2HSEPlanHSE管理方案PrincipleandAimofHSEPlanHSE管理方针、目的Principle:Humanshallbetheroot.SafetyFirst.PreventionisthemajorMeasurement.方针：以人为本，安全第一，防止为主。Wewillprovideasafeworkingconditionforworker.Theenvironmentharmlessconstructionmethodshallbeused.我们承诺为作业人员提供一个安全的作业环境，采用对环境没有损害的施工方法。Aim目的Toavoiddeathaccident,seriousinjury,fireandotherserioussafetyaccident.杜绝死亡事故、重伤事故、火灾事故和其它重大安全事故。ZeroAccident,ZeroInjury零事故，零伤害。NoHarmandContaminationonEnvironment不损害、污染环境。Payattentiontohousekeeping,materialshallbestoredinorder.文明施工、现场整洁，材料堆放整齐。Canteen,bathroom,toiletandcampforCNCECpersonshallbeclean.职工食堂、浴室、厕所、住所符合健康、清洁规定。HSEManagementOrganizationHSE管理组织机构HSEmanagementcommitteewillbeestablishedforprojectteamwhichtheprojectmanagerandsitemanagerwillbethechairman.ThememberofHSEmanagementcommitteeisconstructionmanager,safetymanager,departmentmanagers，constructionteamleader（engineer），safetyengineer,safetyinspector.Safetymanager,asanassistantofprojectmanager,whoseresponsibilityissuperviseandmanagethesitesafety,housekeeping,environmentprotection.Safetyengineer,safetyinspectorshallbearranged,PleaserefertotheattachedHSEorganizationchart.项目部成立以项目经理和现场经理为首的安全生产领导小组，小组成员有施工经理、安全经理、部门经理和施工队长（工程师）、安全工程师、安全员。项目部设立安全经理，协助项目经理对施工现场安全生产、文明施工、环境保护、职业健康工作进行有效的监督、管理。安全经理下设专职安全员、HSE管理组织机构图（见附图一）。MonthlymeetingwillbeheldbyHSEmanagementcommitteetosolvetheproblemduringinspection,toconsidercountermeasure,tochecktheun-completeditemduringinspectionoflastmonthandotherissuesrelatedtoHSE.HSEmassmeetingwillbeheldbysafetymanageroneveryweek,allthesafetymanagementstaffshalltakepartinthemeeting.Intheconstructionpeaktime,thetoolboxmeetingshallbeheldeverydaytosolvetheproblemassoonaspossible.HSE领导小组应进行每月一次例会，会议议程涉及检查中发现的问题和规定采用的措施，上月检查未完毕的事项和其它与HSE相关的问题。项目部每周一上午举行全体职工参与的安全例会（在施工高峰期每日召开安全碰头会，及时解决安全问题，消除生产安全隐患）。HSEManagementOrganizationChartHSE管理组织机构图CC-7ProjectManagerGuoLinCC-7ProjectManagerGuoLinCC-7FieldManagerYangJunMeiCC-7ViceFieldManagerZhangXianQiCC-7SafetyManagerLiuXiaoNongSafetyTrainingEngineerSafetyEngineerForScaffoldingsSafetyEngineerForEquipmentsAreaSafetyinspectorsTCCSafetyManagerYinQingboConstructionManagerShanDongyanTCCProjectManagerYangKejianTCCSiteManagerYangXiaokangAreaSafetyinspectorsMoreSafetyinspectorsHSEResponsibilityHSE生产责任制ProjectManager’sHSEResponsibilities项目经理HSE职责ProjectmanagershallbethemajorresponsiblepersonforHSE.项目经理是项目部HSE工作第一负责人。Projectmanagershallorganize,design,controlandcoordinatefortheprojectHSEwork.组织、策划、控制与协调对本项目的HSE管理工作。ProjectmanagershallestablishtheHSEmanagementsystem,organizetherelativepersontopreparetheHSEmanagementrulestoensuretheconstructionandthehousekeepingcanbeperformedinsafecondition,sothattoreachtheHSEmanagingaim.负责建立项目部HSE管理机构、组织制定HSE管理制度，保证安全生产、文明施工，达成预期的管理目的。Projectmanagershallarrangeenoughcost,manpowerandequipmentforHSEmanagingwork.贯彻项目HSE管理所需的资金、人员、设施等资源。Projectmanagershallgivetheinstructiontocarryouttheemergencyplanwhiletheemergencyincidentoccurred;shallorganizethesalvageandcooperateforinvestigation.发生紧急情况后启动应急救援预案，组织抢救并配合调查。