These events could be caused 有以下的原因所导致:due to any of the following :OIL POLLUTION(油污)GROUNDING / STRANDING(抢滩/搁浅)FLOODING(进水)FIRE / EXPLOSION(着火/爆炸)COLLISION(碰撞)MAN OVERBOARD / SEARCH & RESCUE(人员落水/搜救)SERIOUS INJURY(严重受伤)ABANDON SHIP(弃船)MACHINERY DAMAGE LEADING TO ANY OF ABOVE SITUATION(机器受损而导致的上述情况)PIRACY(海盗)CONTAINER GOING ADRIFT(集装箱移位)MAIN ENGINE FAILURE(主机失灵)STEERING FAILURE(舵机失灵)GENERAL POWER FAILURE(发电机故障)GYRO COMPASS FAILURE(电罗经失灵)HEAVY WEATHER DAMAGE(恶劣天气的损害)STRUCTURAL FAILURE(船体结构故障)EXCESSIVE LISTChapter 1 : IntroductionContingencyManualPrepared by :D M RApproved by : PresidentRevision : 2Effective Date :01 July, 2003Page : 3 of 61.2 CONTINGENCY PLAN ON BOARD VESSELThe procedure to be adopted in case of various contingencies that may arise on board a vessel has been described in the Vessel Operation Manuals following chapters :- Chapter 16 (Emergency Preparedness and Training)- Chapter 10 (Pollution Prevention Measures).In implementing the contingency plan on board the Master shall be responsible to ensure that :- Training for action to be taken in case of potential emergency situation that are likely to involve the ship are practiced at drills.- Each individual of th emergency party is aware of his allocated duties and responsibilities.- The fire fighting and life saving appliances are properly maintained and readily available.- All incidents are immediately reported to the office using the best method of communication available.- The safety of the vessel and personnel are uppermost in his mind and he should use his discretion to take whatever actions he considers to be in the best interest of the crew, cargo and the vessel.- A record of the sequence of events is maintained.Chapter 1 : IntroductionContingencyManualPrepared by :D M RApproved by : PresidentRevision : 2Effective Date :01 July, 2003Page : 4 of 61.3 CONTINGENCY PLAN IN SHORE SIDE OFFICE(i) Emergency Response TeamThe Emergency Response Team of the Shore Based Office shall consist of following personnel :Senior MemberStand-byGeneral Manager (Technical)Designated Management RepresentativeSuperintendent (1)Superintendent (2)Designated Management RepresentativeAssistant DMR The Support Team shall consist of the following personnel if required :Technical AssistantPersonal Assistant(ii) Duties of the Emergency Response TeamThe General Manager (Technical) shall be in charge of the activities of the Emergency Response Team and will keep the President informed about all major decisions.He shall he responsible for directly communicating with the vessel and providing the necessary guidance and assistance as required. He shall also be responsible for the following :-Determining, in consultation with the Master, the necessity of engaging the services of third party contractors to help contractors to help the vessel/Master.- Determining the necessity of calling for the services of the Support Team.- Allocate duties to the Emergency Response Team, keeping in view rest periods to be given during prolonged emergency periods.Chapter 1 : IntroductionContingencyManualPrepared by :D M RApproved by : PresidentRevision : 2Effective Date :01 July, 2003Page : 5 of 6The Engineer Superintendent shall ensure that all the technical information and data pertaining to the vessel and relevant to the Emergency at hand is made available to the Emergency Response Team. He shall also assist the Fleet Superintendent in making technical decisions.The Designated Management Representative shall be responsible for maintaining a record of all verbal and written communication between the office and the vessel, plus communication between office and other bodies with interest in the vessel (P&I, H&M, Charterer, etc). He shall also be available to be dispatched to the scene of incident, if required.(iii) Mustering the Emergency Response Team during after office hoursIf an emergency situation should arise after office hours, The emergency Response Team shall muster in the office as per following procedure :-The Designated Management Representative / Engineer Superintendent shall carry out the following:- The Designated Management Representative shall receive the initial report from the Master of the vessel and shall call the General Manager (Technical) and inform him of the incident with all the available details.- The General Manager (Technical) shall determine whether an Emergency situation exists and whether the Emergency Response Team should be mustered in the office.- On instructions from the General Manager (Technical), he shall then make all the necessary arrangement to muster in the office.- All members of the Emergency Response Team shall try to ensue that they are in the office as soon as possible after being notified of the emergency Situation.Chapter 1 : IntroductionContingencyManualPrepared by :D M RApproved by : PresidentRevision : 2Effective Date :01 July, 2003Page : 6 of 6(iv)CommunicationThe Master shall intimate the DPA in case of any emergency situation first by telephone. All subsequent communications shall preferably be by telex or fax, failing either telephone may also be used. A separate register to be maintained for keeping this record. A telephone log shall be maintained to record the gist of all telephone conversations.During the period of the Emergency, all written communication between the vessel and the office shall be copied to the President.(iv) Post incident ReviewWithin 48 hours after the Emergency situation has passed over, the emergency Response Team shall meet the prepare a detailed report of the incident,.
