It should be noted that every contractor has a legal obligation to provide safety in the work place both for his employees and for any other person and to ensure compliance with environmental legislation.此手册确立了承包商于雇主的项目上所需具备的EH&S要求。承包商在项目上必须依法对其员工及他人提供一个安全的工作环境。1. EH&S Policy and Plans EH&S 规定及计划1.1. The contractor, who will work with Employer, shall agree that they will conduct their work according to the Chinese national regulations, local government regulations, Employers EH&S requirements (e.g. and specific safety standards set for the project (available from Employer).作为 雇主的承包商, 在整个承包过程中必须遵守国家和地方的有关 法律, 雇主的要求规则 (可向雇主索取)。1.2. The contractor must present as part of his tender an EH&S Plan and shall incorporate, but not be limited to, the following general requirements.EH&S的计划必须作为标书文件的一部分,此计划必须包括但不仅限于下列内容:l Description of the roles and responsibility and organization chart of the project team. 项目组人员的职责及组织机构图。l An assessment of EH&S risk categorised as Class 1, 2 or 3 for each activity to be carried out by that trade or contract and reviewed by EMPLOYER (see Classification of Risk Severity section). 根据其工种评价其危险等级,按1、2、3等级对每项具体工作划分并提交雇主审核 (见危险等级章节)。l Detailed Safe Work Method Statements and procedures to ensure any activity that has a potential risk of an accident occurring is controlled. 制定详细的安全施工方案,确保每项有危险隐患的工作有控制措施。l Inspection and test plans must accompany Safe Work Method Statements. 在安全施工方案中应包括检查及测试计划。l Provide Waste Management Plan. 提供废物管理计划。l Provide additional Environmental Plans where needed (e.g. sedimentation controls, dust, noise etc). 根据需要提供相应的环保计划(例如沉降控制、防尘及防噪音等)。2. Contractor EH&S Responsibilities 承包商环境、健康及安全责任All contractors performing construction activities on the site, whether under contract with Employer or the Employer, are responsible for compliance with all regulations and rules set for the project and are DIRECTLY RESPONSIBLE for the safety of their employees and their contractors employees. Responsibilities for contractor include:无论是在雇主或是雇主合同下的承包商在现场进行施工时,都应遵循项目所设立的所有规则条例,并直接负责其自己员工及其下包员工的安全,责任包括:2.1. Provide a written safety plan to Employer. The safety plan must be site specific and identify the particular hazards to / of their trade and provide precautions to perform the work safely. The plan must be submitted before any work is performed on the jobsite and must include provisions for implementation on the project for work to be conducted by any additional contractors.向雇主提交书面的安全方案,此方案必须是专门针对现场的,要指出对涉及到的危险及危害所做的防范措施,使其能安全工作。此方案必须在现场工作开始前提交,且包括对为完成项目而由其他下包进行工作的规定。2.2. Ensure that sufficient resources are allocated to achieve the site EH&S requirements and responsibilities for own employees and site supervisory staff are assigned.应确保为满足现场安全卫生要求所需的资源,并对其员工和现场监工的责任进行明确分配。2.3. Designating enough number of jobsite Safety Officers (one per 50 workers in China) to be responsible for implementation of their Safety Program, to co-ordinate with Employer, to maintain records, and to organise and participate in training sessions. The Safety Officer provided by the contractor must be certified by the government or by organizations that are accredited by the government. 分配足够的安全员(在中国,每50个工人有一个安全员)负责执行安全计划、配合雇主记录、组织并参加培训。超过50人(含50人)承包商所指定的安全员必须是由政府认可或由政府的组织机构证明合格的人员。少于50人承包商需配备经过安全部认可的安全协调员。2.4. Attend safety inspection and safety meetings as required by Employer. Also be part of the E.R.T.