FieldManager’sHSEResponsibilities项目现场经理HSE职责FieldManagershallbeasassistantofprojectmanager,andheisauthorizedpersontoperformtheprojectmanager’sHSEresponsibilitieswhileprojectmanagerisabsent.协助项目经理工作。在项目经理不在项目部时，履行项目经理HSE职责。ConstructionManager’sHSEResponsibilities项目施工经理职责ConstructionmanagershallresponseforthesiteHSEmanagingworkunderprojectmanager’sleading.在项目经理的领导下对施工现场的HSE管理工作负责。ConstructionmanagershallresponsefortheHSEplanorganizationandexecution.Constructionmanagershallmaketheschedule,arrangement,inspection,summarizingandappraisingthroughcomparisonforconstructionwork,meanwhile,thesameworkshallbedoneforHSEbyconstructionmanager.组织实行HSE管理规章制度，在计划、布置、检查、总结、评选生产时，同时计划、布置、检查、总结、评选HSE工作。Constructionmanagershallpushtherelativepersonforthesafetypotentialdangerousandstoptheinfringedwork.督促安全隐患的整改，制止违章作业。Incasetheaccidentoccurred,constructionmanagershallgivetheinstructiontoprotecttheaccidentarea,shallparticipatetheaccidentinvestigation,andshalltakeactionofcountermeasure.发生事故后，组织保护现场，参与事故分析，贯彻防范措施。TCCSafetyManager’sResponsibilitiesTCC安全经理职责Safetymanagershallsuperviseandmanagethesiteconstructionsafety,housekeeping,environmentprotective,andoccupationhealthworkunderprojectmanager’sleading.Healsoshallensureallsiteworkactivitiesmeettherequirementonhealth,safetyandenvironmentprotectivebyEPCLPVC,CNCECandlocalgovernment.HSEinspectionshallbeorganizedandexecutedbysafetymanager.SuitableHSEplan,rulesshallbepreparedbysafetymanger.SafetymanageralsowillsupervisethePPEissuingandusage.Incasetheaccidentoccurred,safetymanagershallperformtheinvestigationandhandling.Safetymanagerisauthorizedtostoptheworkunderthedangerouscondition.在项目经理的领导下，负责对施工项目安全生产、文明施工、环境保护、职业健康工作进行有效的监督和管理，保证在现场进行的所有工作符合业主、公司、本地地方政府对健康、安全、环保的标准规定。组织进行HSE检查，制定适合项目的HSE管理制度、管理方案，监督劳动防护用品的发放和使用。组织事故调查和解决。在危险危急情况下有权发布停工令。CC-7SafetyManager’sResponsibilitiesCC-7安全经理职责ToimplementtheHSEprocedurestrictlyandconfirmresponsibilitiesofsafetyengineerandinspectors,andtheyshouldbesupervisedandmanagedeffectively.Youshouldensuretoimplementthesafetytechnologyduringtheconstruction.ToorganizeHSEchecking.Andheshallnegotiatewithotherdepartmentstomakesurethespecificpersontosuperviseandrectifythehiddenhazards.Meanwhile,it’snecessarytoinvestigateaccidentsandissuetheorderofstoppingworkundertheemergencycondition.严格有效执行安全管理体系，明确安全工程师和安全员的职责，并对其进行有效的监督和管理。组织HSE检查，保证施工过程中安全技术措施的贯彻。与项目其它部门进行有效协调，明确隐患整改人，督促安全隐患及时整改。组织事故调查解决，在紧急危险情况下，有权发布停工令。SafetyEngineer’sResponsibilities安全工程师职责ThesafetytrainingengineerisresponsibleforthetrainingofChinesenewcomersandspecialtypeofwork,andeducatesthemtoredresstheincorrectoperation;thetrainingofemergentrescueandthemanagementofsafetydocument.Safetyengineerforscaffoldingsisresponsibleforcheckingthedesignofscaffolding.Beforeusingthescaffoldings,thesafetyengineershouldhangthetagandsupervisetorectifythehiddenhazard.Safetyengineerforinstrumentsisresponsibleforcheckingandpostingtheequipmentsatthesiteandtorectifythehiddenhazard.安全培训工程师负责全场新进中国人员的入场培训，特殊工种的专项培训，违章操作再教育，紧急救援培训及安全文献资料管理。专业安全工程师负责对脚手架搭设方案审核，脚手架搭设完毕后挂牌验收，督促脚手架隐患整改。专业安全工程师负责现场机具，用电设备检查贴标，督促机具设备隐患整改。