2、rk; work at height;3. 定义/Defi niti ons3.1 动火作业：进行焊接与切割作业及在易燃易爆场所使用喷灯、电钻、砂轮等进行可能产生火焰、火花和赤热 表面的临时性作业；Hot work : Any work involving welding, cutting, grinding or other similar operation produc ing sparks3.2 高处作业：利用脚手架、升降平台、便携式木梯和金属梯等登高工具从事超过基准面2米以上，存在坠落风险的作业；Work at height:4. 程序 /Procedure序号NO.危险作业内容Ha

3、zardous operati on4.1动火作业Hot work4.1.1职责 Resp on sibility4.1.1.1申请人的职责（必须由我司项目负责人担当）：1.动火作业前负责动火作业申请，并遵守“一天一证一动火点”为原则；2对外来施工单位的资格合法性审查，提醒外来施工单位带齐动火所需要的相关证件、防护装备；以备 我司检查；3. 对动火作业现场进行确认，确保现场符合动火要求；4. 对动火人员的劳动防护、动火设备的安全状态负责；5. 对动火人员进行培训，确保熟悉动火作业要求、消防器材使用方法和应急处理流程（必 要时，联系EHS人员助指导）；6. 每天上、下午各不少于一次不定时对动火现

4、场进行巡视，发现异常立即予以纠正；7. 所有供应商外服人员、外来施工人员等进行的焊接、气割、烘烤等明火作业均需要提前申请；Resp on sibility of applica nt （The applica nt should be the project leader）:1. 7、 job site at least one time irregular in morning and after noon. Shut down and make corrective action if there are any unsafe conditions or acti ons regard ing cutt ing, weldi ng and/ or heati ng operati ons.7. A hot work permit is required prior to start any hot work.4.1.1.2审核人的职责(动火区域主管，及EHS部门担当)：

Go to job site and check out the fire-proof measures before approving the operation, ensure that it1 s4.1.1.3动火人职责1. 查验动火作业申请单审批情况，无动火作业申请单、申请单上相关流程未审批完毕的应拒绝 进行动火作业；2. 逐项确认相关安全措施的落实情况；3. 确认动火地点和时间，是否和

9、动火作业申请单一致；4. 发现不具备安全条件时不得进行动火作业；5. 随身携带动火作业申请单，以备检查；6. 所有金属焊接、气割作业人员必须有“金属焊接切割作业操作证”,并熟悉动火要求及消防器材使用 方法；7. 动火人员必须佩戴焊工专用绝缘手套、绝缘鞋及其它防护用品；8. 在动火作业完成后，清理现场，清除残火，确认无遗留火种后方可离开现场。Resp on sibility of fire operator:1 Check out hot work got approval. 11、site6. Fire operator must be trained specially and get the lice nse.7 Wear proper pers onal protective equipme nt such as gloves, safety shoes and face shield.8. Clean the job site; make sure no fire in job site when finishing fire operation.4.1.1.4监火人职责1.坚守岗位，不准脱岗若暂时离开需找适合人员替岗，并尽快返回；动火期间，不准兼做其它工作；2

12、. 负责现场动火前的准备工作并监护与检杳，发现异常情况应立即通知动火人停止动火作业，及时联系有关人员采取措施；3. 当发现动火人违章作业时应立即制止；4. 在动火作业完成后，应会同有关人员清理现场，清除残火，确认无遗留火种后方可离开现场。5. 监火人需在动火作业完成1小时及3小时内，返回现场检查确认无遗留火种。Responsibility of fire watcher:1 Fire watcher must constantlyroam the work point that he is assigned for,aware for pote ntial hazards an d/or ch

The fire watcher shall be presented for at 1 hour and 3 hours after hot work isdon e.4.1.2动火作业安全要求 Safety requireme nt for hot work:4.1211.所有动火申请由本公司项目负责人负责，以“一天一证一动火点”为原则，负责现场安 全检查和应急 处理，熟悉消防器材使用方法；2动火人、监火人都必须