根据雇主要求参加安全检查和安全会议以及紧急反应小组。2.5. Conducting regular safety meetings and Toolbox Talks for all their employees on the jobsite, submitting records to Employer as required and maintaining records of all such meetings.对现场所有员工进行安全例会及每天的班组安全会议,根据要求向雇主提交报告并保存所有会议记录。2.6. Performing their work in accordance with Employers requirements and all government fire, safety and health regulations.所有工作的开展必须符合国家法律法规和地方规范以及雇主的相关安全、消防的要求。2.7. Using proper tools for each task and maintaining these tools in a safe operating condition. Contractors will verify that all employees are trained and understand operating procedures before machinery or equipment is used.供应商使用的工具设备使用前必须经过检查,确保设备设施工具完好,符合作业要求。操作人员作业前必须接受过承包商的安全作业交底。2.8. Not permitting the interchange and use of tools and/or materials by another trade without written permission.没有书面许可,禁止使用其他替代工具和材料。2.9. Prior to delivery of any plant and equipment to the Site, provide to Employer a schedule of the details of the plant and equipment, the regulatory operating conditions and the current maintenance and inspection records applicable to that plant and equipment.在任何机具进入现场前,必须向雇主提供机具进场的详细计划,适用于机具的操作手册维护状况以及检查记录。2.10. All equipment such as front end loaders, bulldozers, backhoes and forklifts are to have their bucket or blade lowered when not in use and checked or blocked during maintenance or repairs.所有的设备如装载机、推土机、铲车、升降机在为使用时应将吊筒或者叉齿等降低,在维护或修理时应垫上垫块做好围护。2.11. Providing and enforcing the use of personal protective equipment as required by Employer and government regulations.必须提供符合雇主要求和政府规定的个人防护设备。2.12. Establish a register for and, where required, provide evidence to Employer of all licenses, registrations, orders and approvals, and all renewals thereof required of the contractor for the Works, including those for the disposal of any materials under any occupational health and safety and environmental legislation.建立登记册,如有需要提供给雇主。该登记册包括所有的企业资质证书,安全生产许可证,特种作业资质等,还包括在环境健康安全法规下处理的废物的资质证明。2.13. Checking for and correcting any unsafe practices and conditions (e.g. P1) that exist in the performance of their work, and reporting to Employer any unsafe conditions created by others.检查并消除其工作过程中不安全的因素和情况(如,P1),并向雇主及时报告其他原因造成的不安全因素。2.14. Making a complete investigation of accidents to determine the cause so that corrective action can be taken. In addition, contractors will report all accidents and injuries on a timely basis as defined in the Accident Reporting section.对事故进行全面的调查,确定原因以便采取正确的措施。承包商应及时、准确的汇报所有发生事故报告及整改措施。2.15. Providing appropriate first aid supplies for their employees and also providing personnel qualified to administer first aid as required.为自己的员工提供必要的急救设施并配备有资格的人员进行急救。2.16. If for any reason during the performance of the work, perimeter cables, barricades, floor or roof opening covers, holes covers, or fall protection related items are removed to perform work, it will be the responsibility of that contractor to replace them promptly.如施工时有需要将电缆、路障、地面及屋顶盖板、洞盖及防坠落等临时设施拆除,承包商应负责采取安全替代措施。2.17. Provide materials which are adequately equipped, guarded, protected, approved and serviced on a regular basis and to manufacturers specification so as to maintain safety protection for the contractors personnel, other persons on the Site, members of the public who may be affected by the Works and protection to the environment.提供足够安全设备设施,以保护承包商人员以及其他可能会被施工影响的人员的安全,同时确保环境不受到破坏。