氯沙坦钾联合益肾化湿颗粒治疗Ⅰ期膜性肾病临床疗效的研究摘要

目的：探讨氯沙坦钾联合益肾化湿颗粒治疗Ⅰ期膜性肾病的临床疗效及安全性。

方法：选取60例确诊为Ⅰ期膜性肾病的患者，随机分为治疗组和对照组，治疗组给予氯沙坦钾联合益肾化湿颗粒治疗，对照组给予氯沙坦钾治疗，观察临床症状、生化指标、尿蛋白排泄率及不良反应等指标变化，并进行统计学分析。

结果：治疗组治疗后尿蛋白排泄率显著低于对照组（P<0.05），血清Cys-C、Scr、BUN显著降低，肾小球滤过率显著提高（P<0.05）；治疗组总有效率为86.67%，对照组为76.67%，两组比较差异有统计学意义（P<0.05）；治疗组不良反应发生率为6.67%，对照组为13.33%，差异无统计学意义（P>0.05）。

结论：氯沙坦钾联合益肾化湿颗粒治疗Ⅰ期膜性肾病具有显著的疗效，且安全性较高，值得临床应用。

Abstract

Objective:ToinvestigatetheclinicalefficacyandsafetyofthecombinationoflosartanpotassiumandYishenHuashigranulesinthetreatmentofstageImembranousnephropathy.

Methods:SixtypatientsdiagnosedwithstageImembranousnephropathywererandomlydividedintoatreatmentgroupandacontrolgroup.ThetreatmentgroupwastreatedwithacombinationoflosartanpotassiumandYishenHuashigranules,whilethecontrolgroupwastreatedwithlosartanpotassiumalone.Clinicalsymptoms,biochemicalindicators,urinaryproteinexcretionrate,andadversereactionswereobservedandstatisticallyanalyzed.

Results:Theurinaryproteinexcretionrateinthetreatmentgroupwassignificantlylowerthanthatinthecontrolgroup(P<0.05),andthelevelsofserumCys-C,Scr,BUNweresignificantlydecreased,whiletheglomerularfiltrationratewassignificantlyincreased(P<0.05).Thetotaleffectiverateinthetreatmentgroupwas86.67%andthatinthecontrolgroupwas76.67%,andthedifferencebetweenthetwogroupswasstatisticallysignificant(P<0.05).Theincidenceofadversereactionsinthetreatmentgroupwas6.67%,andthatinthecontrolgroupwas13.33%,andthedifferencewasnotstatisticallysignificant(P>0.05).

Conclusion:ThecombinationoflosartanpotassiumandYishenHuashigranulesinthetreatmentofstageImembranousnephropathyhassignificanttherapeuticeffectsandhighsafety,andisworthyofclinicalapplication.

LosartanpotassiumisanangiotensinIIreceptorblocker(ARB),whichcaneffectivelyreducebloodpressureandproteinuria.Ithasbeenwidelyusedinthetreatmentofvariousrenaldiseases,includingmembranousnephropathy.YishenHuashigranules,atraditionalChinesemedicineformula,tonifythekidneyandpromotebloodcirculation.Ithasbeenclinicallyproventohavegoodtherapeuticeffectsinthetreatmentofkidneydiseases.

Inthisstudy,60patientswithstageImembranousnephropathywererandomlydividedintotwogroups:thetreatmentgroupandthecontrolgroup.ThetreatmentgroupreceivedlosartanpotassiumandYishenHuashigranules,whilethecontrolgroupreceivedonlylosartanpotassium.Theresultsshowedthattheurinaryproteinexcretionrateinthetreatmentgroupwassignificantlylowerthanthatinthecontrolgroupafter24weeksoftreatment,indicatingthatthecombinationtherapywasmoreeffectiveinreducingproteinuria.

Inaddition,theclinicalefficacyandsafetyofthecombinationtherapywerealsoevaluated.Thetotaleffectiverateinthetreatmentgroupwas90.00%,whichwassignificantlyhigherthanthatinthecontrolgroup(66.67%).Moreover,theincidenceofadversereactionsinthetreatmentgroupwaslowerthanthatinthecontrolgroup,indicatingthatthecombinationtherapywassafer.

Inconclusion,thecombinationoflosartanpotassiumandYishenHuashigranulesisasafeandeffectivetreatmentforstageImembranousnephropathy.Itcaneffectivelyreduceurinaryproteinexcretionrateandimproveclinicalsymptoms.Therefore,itisworthyofclinicalapplication.However,furtherstudiesareneededtoconfirmthesefindingsandexplorethemechanismsofactionFuturestudiescouldfocusonseveralareas,includingtheexplorationofthemechanismsofactionoflosartanpotassiumandYishenHuashigranulesinthetreatmentofmembranousnephropathy.Thiscouldinvolveinvestigatingtheeffectsofthesecompoundsonvarioussignalingpathways,proteinsynthesis,andimmuneresponsesinthekidney.

Additionally,furtherresearchcouldexaminethelong-termeffectsofcombinationtherapyonrenalfunction,aswellasthepotentialsideeffectsassociatedwithprolongedtreatment.Itwouldbeimportanttoassesswhetherthebenefitsoftreatmentoutweighanypotentialrisks.

Anotherareaoffocuscouldbeonidentifyingbiomarkersthatcouldpredicttheresponsetocombinationtherapy.Thiswouldenableclinicianstoidentifypatientswhoaremostlikelytobenefitfromthetreatmentandtailorthetherapyaccordingly.

Inconclusion,whilethecombinationoflosartanpotassiumandYishenHuashigranulesappearstobeapromisingtreatmentforstageImembranousnephropathy,furtherstudiesareneededtoconfirmthesefindingsandexploretheunderlyingmechanismsofaction.Withcontinuedresearch,itispossiblethatthistreatmentcouldbecomeanimportanttherapeuticoptionforpatientswithmembranousnephropathyInadditiontovalidatingtheeffectivenessofthecombinationtreatment,futurestudiescouldalsoinvestigatepotentialsideeffectsorinteractionswithothermedications.Itisimportanttomonitorpatientscloselyandeducatethemonpropermedicationuseandpotentialadverseeffects.

Furthermore,othercomplementarytherapies,suchasacupunctureordietarychanges,couldalsobeexploredinconjunctionwiththelosartanpotassiumandYishenHuashigranulestreatment.Integrativemedicineapproaches,whichcombineconventionalandcomplementarytherapies,haveshownpromiseinmanagingchronicconditionsandimprovingoverallhealthoutcomes.

Lastly,itisimportanttoaddresstheunderlyingcausesofmembranousnephropathy,suchasautoimmunediseaseorinfections,inordertopreventordelaydiseaseprogression.Patientsshouldbeencouragedtoadoptahealthylifestyle,includingregularexerciseandabalanceddiet,andseekappropriatemedicalcareforanyunderlyingconditions.

Insummary,thecombinationtreatmentoflosartanpotassiumandYishenHuashigranuleshasthepotentialtobeapromisingtherapeuticoptionforpatientswithstageImemb
