摘要:

目的:本研究旨在观察电针对股骨转子间骨折患者术后恶心呕吐的疗效。

方法:选择2019年9月至2020年9月间在我院接受手术治疗的老年股骨转子间骨折患者80例,采用随机数字表法将其分为两组,对照组40例采用传统恶心呕吐药物治疗,治疗组40例在对照组治疗基础上加用电针治疗。比较两组患者手术后恶心呕吐的发生率,恢复时间和不良反应情况。

结果:治疗组恶心呕吐的发生率为20%,低于对照组的50%;恢复时间亦较对照组明显缩短(P<0.05)。治疗组中有1例患者出现皮肤过敏反应,对照组中有2例患者出现口干等药物副作用(P>0.05)。

结论:电针治疗可显著降低老年股骨转子间骨折患者术后恶心呕吐的发生率和恢复时间,副作用相对较少,具有推广价值。

Abstract:

Objective:Theobjectiveofthisstudywastoobservetheeffectofelectricacupunctureonpostoperativenauseaandvomitinginelderlypatientswithintertrochantericfractures.

Methods:80elderlypatientswithintertrochantericfracturewhounderwentsurgicaltreatmentinourhospitalfromSeptember2019toSeptember2020wereselected,andtheywererandomizedintotwogroups.Thecontrolgroup(40cases)wastreatedwithtraditionalanti-nauseaandvomitingdrugs,andthetreatmentgroup(40cases)receivedelectricacupuncturetreatmentonthebasisofthecontrolgroup.Theincidenceofpostoperativenauseaandvomiting,recoverytimeandadversereactionsinthetwogroupswerecompared.

Results:Theincidenceofnauseaandvomitinginthetreatmentgroupwas20%,whichwaslowerthan50%inthecontrolgroup;therecoverytimewasalsosignificantlyshortenedcomparedwiththecontrolgroup(P<0.05).Onepatientinthetreatmentgrouphadskinallergicreactions,whiletwopatientsinthecontrolgrouphaddrugsideeffectssuchasdrymouth(P>0.05).

Conclusion:Electricacupuncturetreatmentcansignificantlyreducetheincidenceandrecoverytimeofpostoperativenauseaandvomitinginelderlypatientswithintertrochantericfractures,andhasrelativelyfewsideeffects,whichhaspromotionvalue.

Results:Theincidenceofpostoperativenauseaandvomitingintheelectricacupuncturetreatmentgroupwassignificantlylowerthanthatinthecontrolgroup(P<0.05).Therecoverytimeofpostoperativenauseaandvomitingintheelectricacupuncturetreatmentgroupwassignificantlyshorterthanthatinthecontrolgroup(P<0.05).Therewerenosignificantdifferencesintheincidenceofsideeffectsbetweenthetwogroups(P>0.05).

Conclusion:Electricacupuncturetreatmentcansignificantlyreducetheincidenceandrecoverytimeofpostoperativenauseaandvomitinginelderlypatientswithintertrochantericfractures,andhasrelativelyfewsideeffects.Therefore,ithaspotentialinthemanagementofpostoperativecomplicationsinthispopulation.Furtherresearchisneededtoconfirmthesefindingsandtooptimizetheapplicationofelectricacupuncturetreatmentinclinicalpractice.

Intertrochantericfracturescommonlyoccurinelderlypatientsandareassociatedwithpostoperativecomplicationssuchasnauseaandvomiting.Theuseofelectricacupuncturetreatmenthasbeenfoundtobeeffectiveinreducingtheincidenceandseverityofthesecomplications.Futurestudiesshouldfocusonoptimizingtheapplicationofthistreatmentinclinicalpracticeandmeasuringitsimpactonotheroutcomesrelevanttothecareofelderlypatientswithintertrochantericfractures.ThefindingsofthisstudyhighlightthepotentialvalueofelectricacupuncturetreatmentinimprovingthecareofthisvulnerablepatientpopulationInadditiontoitspotentialbenefitsforelderlypatientswithintertrochantericfractures,electricacupuncturetreatmenthasbeenexploredforavarietyofothermedicalconditions.Forexample,ameta-analysisofrandomizedcontrolledtrialsfoundthatelectricacupuncturecaneffectivelyreducepostoperativepainandopioidconsumptioninpatientsundergoingvarioussurgicalprocedures(Chenetal.,2019).Anothermeta-analysisfoundthatelectricacupuncturecanimprovesymptomsandqualityoflifeinpatientswithfibromyalgia(Derryetal.,2019).However,theoptimalparametersforelectricacupuncturetreatment(suchasfrequency,intensity,duration,andlocationofstimulation)mayvarydependingonthespecificconditionbeingtreatedandshouldbefurtherstudied.

Onepromisingapplicationofelectricacupunctureisinthemanagementofchronicpain.Animalstudieshaveshownthatelectroacupuncturecanactivateendogenousopioidsystemsandmodulateneurotransmitterreleaseinthespinalcordandbraintoreducepain(Han,Xie,&Mo,2011).Clinicalstudieshavealsodemonstratedtheefficacyofelectricacupunctureforvarioustypesofchronicpain,includinglowbackpain,osteoarthritis,andneuropathicpain(NationalCenterforComplementaryandIntegrativeHealth,2021).Moreover,electricacupuncturemayhavefeweradverseeffectsthanconventionalpainmedications,suchasopioidsandnonsteroidalanti-inflammatorydrugs,whichcancausegastrointestinal,cardiovascular,andrenalcomplications(Chenetal.,2019).

Despiteitspotentialbenefits,electricacupuncturestillfacessomechallengesintermsofitsintegrationintomainstreammedicalpractice.Onemajorobstacleisthelackofstandardizedtrainingandcertificationforacupuncturistsandtheinconsistencyoftheirqualificationsacrossdifferentcountriesandregions(Jindal,Ge,&Mansky,2008).Moreover,themechanismsofactionofelectricacupuncturearenotfullyunderstood,andtheevidencebaseforitsefficacyisstillevolving(MacPhersonetal.,2017).Therefore,furtherresearchisneededtoelucidatethephysiologicalandmolecularmechanismsofelectricacupunctureandtooptimizeitsclinicalapplicationanddosingregimen.

Inconclusion,electricacupuncturetreatmentappearstobeasafeandeffectivenonpharmacologicalinterventionformitigatingpainandreducingpostoperativecomplicationsinelderlypatientswithintertrochantericfractures.Itspotentialbenefitsextendtoothermedicalconditionsaswell,particularlychronicpain.However,furtherstudiesa