2.18. Where the contractors personnel carry out work on the Site outside usual working hours, ensure the attendance at the Site during those hours of at least one of the contractors personnel who is a certified first aid attendant.承包商在正常工作时间内发生意外事件应立即通知医务室进行处理(医务室急救电6435);承包商在非正常工作时间以外的施工,必须在现场配备一名有资质的急救人员,以防紧急情况发生。2.19. Maintain a good general housekeeping in their work area and minimising all combustible debris in their work areas.必须保持工作区域的环境卫生,工完料尽场地清。2.20. Fulfil other requirements as required by Employer.执行雇主的其他管理规定。3. Classification of Risk Severity 危险等级For the purpose of assessing potential risk when compiling Safety Plans, the following classifications are to be used which combine both safety & environmental severity.编写安全计划进行潜在危险评估时,应使用下面的安全环境等级。Class of Risk Severity危险级Description说明Class 1一级P1Potential Death, Permanent Disability or Major Structural Damage or Potential incident resulting in permanent or significant detrimental impact on environmental elements (natural or built).潜在的死亡,永久失能,或主要结构损伤,或潜在的事故导致对环境(自然的或建造的)永久性或重大损害性的影响。Class 2二级P2Potential Temporary Disability or Minor Structural Damage orPotential incident impacting on environmental elements (natural or built) that can be contained and remediate to acceptable conditions with no long-term effect. Any exceed of statutory condition.潜在的临时性失能,次要的结构损伤或潜在事故会造成环境(自然的或建造的)影响,但能控制并将其改进至可被接受的状态,且无长期的影响,任何超出法规条款的。Class 3三级P3Potential incident, which has the potential to cause minor damage to the environment or persons that would require first aid.可能对环境产生小的危害或对人员产生伤害而需急救的潜在事故。Class 1一级A1Actual Death or Permanent Disability orActual incident resulting in permanent or significant detrimental impact on environmental elements (natural or built). To Ops Manager ASAP实际的死亡,永久失能,或实际事故导致的对环境(自然的或建造的)永久性或重大损害性的影响。立即报告给运营经理。Class 2二级A2AActual Temporary Disability which may convert to Class 1 orActual incident impacting on environmental elements (natural or built) that can be contained and remediate to acceptable conditions with no long-term effect. Any breach of statutory condition. To Ops Manager ASAP可能转为一级的实际的暂时失能或实际事故会造成环境因素(自然的或建造的)影响,但能控制并将其改进至可被接受的状态,且无长期的影响。任何违反法规条款的,立即报告给运营经理。Class 2二级A2BActual Temporary Disability Resulting in Lost Time. To Ops Manager within 24 hours实际的暂时失能导致“损失工时”。在24小时内报告给运营经理。Class 3三级A3Actual incident which has caused minor injury or damage to the environment对环境造成微小的伤害和破坏的实际事故。4. Mandatory Standards 强制性标准The contractor shall comply in all respects with all Employer EH&S Standards and with any other such standards produced during the course of the contract. These Standards set minimum parameters for EH&S workplace practices and is in addition to all regulatory requirements (national and local). Standards that are applicable to Employer projects, and which are available from Employer are as follows:承包商必须遵守国家和地方的法律法规及规范要求以及雇主在安全、健康和环境相关规章制度。这些标准包括但不仅限于如下条目,业主的相关要求由雇主提供:Aerial Lift吊篮Chemical and Gas Delivery, Storage, & Handling 化学物品及气体的运输、储存及使用Chemical Pipe Work化学管道工作Clean room Construction Safety洁净厂房施工安全Confined Space Entry密闭空间的进入Crane起重机Demolition works拆除工作Electrical Work电工作业Excavation开挖Fall Protection坠落保护措施Hot Work动火作业Ladder Safety爬梯安全Lock Out / Tag Out挂锁/挂牌Material Lifting Device材料升降机Noise & Hearing Protection噪音防护Overhead Work交叉作业Penetration protection孔洞保护Power Tools电动工具Personal Protective Equipment个人保护装置Radioactive Energy放射能量Respirator Protection防毒面具Scaffolds脚手架Site Security工地保安Steel Erection钢结构安装Vibration Control防震控制5. Work Permit Applications 工作许可证申请Before commencement of the work activities on site the Contractor shall apply for a Permit to Work (PTW) Employer. 