30岁更为多见 基本个性改变与人的关系方面 病人逐渐变得少言寡语 不愿与人交往 在工作与学习方面 他们懒于劳动 工作 学习 不守纪律 工作或学习质量逐渐下降 在个人生活方面 病人懒于料理个人生活 有的常年不理发 不更衣 不洗澡 不打扫房间卫生等 自己不干也不允许别人帮助 A Antipsychoticdrugs 感知觉改变突出的表现是产生幻觉 幻听 病人可听到别人听不到的声音 往往长时间存在 可以是谈话声 叫喊声 咒骂声等 而且内容与他有关 幻视 可在意识清醒的情况下看到奇怪的景象 内容可以是模糊的形象 也可能是清晰的人 动物 怪物或整套景象 甚至还能看到脑内 体内的东西 幻嗅 可嗅到常人嗅不到的怪味 往往是令人厌恶的气味 如粪便 农药 血腥味等 幻触 可感觉到皮肤上有电刺 虫爬感觉 本体幻觉 感到自己的内脏被扭转 断裂 穿孔 游走或晃动 内脏幻觉 感到运动的物体处于静止 静止的物体在运动 运动幻觉 感到自己的身体在倾斜 不能保持平衡 前庭幻觉 思维改变可表现为 思维散漫 在文章或谈话中 对问题的描述离体甚远 缺乏主题 句子之间的联系不紧密 思维破裂 整段文章或通篇讲话中 句与句之间毫无联系 严重时言语支离破碎 思维贫乏 讲话内容空洞 概念和词汇短缺 贫乏 病人常诉说脑子空虚 没什么可说的 与情感淡漠 意志贫乏相伴随 思维中断 在没有明显外界干扰时 思维联想会突然出现短时间不自主中断或言语停顿 思维被剥夺 思维活动突然停止 并将此归咎于某种外力将自己的思维抽走 思维被插入 病人感到脑内的思想不是自己的 是由某种外力强加于他头脑中的 思维被扩散或被广播 病人感到自己的情感被众人所体验或被到处传播 妄想 被害妄想 整天疑神疑鬼 担心别人会伤害 杀害他 给他吃的东西怀疑是下了毒的 看到别人手里的东西怀疑是杀他的刀子 关系妄想 怀疑别人背后议论他 说他的坏话 或跟踪 监视他 夸大妄想 可自称是某一伟人的后代 或自己就是一个地位很高的人物 甚至认为自己是 地球球长 宇宙长 妒嫉妄想 可长期无端地怀疑自己的妻子或丈夫有外遇 进行跟踪 盘问 纠缠 钟情妄想 还有部分病人感到某一异性对自己有好感 想与自己交友 对方回避他 也认为是怕难为情 被洞悉妄想 觉得自己的一举一动都在别人监视之下 内心的想法都被别人知道 特殊意义妄想 感到电影 电视里或周围人的一举一动 都有某种特殊的意义 均与自己有关 情感改变病人面无表情 缺乏常人的喜怒哀乐及内心体验 对朋友疏远 对亲人缺乏关心和体贴 可以用喜悦的语气描述悲惨的事情 部分病人木无表情 如同一尊塑像 行为改变病人可以在幻觉或妄想的支配下采取异常的行动 例如在被害妄想支配下可以采取伤人或自伤的行动 平时的行为混乱 乱写乱画 挤眉弄眼 胡乱打扮 伤人损物 大喊大叫 做事不分场合 不知羞耻 如当众赤身裸体 随地大小便 有的病人哭笑无常 经常无故暴怒 不知不觉中却又平静如初 精神分裂症 正常的躯体 异常的思维 阳性和阴性症状阳性症状指典型的幻觉 荒谬偏执的妄想 不合情理的恐惧等 阴性症状则指情感淡漠 情绪退缩 言语内容贫乏以及快感体验丧失 认知障碍包括记忆缺陷和智能低下 病人的社会功能受损 肝等脏器的毒性大 锥体外系反应相对较轻 需要剂量大 以奋乃静 氟哌啶醇等为代表的高效价药物 镇静作用小 锥体外系反应大 心 肝等的毒副反应相对较轻 还可分为 典型的抗精神分裂症药 对阳性症状非常有效 主要通过阻断多巴胺D2受体起作用 如氯丙嗪 氟哌啶醇等 这些药物都引起锥体外系反应 非典型的抗精神分裂症药 对阴性症状往往有效 如氯氮平 舒必利 甲硫达嗪和美呱隆等 锥体外系反应低 电抽搐疗法采用130mA或120V以下的安全电流或电压 在病人头部通电 引起病人意识丧失和全身抽搐 在大脑全面兴奋后逐步恢复正常精神活动 达到控制精神症状的目的 现在 采用改良的电抽搐疗法 通电前给予肌松药或麻醉药 通电后仅产生意识丧失 不发生肌肉抽搐 增加了安全性 并扩大了适用范围 心理治疗通过治疗者的语言和病人对治疗者的信任 对疾病给予科学的解释 对轻症病人进行指导和暗示 促使病人了解自己病变的特点 明确消除疾病的措施 唤起病人的积极情绪 改善与社会不协调的行为 逐步恢复到正常精神状态 其他治疗方法还有音乐疗法 工作娱乐治疗 中医辩证施治 针灸疗法 大脑局限性损伤 精神外科治疗 胰岛素休克 现已少用 等等 可以根据病人特点和具体条件采用 社会的关注精神分裂症病人给家庭和社会带来沉重负担 由于是慢性病 病人不可能常年住在医院 又不能让病人流落街头 伤人和自伤 因此
