Supplementary materials.（六）申办外国人就业证延期a.外国人在苏州就业延期申请表；b. 单位申请书；c. 营业执照副本，组织机构代码证，外商投资企业还需提供批准证书（复印件）；若是苏州常驻代表机构，提供代表机构登记证（复印件）及本人有效代表证（原件及复印件）；d. 用人单位与被聘人员签订的劳动合同；若其劳动合同与境外法人签订，劳动报酬来源于境外，在中国境内一年累计工作三个月以上的，需派遣方出具派遣函，明确工作地点、时间、岗位和报酬支付方式；劳动合同或派遣函是外文的，需同时提供中文翻译件，并由用人单位盖章（原件及复印件）；e.外国人就业证（原件及复印件）；f. 本人有效护照和居留许可、最后一次入境时间记录（原件及复印件）；延期时本人须在境内；g. 临时住宿登记表（没有变化不用提供）；h. 若换证提供照片一张；i. 其他补充材料。(六)Apply for “Alien Employment Permit” Extension1. “Extension Form for Foreigners Employment in Suzhou”;2. Company Application Letter;3. Business License, Organization Code Certificate, Company Certificate of Approval (Foreign Capital Company only) (copy); Registration Certificate of Resident Representative Office (copy) and Valid Certificate of Representative (original and copy) for Representatives of Resident Representative Office;4. Employment Contract; Dispatch Letter (location, period, job title and remuneration payment included) required if being dispatched by foreign company out sea more than three months; the Employment contract or dispatch letter should be written in both native language and Chinese with stamp or signature (original and copy);5. Alien Employment Permit (original and copy);6. Valid Passport, Valid Residence Permit and Latest Entry (original and copy); Foreigner has to be in China for extension; 7. Accommodation Registration Form (not required if the address havent change);8. One photo for the new “Alien Employment Permit” (if the page for extension is full);9. Supplementary materials.（七）外国人更换就业单位申办外国人就业证a.外国人在苏州就业登记表；b. 单位申请书；c. 原单位离职证明（内地单位）；若原单位已注销或关闭的，提供相关部门批文（原件及复印件）；d. 营业执照副本，组织机构代码证，外商投资企业还需提供批准证书（复印件）；若是苏州常驻代表机构，提供代表机构登记证（复印件）及本人有效代表证（原件及复印件）；e. 提供原单位就业证注销证明或外国人就业证（原件）；f. 用人单位与被聘人员签订的劳动合同；若其劳动合同与境外法人签订，劳动报酬来源于境外，在中国境内一年累计工作三个月以上的，需派遣方出具派遣函，明确工作地点、时间、岗位和报酬支付方式；劳动合同或派遣函是外文的，需同时提供中文翻译件，并由用人单位盖章（原件及复印件）；g. 个人简历（含最终学历和完整工作经历）；h. 出入境检验检疫局出具或确认的健康证明（离职一个月内可不用提供）（原件及复印件）；i. 本人有效护照及居留签证、最后一次入境时间记录（原件及复印件）；j. 临时住宿登记表（原件及复印件）；k. 照片二张（一张贴在登记表上，一张用于制作就业证）；l. 若为总经理及以上职务的，提供工商部门备案证明 (外商投资企业) ；m. 其他补充材料。(七)Apply for “Alien Employment Permit” because of Switching Employer company1. “Registration Form for Foreigners Employment in Suzhou”;2. Certificate of Cancel/Transfer of previous “Alien Employment Permit” or “Alien Employment Permit” (Original);6. Supplementary materials. （八）申办外国人就业证变更职务a.外国人就业证（原件）；b. 改任新职的相关证明（新签订的劳动合同或派遣函，职务调整相关证明材料）；若职位调整为总经理的，前任总经理先注销，新任总经理经工商部门备案后方可变更(外商投资企业)。(八) Change of the Position 1. “Alien Employment Permit” (original);2.