Safetyinspector’sResponsibilities专职安全员职责Safetyinspectorsshallresponseforthesafetymanagerasassistantundersafetymanager’sleading.SafetyinspectorsshallsupervisefortheexecutionoftheHSEmanagingrulesandsafetymeasurementintheirownarea.SafetyinspectorsshallparticipatetheHSEinspection.Hecanstoptheinfringedwork.Safetyinspectorisauthorizedtostoptheworkunderthedangerouscondition,andhewillreporttosafetymanagertimely.SafetyinspectorshallstudytheHSEmanagingdevelopmentandsubmitthesuggestionandmeasurementforimprovement.Safetyinspectorshallparticipateororganizetheinspectionworkforrelatedsafetyprotectivefacilities.SafetyinspectoralsowillsupervisethePPEissuingandusage.Monthlystatistic,analyzeandreportforHSEmanagingshallbeperformedbysafetyinspector.Theareasafetyinspectorshouldinsistoncruising,supervisingandmakerelevantrecordseveryday.监督HSE管理制度和安全技术措施的贯彻。对各自区域进行HSE检查，制止违章作业。在危险危急情况下有权发布停工令，并及时报告安全经理。了解HSE管理动态，提出改善建议和措施。参与或组织对有关安全防护设施的验收工作。监督劳动防护用品的发放和使用。对HSE管理情况进行月度记录、分析、上报。区域安全员应坚持天天现场巡查和监督，并作好相关记录。ConstructionEngineer’sHSEResponsibilities工程技术人员HSE职责ConstructionengineershallexecutetheHSErules.认真执行HSE规章制度。Whileengineerpreparetheconstructionmethodstatement,therelativesafetymeasurementshallbeprepared.编制施工方案的同时，制定有针对性的安全技术措施。Safetyclarificationshallbegiventoworkerbyengineer.Anditmusthavewrittenrecords.进行安全技术交底，交底应有书面记录。Constructionengineershallsupervisetheworkerscomplyingtheconstructionmethodandoperationrules.Constructionengineercanstoptheinfringedwork.督促作业人员严格执行施工工艺及操作规程，制止违章作业。Constructionengineershallparticipatetheinspectionworkrelatedthesafetyprotectivefacilities.参与有关安全防护设施的检查验收工作。Constructionengineershallparticipatethesafetymeasurementpreparationworkforthenewtechnical,newmaterial,newconstructionmethodandnew参与制定新技术、新材料、新工艺、新设备相应的安全技术措施。Exceptaboveresponsibilities,electricalengineeralsoresponseforthefollowingitems.a.toprepare“MethodStatementforConstructionTemporaryElectrical”;b.Hewillattendthesafetyinspectionforconstructiontemporaryelectricalwhichwillbeheldonceamonth.Hewillresponsefortherectificationworkregardingthepotentialdangerousofconstructiontemporaryelectrical.电气技术人员除了履行以上职责，还应履行：a.编制“临时施工用电方案”；b.参与每月一次施工用电安全检查，并负责对施工用电安全隐患的整改。Thetechniciansandcaptainmustapplytotheareasafetyinspectorbeforetheyarereadytoworkunderthesafecondition.Thedetailcanbereferredtotherequirementforworkpermit.工程技术人员、队长在进行作业许可规定的作业前，必须向区域安全员申请，区域安全员在保证安全措施的前提下，进行批复后才可作业。具体参见作业许可规定。Worker’sHSEResponsibilities作业人员HSE职责Workershallstrictlycomplywiththecompanyandprojectteam’sHSErules.Workershallperformtheconstructionworkaspersafetyrequirement.Workercanstoptheinfringedworkandrefusetheinfringedinstruction.Workershallcarryoutthehousekeeping,complytheHSEmanagingandcommand.Anyaccidentorpotentialdangerousisdiscovered,itshallbereportedtorelativeperson.严格遵守公司、项目部制定的HSE管理规章制度。严格按安全规程实行作业，有权制止违章作业，有权拒绝违章指挥。文明施工，服从HSE管理和指挥。发现事故或事故苗头，立即报告上级有关人员。HSEManagementPrinciple项目HSE管理原则“Humanshallbetheroot.SafetyFirst.PreventionisthemajorMeasurement.”isprincipleofHSEmanagement.