在开始施工前,承包商需从雇主申请工作许可证。At least 24 hours before start of activities with an increased level of risk the Contractor shall apply for the applicable work permits from Employer. The following work permits can be applied for:如需进行危险性的工作,承包商至少提前24小时向雇主申请工作许可证。以下工作须申请工作许可证:Permit to Excavate (PTE)开挖作业许可证Permit to Work on Temporary Site Electrics (PTWTSE)现场临时用电作业许可证Permit to Work on High Voltage Distribution (PTWHV)高压配电箱作业许可证Hot Work Permit (HWP)动火证Permit to Work in Confined Spaces (PTWCS)密闭空间的作业许可证 Scaffold Permit 脚手架许可证Demolition Permit拆除许可证Crane Usage Permit吊车使用许可证Material Move Permit材料进出许可证Radioactive Use Permit放射性物质使用许可证As part of the application procedure for the permits mentioned above the Contractor shall submit a list with the work activities, the locations, the tools, all potential hazards associated with the work and the protective measures and provisions.作为上述作业许可证申请的一部分,承包商应提交所要进行的工作清单、作业地点、使用工具、所有与工作相关潜在的危险以及所采取的保护措施。6. Notifiable Work 须申报的工作The contractor is solely responsible for notifying the Statutory Authority of any modifiable work and nominating a Safety and Environmental Supervisor for that work prior to confirmation of any prepared procedures with the Authorities. The contractor shall make the Employer Supervisor aware of all implications and the proposed course of action.如有申报工作,承包商可自行与有关政府部门联系,在所有的程序得到政府部门确认之前要求承包商指定一个环境安全专员负责所有的准备程序。承包商必须让雇主知道所有的的程序和采取的行动。7. Safe Work Method Statements 安全施工方案For the safe execution of particular activities the contractor shall develop Safe Work Method Statements. These should meet the standard required by EMPLOYER. Guidelines are available on suitable formats.为了安全地进行一些特殊工作,要求承包商制定“安全施工方案”,并要符合雇主的标准。 雇主有该方案的格式。Safe Work Method Statements should contain management signature, description of work, sequence of work, associated hazards and required qualifications of employees and trainers. An inspection checklist will be developed based on the SWMS and used as required.安全施工方案应包括承包商管理部门的签字,内容包括工作说明、工作程序、安全隐患及相应员工、管理人员的资格证书。根据需要列出清单逐一核对。8. Review by Employer雇主审核The contractor shall assist Employer to review their Safety Plan and make any agreed alterations. An Employer Sample Contractor Competence Review Checklist is available.承包商应协助雇主一起对其安全计划进行审核并作相应的修改,并保留相关审核和修改的记录。Worker and Company Registration 工人和公司登记承包商员工需提供承包商进行的三级安全教育合格证明以及本人身份证复印件。If the above dont happen there will be fines.若不进行上述工作,将对承包商予以处罚。9. Induction 培训Before work commences all workers must attend a general site induction and records of attendance must be kept. In addition all workers must undergo induction by the contractor in the safety precautions of the particular job and the requirements of the safe work method statement. 在开始施工前,要求所有承包商员工参加承包商组织的安全培训并保存其培训记录。另外如需进行一些特殊工作,根据特殊工作的安全预防要求及安全施工方案的规定,必须先经过培训Once a worker has been inducted, he or she will be issued an ID badge. Each ID pass remains the property of the Employer. RMB10 shall be paid as deposit for badge. Loss shall be reported immediately. When the work is completed, all ID badges shall be returned. If an ID badge is lost or damaged, the deposit is lost.经过安全培训的工人会发给胸卡. 每张胸卡需交50元人民币(其含成本费30元、押金20元),如果发生遗失 现象应立即汇报。