SECTION 8.0 SAFETY/ENVIRONMENT PLAN1 general1.1 PurposeHealth, Safety and Environment (H.S.E.) is always a matter of highest priority and concern. For Repair/Replace of Damaged Pipe Supports at NGL Jetty, Mesaieed, Qatar, the regulation of Health Safety and environment (H.S.E.) will be implemented to protect the whole facilities, employees, other relevant QGPC personnel, service and property of the third party.1.2ScopeIt will be implemented by all the PERSONNEL who are involved in the WORK, procurement, overhaul and commissioning of the Project during the execution of the Project. It will be responsible for having a documented safety program including written policies and procedures designed to protect the safety, environment and health of employees to ensure that all CONTRACTOR PERSONNEL are familiar with the content of the safety program, including policy statement and employees orientation plan.2 ORGANIZATION AND RESPONSIBILITY OF HSE DEPARTMENTThe HSE Dept. will be set up under the leadership of the Project Director which carries out and superintendents the WORK with the assistance of QGPC.2.1 Personnel Selection(a) PositionA Dept. Manager, a number of coordinators for health protection, health promotion, safety protection, safety promotion, as well as HSE Auditor, as the functional management.(b) Personnel selectionSelect the personnel with rich experiences as the HSE Department manager, submit his resume to Project Director(s) for discussion. Coordinators shall be responsible for the HSE management.2.2 HSE Dept. Responsibilities(a) implement various HSE works, under the leadership of Project Director,(b) Make the detailed HSE plan and carry it out.(c) Organize constant HSE of QGPC training for all employees. Implement HSE plan of QGPC.(d) Conduct health assessment and safety examination and coordinate with Project Director to settle any HSE problem during the execution of the project.(e) Provide the First Aid and suitable medical facilities.(f) Have health & safety insurance on personnel and safety insurance for mobile equipment and accidents.(g) Monitor sickness absenteeism, keeping records for the employees and the treatment of accident.(h) Draw up all applicable specifications and regulations which will be followed by all employees, according to QGPCs HSE requirements and QGPCs security guidelines for personnel in-transit to off-shore installations, to ensure the security of off-shore operation in respect of personnel, assets and information against ill-legal acts. (i) Be responsible for the training of employees in terms of laws, disciplines, regulations. Employees shall comply with the local laws and various regulations drawn up by the HSE management department. (j) Be responsible for training employees in marine survival craft and for taking precautions against and controlling pollution in the oil and gas field during offshore, onshore and ecological monitoring.(k) Be responsible for coordination with QGPC Security Dept. to search private belongings unaccompanied prior to transporting to offshore site. (l) According to requirements of QGPC and HSE rule of QGPC, be responsible for drawing up the detailed guideline of all lifting equipment and mobile tower appliance and carry it out.2.3 Responsibilities of HSE Dept. Manager(a) Deal with the whole HSE affairs during the execution of the WORK.(b) Coordinate with Project Director to ensure to practise HSE on the employee.(c) Be responsible for the comprehensive management of HSE Dept., especially for health protection and health promotion, safeguarding, labor protection, overhaul monitoring and examination and marine operation safety, specially diving craft.(d) Coordinate with engineering manager, procurement manager, overhaul manager, and etc. to ensure the health & safety measures have been fully considered during the execution of the WORK 2.4 Responsibilities of Coordinators(a) Be responsible for organizing personnel to study relevant laws and regulations of Qatar.(b) Be responsible for health & safety education to the personnel, including Personal Protective Equipment (P.P.E), which shall be worn at all times and covers the following:a) Safety Foot Wearb) Safety Helmetc) Safety Spectacles/Gogglesd) Grinding Face Shieldse) Welding Hood/Maskf) Hand Glovesg) Ear Plugs/Mufflerh) OverallsSpecial P.P.E may be required and will be supplied.(c) Be responsible for investigation and report of occupational sickness and accidents. (d) Be responsible for summary, comparison and assessment of HSE work.(e) Report to Project Manager regularly.(f) Analyze health & safety condition comprehensively every month.Do a report in writing.(g) File various sickness & accident information, fill sickness & accident in report forms and draft relevant precaution measures. (h) Be responsible for constant health & safety education to personnel. Put various health & safety production management system into effect.(i) Organize health & safety examination, in case of finding unhealthy or unsafe elements which endanger employees and equipment, supervise the work conditions. In case of emergency, stop the work and report to HSE Dept. Manager.(j) Be responsible for issuing the permit to work system, such as excavation permits, hot work permits, dive permit, discharge water quality permit and implement of disposal criteria, etc.(k) Supervise the job with health & safety regulations, and safety & safety technical measures, and annunciation of contaminations and marine convention.(l) Be responsible for investigating, analyzing, treating of the health problems and accident and follow up.(m) According to the demands of project, draw up the subsidies granting standards of protection articles, health care, heatstroke prevention. Draw up the procurement schedule of labor protection (health & safety) articles, and put it into effect. Take responsibility for heatstroke prevention in the work force.