15、熟悉动火要求，并承担现场监督、防火管理责任。3. 五级以上风力或雨雪天气，禁止室外动火作业；4. 所有金属焊接、切割作业人员必须有u金属焊接切割作业操作证”,为防止触电事故的发生，动火人员必须佩戴焊工专用绝缘手套、绝缘鞋及其它防护用品，包括外服人员和外 来施工人员， 并保证焊机绝缘层无破损；5. 动火作业前必须将周围易燃易爆物品移至安全位置。一般情况下，在施工区10m范围 内，无可燃物，易燃液体，易燃气体，可燃粉尘等均清理干净。如仍不能满足安全要求，若半径10m范围内的易燃、易爆品不能撤离，用防火布或湿木板隔离；（特别说明：靠近叉车电瓶区域作业时，必须将电瓶拆除或者切断电瓶电源，并进行充分通风

16、，保证作业区域内无燃爆 气体后，方可动火，防止氢气爆炸）6. 动火期间距离动火区域10米内不得排放各类可燃气体，可燃液体；在动火点10米范围 内及用火点下方不得同时进行可燃溶剂清洗、喷漆或抛丸等作业。7. 所有气瓶严禁靠近火源、热源和电气设备，与明火距离不少于5米；氧气瓶和乙快气瓶距离不少于5米。乙快瓶禁止使用其它气瓶进行替代，并保证有止回火阀。所有气瓶必须无 破损，压力正常；8.动火现场配置两只以上灭火器，必要时将消防水带铺设到作业现场。无火警时禁止使用，否则照价赔 偿，使用完成，及时放回指定位置；9.动火结束后，动火人员和监火人须共同进行确认，保证现场无火警后，方可离开作业现 场。动火作业

Hot work in ope n air is forbidde n whe n there is bad weather such as rain storm, and heavy wi nd above five grades that in flue nee the safety of work.4. Fire operator must be trained specially, and gets the license. They sh

19、ould wear proper pers onal protective equipme nt to en sure safety operati on. Any damaged or frayed cord in weld ing machi ne should remove from service The con structors are required to follow this requireme nt.5. Exposed flammable/combustible materials and equipme nt with a 10m radium must remove

20、 or be protected by fire-proof covers or fire bla nkets before hot work begins.(Remark: Shut off the power and keep good ventilation while fire operati on being done n ear charg ing area, it will preve nt gas explosi on)6. All combustible dust,vapor, gas of liquids must be kept 10m away or properly

21、covered with fire-proof curtains, fire blan kets .It mea ns weld/cut within 10m was forbidde n while a paint booth or shot blaster was running.7. Oxyge n cyli nder should keep at least 5m away from acetyle ne cyli nder. Mean while gas cyli nder should keep 5m away from ign iti on source and in good

22、con diti on.8. Two or more fire ext in guishers must be kept in job site.9. Clean the job site when finished fire operation and make sure there is no fire risk. The fire watcher shall be presented for at 1 hour and 3 hours after hot work is done EHS departme nt will archive hot work permit.4.2高空作业 W

Check out there is no work with a 3 meters radiom.421.3审核人的职责(安全部门担当)；1 .高处作业前对作业现场进行确认，确保现场符合作业要求；2检查作业人员咼处作业申请单手续申请是否齐全、作业人员防护是否配齐；施工方针对所有高处 作业的管理要求是否了解；3.审核人员每天对施工现场上下午各不少于1次巡杳，发现异常立即予以纠正，并记录。Resp on sibility of auditor (facility-EHS departme nt):1 Check out the condition of job

29、 on, shut dow n the operati on and take corrective acti ons.4.2.1.4作业人员的职责1.针对身体状况负责，有咼处作业禁忌的症状（咼血压、心脏病、贫血、癫痫等）视力不 佳、疲劳过度 等不适宜进行高处作业的情况，严禁高处作业；2查验咼处作业申请单的审批情况，无咼处作业申请单、申请单上相关流程未审 批完毕的应禁 止作业；3. 掌握高处作业的安全要求，并逐项确认相关安全措施的落实情况；4. 配齐防护设备，对于危险性过高且无法进行安全保障的应禁止作业；5. 高于5米的高处作业（3个脚手架高度）必须持证上岗；6. 确认咼处作业地点和时间，是否

If operator is worki ng above 5m, he shall receive special tech no logy trai ning, pass releva nt test and gain authorizati on.6. Check out the job site and work time is according with work permit.421.5监护人员的职责（可以为施工方的人员）1.高处作业监护人必须全程监护，坚守岗位；对使用靠墙梯进行高处作业的，监护人员必须扶住梯梁2 .监护高处作业人员正确履行各项安全职责、落实各项安全措施；3.随身携带高处作业申请单，以备检查。Resp on sibility
