一、甲、乙双方组成实习辅导小组，由明新科技大学旅馆事业管理系实习委员会代表暨乙方代表共同组成，实习辅导小组任务如下:1.召开协调会，解决学生实习问题或讨论双方合作事宜，以利甲方教学与乙方营运的配合。 2.实习期间甲方分发学生至乙方各单位实习，实习学生除接受甲方实习委员会。 管理外，亦接受乙方实习单位主管之指挥监督，遵守公司政策及工作规则。 3.。 实习期间双方派专人负责指导与考核，藉以督促学生校外实习的能力训练。 4.乙方负责监督学生实习训练与生活辅导，如遇情节重大情事，得由双方开会议定提前终止实习，并通知家长协同处理。 5.其它有关建教合作校外实习事项。 二、实习期间:自民国年月日起至年月日止。 三、甲方之职责:1.协助乙方遴选分发实习学生。 2.协助乙方实习训练内容或课。 程，访视辅导实习学生，提供学生考核资料。 3.通知实习学生接受乙方半年单位轮调安排。 4.负责约束选派之实习学生，切实遵守乙方所安排实习单位工作及作息规定。 四、乙方之职责:1.提供甲方实习学生员额与专业训练课程，唯不使学生担任非相关及危险性工作。 2.安排甲方实习学生半年单位或职务轮调，并提前通知学生与甲方实习委员。 会访视老师，但实习学生于柜台、订房、与餐厅内场三项职务得免除轮调。 3.非约定之学生实习单位异动，须征求本系实习委员会同意。 4.实习期间负责实习学生工作时间内之生活管理与实习成绩之考核。 5.实习期间给予实习学生每名每月新台币元正之实习津贴，并依劳动基准法及其它相关法令为实习学生办理劳保、健保。 6.实习期间应给予实习学生比照一般职员之休假与福利制度。 7.其它有关实习事项，依劳动基准法与乙方人事规定办理。 五、本合约如有未尽事宜或变更事项，由双方协调修订之。 activitiesinpatible orimproper withthis TrainingProgram.b)Not plyingwith therules andregulations ofSponsor,employer orthe lawsof Singapore.c)Certified medicallyunfit bySponsors appointeddoctor oris 明新科技大学旅馆事业管理系海外实习学生家长同意具结书人立同意书签署人_与敝子弟实习生(以下简称实习生)_就读于明于新科技大学旅馆事业管理系，同意于96年年7月月1至日起至97年年6月月30日止，由学校签约之建教合作实习代表EWM ConsultancyPte Ltd范安排第三学年校外实习课程之海外专业实习，为规范EWM ConsultancyPte Ltd与签署人间之权益，签署人与实习生同意下列条款并遵守履行之。 1.实习单位部门:EWM ConsultancyPte Ltd安排之新加坡国际级观光饭店2.月薪:600新加坡币3.每周工作时数:48小时4.实习时间:12个月5.实习绩效保证金:实交习生应于抵达新加坡后缴交$500给元新加坡币给EWM ConsultancyPte Ltd作为实习绩效保证金，实习生完成一年实习后将全额退为回；若未完成一年实习，此实习绩效保证金将作为EWM ConsultancyPte Ltd名誉受损与实习安排经费之赔偿。 6.实习生合约完成之绩效奖金$100新加坡币每月，绩效奖金于实习完成后一成次发给；未完成12个月实习合约，无法领取绩效奖金。 7.实习期间提供宿舍，每间房间住四位实习生。 实习生须负责平均分摊每月水费、电费、瓦斯费及宿舍受损修护等费用；工作时由实习单位提供餐食。 8.若未获机关补助，则实习生自行负担新加坡来回机票费与每日所需之交通费。 9.任何一方若决定终止实习关系，须于一个月前以书面提出或支付一个月的工作薪资；在实习工作期间与工作之后若有行为不当、表现不佳、触犯法律、违反规定或与顾客交往，实习单位有权终止实习合作关系且无须提前告知并遣送回国，由实习生自行负责回国交通费。 10.上班工作时间完全由实习单位安排，实习生能够在同一个工作单位或不同部门工作。 除了实习单位外，实习生不得在新加坡其它单位工作或打工，亦不得吸毒、酗酒、从事不法、不道德、不诚实、犯罪或政治活动等违反新加坡法律之行为。 11.除了EWM ConsultancyPte Ltd安排的宿舍外，实习生不能够在外过夜，也不允许带任何来宾、朋友、亲戚于宿舍过夜；另外不得于宿舍内吸烟、吸毒、喝酒；实习生应尊重宿舍室友的个人隐私、健康、环境及舒适，并维护宿舍内的清洁与卫生。 13.实习生于新加坡实习期间，由实习单位支付移民安全保证金5000新加坡币予新加坡政府，若实习生因任何犯罪而丧失实习机会，须完全赔偿实习单或位或EWM ConsultancyPte Ltd之移民安全保证金5000新加坡币。 另外，实习生护照将由实习单位统一保管以免遗失。 14.实习生保证完全遵守新加坡法律以及实习单位与EWM ConsultancyPte Ltd之相关规定。 15.守实习生须承诺遵守EWM ConsultancyPte Ltd的相关规定。 EWM ConsultancyPte Ltd、实习单位或同事等不负责个人的受伤、死亡、所有的花费、遗失、赔偿与支出。 16.前述15条款依新加坡法律管辖执行之。 除了上开建教合作实习代表约定事项外，实习生于实习期间愿意配合学校督导，遵守贵校各项实习规章及生活条约，服从贵系实习委员会及实习单位之。 指导训练，为规范学校与签署人间之权益，签署人同意下列条款并遵守履行之。 1.实习生于校外实习期间，仍为学校学生，优良或不良表现依明新科技大学学生奖惩办法与旅馆事业管理系校外实习学生奖惩办法办理。 2.实习生前往实习机构实习，依实习单位请假规定及劳动法办理，若实习单位无明确规定则依学校请假规定办理。 海外实习生应完成一年海外旅行平安险(附约疾病医疗住院险)事宜，并缴交保险单据复印件予贵系后方赴海外实习。 3.实习生于校外实习期间，未经告知无故离职、工作懈怠或连续犯错等重大违规事件，经贵系实习委员会派员调查，会同实习单位确认，经本会通过移送本校学务处生活辅导组签请学生奖惩委员会处理，视情节得处以下列处分: (1)勒令退学。 (2)记过处分，经贵系实习委员会同意并由签署人具结后，始得继续实习。