(n) Coordinate with relevant departments, for industrial sanitation operations.(o) Organize regular physical examination to personnel who go in for special job ( e.g. high altitude, high temperature, etc.)(p) File for industrial sanitation when required.3 Health Risk & Safety Management Main Contents3.1 Draft Health & Security Work Plans(a) base on overhaul contents, site environment, climate, QGPCs regulations, etc.(b) include health & safety training plan; health & safety technical regulations and demands in accordance with overhaul contents; special health & safety measures (e.g. first aid, in accordance with special overhaul conditions); security plan; measures of handling with dangers or other problems.(c) monthly or weekly work out relevant health & safety requirements in accordance with the need of overhaul.3.2 Health & Safety Training and Indoctrination(a) Project personnel will get health & safety training prior to taking position.(b) Three Levels of Health & Safety Training Level I: by HSE engineer on relevant laws, regulations and QGPCs policies and rules.Level II: by site HSE coordinator on site work scope, work characteristics, demands of health & safety operation and the use of safety equipment, emergency appliances, e.g. first aid, etc.Level III: by foreman on position condition, the characteristics of work and position scope, regulations and warning, the functions and use of tools and health & safety appliances, and the use and storage of labor protection appliances articles. Draft out the design of marine contamination and emergency crude oil leaking out tank. (c) Special health & safety indoctrinationPrior to the use of new technique, equipment, product, and etc., health & safety Manager shall draw up regulations of safety technical operations, and be responsible for the education at the same time.(d) Constant Safety Training Group leaders or foremen shall address health & safety everyday before work; Work teams shall summarize the health & safety performance once a week; Project Group shall hold a health & safety meeting every month.(e) Prior to overhaul, HSE Dept. organizes health & safety education to personnel at all levels. All the persons who pass the examination can take part in the overhaul. Health & safety training register for personnel is kept on record.3.3 Health & Safety Management on site(a) Health & Safety inspection of all activities including excavation and scaffolding, air freeing, desludge and welding , diving and third party safety on site, etc. Health & Safety inspection requirements- Health & Safety Dept. organizes regular health & safety examinations, at least once a month.- To each field production facility as well as the gathering line, project team organizes health & safety examinations, at least twice a month.- Site health & safety coordinators will examine the site every day, identify instructions and operations contrary to rules and regulations, rectifying and addressing the situation.- Accept superintendents examinations of overhaul sites, living camps at any time, eliminate the harmful & unsafe factors pointed out by superintendent at the time. Content to be examined- Health & safety rules, specifications, regulations, management actions, hidden dangers, unhealthy factor, and the implementation of plans and regulations. Means of safety examination.- Assess, look, listen, smell, ask, check, test and analyze.(b) Health & Safety requirements for air freeing and desludge Be responsible for establishing and implementing the detailed “tank cleaning/gas freeing procedure of the operation procedure guide-production department. All equipment must be earthed to eliminate the contact with electricity, organize constant procedure safety for electricity. Inspection must be made for cleaning the manhole on the roof. Protective clothing (e.g. Rubber boots, hard hats, gloves etc.) must be worn at all time. Safety inspector and foreman will always wear an audible gas alarm, and monitor H2S at any time, and carry out the work in accordance with the approved guideline and safety regulations. Be responsible for that maximum allowable time of working inside the tank during the early stages of desludging , and to be specified on work permit and must be strictly followed. Safety supervisor must be familiar with tank desludging, gas freeing, safety procedures and can operate safety equipment including resuscitation / revival equipment and BA sets, fully aware of the hazardous nature of the WORK . Review and inspect resident engineer shall ensure that desludging work is carried out technically correctly and complies QGPC General Safety Regulations and those specifically mentioned on the work permit. Production initially declare the tank gas free and safe to work and issue a gas free certificate upon the completion of desludging works, Also regularly carry out gas level checks as stated on the permit.(c) Waste controlling management Waste management incorporates the storage, collection and disposal of solid waster as well as residues from the treatment of waste water, sludge ,demolition materials, debris, outside package and rubbish. All wasters shall be disposed. Classify the materials into three broad levels, depending on biological, chemical, physical toxic and infection Characteristics. On site, containers shall be chosen for the specific purposes and shall be equipped with tight fitting lids (except those containing non-hazardous solid materials which shall not be blown away by the wind.) Debris and demolition materials shall be removed to the solid rubbish treating centre as early as possible. Hazardous rubbish must be stored in an adequately designed receiving facility with impervious flooring, roofing, and suitable drainage control. Rubbish shall not be accumulated on site. The monthly average contents shall never exceed 75% of the designed capacity. Materials shall be stored in order with clearly identified labels. Supply the evidence of the safe and acceptable disposal of the rubbish to the appropriate authorities and the Environmental Affairs Department. Register the quantities and characteristics of the rubbish, including origin, type, quantities and landfill locations. Monitor hazardous component of the rubbish and testing the leaching characteristics of hazardous rubbish. Accept the examination of the state of Qatar and the third party.