事故,应当保护事故现场,并迅速采取必要措施抢救人员和财产,防止事故扩大。5.1.3.3 when death accidents or major death accidents occur , should protect the accident scene , and quickly take the necessary actions to rescue people and property , to prevent accidents expanding.5.1.4 工伤事故调查work injury accident investigation:5.1.4.1 事故调查应Accident investigation should be:A. 查明事故发生原因、过程和人员伤亡、经济损失情况。A. to identify the accident causes, processes, personnel casualties and economic losses .B. 确定事故责任者。B. to determine the responsibility for the accident.C. 提出事故处理意见和防范措施的建议。C. to propose the recommendations for accident hanlding and preventive actions suggestions. D. 写出事故调查报告,事故发生部门需填写事故报告单 交到人力资源部。无论轻重与否,报告上需详细载明伤者姓名、受伤日期及时间、伤员状况等。D. to write the accident investigation report , accident departments need to fill out the Accident Report note to Human Resources department . Whether minor or major, the report should contain and detail name of injury person, date and time of injury, the injured status, etc.5.1.4.2 轻伤、重伤事故:由人力资源部或其指定人员组织生产、技术、安全等有关人员进行调查。5.1.4.2 minor injuries, serious injuries : Human Resources Department or its designee should organize production , technology, safety and other relevant staff for investigation.5.1.4.3 死亡事故:公司上报主管部门后,由主管部门组织调查,公司给予所需的支持。5.1.4.3 deaths accidents : after the company report to the authorities, should be investigated by the supervisor department and the company should provide the necessary support.5.1.4.4 重大死亡事故:公司上报主管部门后,上级部门按照公司的隶属关系由省、自治区、直辖市企业主管部门或者国务院有关主管部门组织调查,公司给予所需的支持。5.1.4.4 major death accidents : after the company report to the authorities , the higher authorities organize the investigation in accordance with the companys affiliation with the provinces, autonomous regions , municipalities, companies or the relevant authorities department of State Council, and the company should provide the necessary support.5.1.5 事故处理accident handling5.1.5.1 彻查每次工伤的成因及有关之调查结果及建议采取的实时和长期预防措施,需记录,并送交管理者代表审阅。5.1.5.1 To totally investigate the causes of each injury and the findings and recommendations related to real-time and long-term prevention actions , should be recorded and sent to the management representative for review.5.1.5.2 人力资源部须把有关员工的伤亡数据及调查经过等数据记录上交相关部门,并根据工伤保险的相关程序向社保局申报。5.1.5.2 Human Resources Department should return the employees injury data, the investigation process and other data records to the relevant departments and declare work injury insurance to the IESS in accordance with the related procedures.5.1.5.3 人力资源部须审核事故报告单 内所作出的纠正/预防措施是否适当,并跟进结果。5.1.5.3 Human Resources department should review whether the corrective/preventive actions are appropriate in the accident report note” and track results; 5.1.5.4 所有工伤事故将会在管理会议上讨论。人力资源部及各部门负责人需监察订于预防事故的安全措施有否被执行。5.1.5.4 All accidents should be discussed at management meetings . Human Resources department and each department head should monitor whether the safety action to prevent the accidents have been implemented. 5.1.5.5 如可行,人力资源部负责人将工伤事故调查的摘要公告展示,以引起各员工的注意。5.1.5.5 Where practicable, human resources department head should publish the accident investigate summary to attract the attention of the staff .5.1.5.6 人力资源部按照雇员工伤的呈报数据计算每月的工伤事故发生率,该月份及累积工伤数目连同每月的工伤事故发生率需张贴于公告栏上。5.1.5.6 Human Resources department should calculate work injury incidence for each month according to the reported employees work injuries data, and post the injuries number of this month, the cumulative number of accidents and monthly work injury incidence on the bulletin board .5.1.5.7 人力资源部负责人须按每年的工伤事故记录,分析普通工伤事故的原因,以协助预防意外发生。5.1.5.7 Head of human resources department should analyze the ordinary work injury causes and to help prevent accidents according to the work injuries record of each year , analyze the reasons for ordinary accidents to help prevent accidents.5.2意外事件Accident5.2.1 意外事件报告Accident report5.2.1.1 任何部门人员发生意外都需及时向其部门负责人及人力资源部负责人报告。5.2.1.1 Any department staff suffer accidents should timely report to their department heads and head of human resources.5.2.1.2 除非有进一步危险,否则部门负责人有权控制意外现场环境,阻止任何机械或设备的移动,直至人力资源部负责人完成意外/事故调查为止。5.2.1.2 Unless there is further danger , or department heads have the right to control the accident scene environment , to prevent any movement of machinery or equipment until the head of human resources department to complete an accident / incident investigation so far .5.2.2 意外事件调查,当人力资源部负责人收到通知后需:5.2.2 The accident investigation, when the head of human resources department receive the notification, should :5.2.2.1 到达发生事故地点和进行调查及找寻该事件/事故发生的原因。5.2.2.1 to reach the accident site and to investigate and find the causes of the event/accident .5.2.2.2 面见包括部门负责人、现场管理人员、各级员工及事件内的所有证人,并作详实记录。5.2.2.2 to meet with department heads, on-site management staff , all levels staff and all witnesses, and make detailed records. 5.2.2.3 在现场检查证据及当需要时拍照。5.2.2.3 to check the evidence on site and when necessary, take pictures .5.2.2.4 将所有事实及证据联系起来,以求得事发经过。5.2.2.4 to link all the facts and evidence to get the incident process.5.2.2.5 当事件涉及机械时,要求供货商/保养商技术人员协助调查。5.2.2.5 When the incident involves the mechanical , ask the technical personnel of suppliers/maintenance providers to assist in the investigation.5.2.2.6 准备包括以上各点在内的详细报告。5.2.2.6 to prepare a detailed report , including the above points .5.2.2.7 拟写纠正预防措施,以防止同类事件再次发生,并收集事故发生部门填写的意外事故报告。5.2.2.7 to draft corrective and preventive actions to prevent similar incidents from recurrence , and collect the accident report from the accident depar
