Methods:A total of 3,012 patients who underwent insertion of a PICC were

Thelocations of the tube tips were recorded by chest Xray examination.Results:Malposition of the PICC was observed in 237 cases (7.87%), with the most frequently occurring site of misplacement being the jugular vein, followed by the axillary vein. 16、因分析与护理对策真实性重要性适用性NOS质量评价法主要内容目的:探索PICC异位的原因与护理对策。方法:通过回顾性分析,对25例PICC异位的发生部位 置管途径、处理方法及结果进行研究、总结。结果:PICC异位与静脉选择、置管长度的外测量方 法、患者局部血管解剖变异、穿刺时患者体 位、操作者送管技巧等因素有关。结论:为了减少PICC异位的发生，置管部位宜选择右 上肢、贵要静脉；传统外测量方法与送管时患 者体位有待改进；多数PICC异位可以通过各种 方法纠正。真实性1.其研究是否具有真实性 导管异位是PICC的常见问题，发生率约6.7。我科于2003年5月至2008 年7月共进行PICC置管36

17、1例，发生导管异位25例（692）。2.研究对象是否能代表所研究的目标人群 可以，本组361例PICC置管患者发生PICC异位25例。25例中，男16例，女9例；年 龄1584岁，平均39.5岁；白血病9例，淋巴瘤11例，多发性骨髓瘤3例，其他2例。3.研究方法论证强度 通过回顾性队列研究，25例PICC异位的发生部位、置管途径、处理方法，可行性较好。 4.随访时间是否足够？随访是否完整？ 我科于2003年5月至2008年7月共进行PICC置管361例，发生导管异位25例，本文提及了队列的开始时间及终止时间，随访时间足够。5.是否对影响结局的重要因素进行了统计学的校正 否，本研究并没有对混杂因

18、素进行校正6.病因学研究因果效应的先后顺序是否合理 合理，因：静脉选择、置管长度的外测量方法、患者局部血管解剖变异、穿刺时患者体位、操作者送管技巧等因素。果：PICC异位7.病因学研究的结果是否符合流行病学的规律 符合，属于历史性队列研究8.病因致病的因果关系是否在不同的研究中反映出一致性 是。已有多个队列研究、病例对照研究涉及PICC异位与静脉选择、置管长度的外测量方法、患者局部血管解剖变异、穿刺时患者体位、操作者送管技巧等因素有关，结果都是肯定的。重要性 导管异位可明显增加PICC其他并发症的发生，如液体渗漏、肢体肿胀、疼痛等，也可发生一些特殊的危险，如椎体旁积液、房颤等，常常导致不能完成治疗而拔除导管，有时需要重新置管。 适用性作为指导防治政策:PICC异位与静脉选择 、置管长度的外测量方法 、患者局部血管解剖变异、穿刺时患者体位、操作者送管技巧等因素有关。置管后必须摄X线片明确导