每张胸卡用完后由业主保留。在工程结束后,所有的 胸卡应该归还。如果任何一张胸卡遗失或者损坏,押金将不予归还。10. Basic Site Safety Rules 基本现场安全规则Contractors should note the following abridged site safety rules on an Employer Project. As well as compliance with the Country specific, all contractors employees and visitors on the site are required to observe the following basic safety rules:承包商在雇主项目中应遵守以下的安全规则,同时要求遵守国家的安全守则的安全规定。所有在工地上的承包商员工及访客者均被要求遵守以下的基本条例:10.1 Before commencing work, contractors must submit evidence of competence and Safe Work Method Statements. All contractor employees (whether direct or contracted) are to comply with the established Safe Work Methods.在施工之前,承包商应提交安全施工方案。所有承包商员工(不管是其员工还是其下包员工)均要求遵守此方案。10.2All project personnel must wear approved personal protective equipment as directed. Further, such equipment must be kept in good condition and replaced when necessary.施工人员应穿戴合格的个人防护用具,而且这些个人防护用具须被妥善保管,并在必要时候更换。10.3 Adequate fire protection shall be provided as necessary. In particular a suitable fire extinguisher shall be securely attached to each electric or oxy-acetylene or oxy-LPG welding plant brought on site.必须充分做好防火工作。尤其是现场的用电设备、氧炔焊接、气割、电焊等动火作业的作业地点必须配备合适的灭火器材。10.4 Explosives shall not be used without the express permission of Employers site manager.未经雇主现场经理批准,禁止使用爆炸物。10.5 Proper care shall be taken in the use of compressed air.使用压缩空气时必须特别小心。10.6 Only certified or qualified equipment and tools are allowed on site. Only the right tools and equipment for the job must be used, used safely and replaced when faulty.现场只允许使用被鉴定或者合格的机具,且要使用正确的工具及设备。一经发现问题应立即更换。10.7 All permit-required activities shall have appropriate permit before activities start.需办理作业许可的施工必须在作业前办理完毕,否侧禁止施工。10.8 All electrical installations must be protected by an earth leakage system. All tools and equipment must be fitted with IP44 plugs and sockets. No direct wires are to be used at any time.所有电工操作需有接地保护系统。所有工具及设备的插座必须有防浸入保护,禁止使用裸线直接插在插孔上。10.9 Avoid distracting others. No throwing of objects from height.严禁从高空向下抛物。10.10 Report immediately any condition or practice you think might cause injury to employees or damage to equipment.如发现有导致人员受伤或设备事故的隐患,应立即报告。10.11 No alcohol is permitted inside the boundary of the site.工地上禁止饮酒。10.12 Smoking is allowed only in designated areas.只允许在吸烟区吸烟。10.13 Do not interfere with fire fighting equipment.勿乱动消防设备。10.14 All visitors to the site are required to report to the site office.到访人员需经过雇主批准后方可进入。10.15 Only proper access is to be used to access areas of the site. No short-cuts are allowed.进入工地走专门的通道。不许走捷径。10.16 Use toilet facilities provided.使用已有的卫生设施,禁止随地大小便。10.17 Good housekeeping to be practised all the time.保持现场的干净整洁。10.18 Observe all the safety and warning signs and obey traffic laws whilst driving.注意各种安全警示标记,遵守交通规则以及厂区限速20公里/小时的要求。10.19 When lifting, bend your knees, grasp the load firmly, then raise the load keeping your back as straight as possible. Get help for heavy loads.在背负重物时,须双膝弯曲,紧紧抓住重物,然后慢慢站起并尽量挺直后背。如系重物,须多人同时合作。10.20 In case of emergency, follow the instructions from the Emergency Response Team.如遇紧急情况,请听从紧急应变小组的指挥。10.21 Whenever you or the equipment you are operating is involved in an accident, regardless of how minor, report it immediately.任何时候无论自身或者操作的设备发生事故,不论其轻重,须立即报告。10.22 When in doubt ask, dont take chances.若有疑问须确认,不允许冒险行动。10.23 Think safe living habits both on and off the job.不管是在工地上还是在工地外,都要养成安全的生活习惯。11.