(d) Waste Water controlling managementThe process water from the workshop, the waste water from the desluging and at site shall be disposed. Waste water of different character shall be segregated to the ultimate extent. Uncontaminated surface runoff will be discharge to receiving water without treatment. Waste water and polluted water shall be collected together to drain out. Under the requirements of the Environmental Affairs Department, monitoring the waste water, including: PH, BOD5, Chemical Oxygen Demand, Total Suspended Solid, Oil and Grease, Phenol, Cyanide(total),Arsenic, Cadmium, and Total Coliform, etc.(e) Health & Safety requirements of Lifting Equipment Before the transportation of heavy equipment, the carrier shall fully understand the specifications of the equipment (such as weight, shape and dimensions, and etc.) and shall check the bearing weight of bridges, width of roads and distance between crossover facilities and the road surface; and work out a transportation plan for review and approval by the HSE Dept; and submit it to superintendent seven (7) days before the execution. The representative of the Superintendent shall reply it in writing within three (3) days after receiving such a report. Any damages to the facilities along the way resulting from the transportation of heavy equipment must be immediately repaired and restored to the original conditions by CPECC.(f) Fire Fighting Management Appoint an experienced fire-fighting person as the charger, in accordance with the responsible system , on every overhaul site and living area. Set up the fire-fighting management system. Regularly check helideck foam system, fire detection system, gas detection system. Offer the fire-fighting education to the overhaul personnel. Study and carry out the fire and safety regulations of QGPC. Provide enough extinguishers, and assign persons for maintenance to ensure the equipment in good condition. The temporary power transmission lines in the living area, office area, overhaul site must meet the requirements. Every individual electrical appliance on overhaul site shall be installed with one single switch. The height of switch box (board) should be up to no less than 1200 mm from the ground to keep from rain and damp, or electrical appliances must have earthing connections which shall be tested regularly. Site materials shall be kept in good order. Combustible, corrosive articles and poisonous articles shall be stored in special containers or where meet the relevant safety demands. Oxygen cylinders and acetylene cylinders shall not be confused. All containers contents to be identified. In overhaul areas, smoke is not allowed and safety signs shall be posted. On any overhaul site, attention should be put on examination of tinder and good housekeeping at all time. Evacuation- In case of emergency, contact QGPC emergency services and report to HSE Dept. Manager. Follow emergency response plan.- Organize overhaul employees to evacuate the area.- Organize putting out fire, secure employees and properties, first aid and medical rescue, maintenance of public orders, etc.- Pay close attention to the developments around site preventing equipment access.- Treatment of injured persons(g) Health & safety requirements for excavation overhaul Comply with QGPC Regulations, apply to superintendent for overhaul three days earlier, carry out excavation overhaul after permit is issued. In accordance with Particular Conditions of Contract, during survey, install correct signs for underground hidden facilities (pipe, cables, etc.) and follow drawings provided by QGPC. During excavation overhaul, if damage occurs to the original facilities of QGPC, the constructors shall be responsible for the expeditious repair, however, if the damaged facilities are not shown in the drawings provided by QGPC, the cost of repair shall be paid by QGPC, in accordance with Particular Conditions of Contract.(h) Health & Safety requirements for hot work system Comply with QGPC regulations on application for permit to work as detailed in the contract, with attention to time scale and associated work scopes. Prior to overhaul, electrical equipment will be examined carefully to see whether it has leakage or short circuit or not, and the condition of safety ground connection is good or not. Provide enough fire fighting equipment on site at all times. Persons having no direct contact with the WORK must be kept away from the WORKSITE. Operators shall obey the safety rules and regulations, and check the WORKSITE at any time. While completing hot work, ensure that there is neither hidden fire danger nor unhealthy element left. Check the WORKSITE at any time. Provide for compliance with all QGPC emergency and evacuation procedures and QGPC representative instructions in the event or any emergencies.(i) Transportation of Heavy Equipment Before the transportation of heavy equipment, the carrier shall fully understand the specifications of the equipment (such as weight, shape and dimensions, and etc.) and shall check the bearing weight of bridges, width of roads and distance between crossover facilities and the road surface; and work out a transportation plan for review and approval by the HSE D