“Anyonewhoresponseforconstruction,hewillalsoresponseforsafety.”istheprincipleofHSEmanagement.BothprincipleandprincipleshallbecompliedforprojectHSEmanagement.项目HSE管理必须坚持以人为本，安全第一，防止为主的方针和管生产必须管安全的原则。“SafetyFirst”,itsmeansthatsafetyworkistheprimaryimportantworkandthepriorityworkduringconstruction.“PreventionisthemajorMeasurement.”itsmeansthattherelativecountermeasureshallbetakenagainstthepotentialdangerouswhilearrangingconstructionwork.Constructionworkcannotbecarriedoutunlessthesafetyprotectivemeasurementhasbeenexecuted.安全第一，就是规定在进行施工活动时，把安全工作放在一切工作的首要位置，实行安全优先的原则。防止为主，就是要在安排、布置施工任务时，针对施工中也许出现的危险、危害因素，采用防止措施予以消除，实行安全防护措施不贯彻不开工的原则。Fallingdownfromheight,hittingbyobject,injurybymachine,collapseandelectricalshockisthemostlikelyoccurredaccidentduetothesiteconditioniscomplexwithlargequantityworker,constructionmachineandmaterial,crossworkbymulti-discipline,someworkatopeningarea,variousworkingcondition,manpowerandmachinepositionchangedfrequently.Therefore,tocontroltheun-safeactivityofmanpowerandun-safestatusofobjectisthekeypointofsafetymanaging.“Humanshallbetheroot”shallbekeptineverybody’smind.HSEshallbecontrolledbyeverybody,ineverywhere,duringwholeprocess.Tocreateagoodsafetyconstructioncondition,andtoreachtheHSEmanagementaim,therelativeresourceshallbecost.Therelativeresourceincludeprofessionalsafetyinspectorarrangement,doctor,firstaidpoint,firstaidboxandscaffoldingarrangement,plankplacing,fireextinguisherarrangement,PPEprovisionandsafetynetinstallation,etc..HSEplanwillbearrangedandcheckedaccordingtotheconstructionschedule.SafetymanagerwillattendthecoordinationmeetingbetweenEPCLPVCandCNCECforthesafetyissuescoordination.施工现场作业人员密集，施工机械、材料集中，多工种立体交叉作业多，露天作业多，作业环境多变，人员、机具流动性大，高处坠落、物体打击、机械伤害，坍塌、触电是容易发生的事故，控制好人的不安全行为和物的不安全状态是安全管理的重点。必须坚持“以人为本”的管理思想，进行全员、全方位、全过程的动态控制，调动全体人员参与HSE管理，形成良好的安全文化氛围。要发明一个良好的安全环境和氛围，实现HSE管理目的，必须投入相应的资源。这些资源涉及：配备专职安全员，配备专职医生和急救站，急救箱，脚手架，跳板铺设，消防灭火器的配备，PPE配备及安全网铺设等。施工HSE计划随着施工计划进行安排和检查。安全经理将出席EPCLPVC和项目部协调会，在会上对HSE信息进行沟通。HSEResponsibilitiesExecutionandManagement贯彻HSE责任制，实行责任管理ThecoreofHSEresponsibilitiesprincipleistoachieve“5safetywork(maketheschedule,arrangement,inspection,summarizingandappraisingthroughcomparison)shallbedonewhileexecutingthe5sameworkforconstruction.”.EachsubcontractorshallsignforthesafetyconstructionagreementwithCNCECprojectteam(orthesafetyconstructionandhousekeepingrequirementincludedinthecontract),sothat,thesafetyconstructionandhousekeepingcanbeeffectedbycost.Theriskcostforsafetyconstructionandhousekeepingshallbe5%oftotalamountofsubcontract.IfthesubcontractorcannotreachtothesafetyandhousekeepingtargetinstructedbyCNCEC,thenthe5%riskcostshallbededucted.Incasesomeinfringedthingwasdiscovered(suchasconstructionmaterialplacedeverywhere,constructionrubbishcleaningun-timely,safetynetarrangementisn’tmeettherequirement,airbreakandflammableexplosivematerialplacingisn’tmeettherequirement),rectificationisrequiredtimely;otherwise,1%oftotalamountofsafetyandhousekeepingriskcostshallbedeductedforonce.安全生产责任制，其核心是要实现安全生产的“五同时”：在计划、布置检查、总结、评选的时候，同时计划、布置、检查、总结、评选HSE工作。各分包单位与项目部签定安全生产协议（或承包协议书上必须有安全、文明施工指标），使安全生产、文明施工与经济责任制挂钩。安全生产、文明施工风险金占分包协议总额的5%，分包单位未完毕项目部下达的安全、文明施工指标，扣除责任单位5%风险金。施工材料乱堆乱放，建筑垃圾不及时清理，不按规定设立安全网，漏电保护器，易燃易爆物放置不符合规定等，经指出的不及时整改的，发生一次扣安全生产、文明施工保证金总额的1%。HSEEducation&TrainingHSE教育与培训AllthepersonneltobemobilizedshalltakepartinsafetytrainingandeducationorganizedbyOwnerandprojectmanagingdepartment.OneprofessionalsafetyengineershallbedesignatedforHSEeducationandtrainingwork.HSEtrainingshallbegiventoallpersonwhowillenterthesite.Moresafetytrainingandeducationsuchassafetyrequirementforexactlyworkandsafetyoperationprocedureshallbegivenbyforemanbeforestartingthework.EPCLshallresponsefortrainingandeducationforlocalworker.ProperPPE,IDcard,uniformshallbeissuedtoworkers.项目所有进场人员，除参与业主的安全培训和教育外，还要参与项目的安全教育。项目部设一名专职安全师负责对职工进行HSE教育、培训工作，进入施工现场的人员，必须通过项目部安全培训；到作业班组后由班组长再进行本岗位安全注意事项和安全操作规程等培训教育。配备PPE，统一工作服，持工作卡上岗。应特别注意对本地雇用民工，由业主进行安全教育和培训获得入场资格同时按CNCEC规定，配备统一工作服，PPE并颁发工作卡后才干入场工作。现场违章人员，于当天下午接受安全教育培训，提高安全意识。职工经培训教育后登记备案，对违章人员要进行登记。HSEEducationandTrainingContentforNewPerson.新进现场人员HSE培训教育内容：ProjectSafety,Environment,ImportantEnvironmentFactor&DangerousResource,Countermeasure项目安全、环境特点、重大环境因素和危险源、防止措施。HSEManagementRules,Housekeeping,UsageofSafetyDeviceandPPEHSE管理规章制度、文明施工、安全装置、个人防护用品的使用。EmergencyPlanInCaseAccidentOccurred事故应急救援。CNCECwillarrangedthreesetsofTVandonesetofDVDtodisplaysomemoviesanddocumentsregardingthesafetyissueforgivingstaffandworkervarioussafetytraining.ProjectorshallbeusedtoplayingthesafetyeducationPPTforthepersonswhichshallenterthesiteandparticipateinspecialconstruction.项目部配备三台电视机，一台DVD播放机，可以从国内购买一些安全宣传电视节目和安全宣传资料，准备入场安全教育，特种作业人员安全教育，专项培训等PPT，用投影仪进行播放，对作业人员进行形式多样的安全教育培训。HSEAwardandPunishmentHSE奖励与处罚HSEawardandpunishmentrulesshallbeestablishedforsiteconstructioninordertostrengthenthesafetyandhousekeepingmanagementandtoavoidserioussafetyaccidentoccurred.为了加强安全生产、文明施工管理，贯彻安全、文明施工责任制，有效防范重大伤亡事故的发生，文明施工，特制定施工现场安全生产、文明施工奖励与处罚规定。HSEAward奖励HSECheckListshallbe:HSEmanagingsystem,standard,rulesandqualitymanagingmeasurementareallperfect.Death/GBHAccident=0OccupationalDisease=0EnvironmentalPollutingAccident=0Fire/ExplodeAccident=0PropertylossAccidentwhichvalueisupperthanRMB50000=0Therecordedaccident≯1(per202300MH)Iftherehasnohereinbeforeitemhappens,thesafetybonusshallbeawardedmonthly.HSE考核指标如下：a.HSE管理体系完整，制度标准健全，质量管理措施贯彻到位；死亡/重伤事故=0；职业病=0；环境污染事故=0；火灾/爆炸事故=0；超过5万元的财产损失事故=0；可记录事故≯1（每20万人工时）。所有达标后，将按月发放安全奖。具体额度现场制定。Incase“ZeroAccident”targetisreachedineverythreemonths,everyhalfanyear,everyyear,every500thousandman-hours,every1millionman-hours,someactivitieswillbeheldforcelebration.ThedetailsshallbedecidedbyHSEmanagementcommittee.每季度、每半年、每年、每50万工时、每100万工时施工无事故，项目部将适时组织参战职工进行庆祝，发纪念品或奖金。具体活动将由HSE领导小组讨论决定。HSEawardshallbegiventosomepersonsforeachconstructionteamineverymonth.Thepercentageofawardpersonsis2%.HSE领导小组每月在现场评出安全、文明施工先进个人，人数占总人数5%，给予物质奖励，并通报表扬。Somepersonwhocomplythesafetyrequirement,stoptheinfringedworktimelyandavoidtheserioussafetyaccident,willbeawarded.对于在施工中遵章守纪，及时制止违章作业，避免了重大安全事故发生，项目部给予物质奖励，并通报表扬。PunishmentfortheInfri
