代办签证中介服务协议书甲方： 证件号码： 详细地址：_ 电话： 乙方：浙江长诚出国服务有限公司 地址：浙江省青田县鹤城中路26号 邮编：323900电话6831233 6831728第一条：签证服务内容1、 甲方提供材料，包括：护照，身份证，户口簿等材料。2、 乙方为甲方代办_（国家名）签证服务，该签证真实性，符合国际标准，并符合国际出入境相关条文条规。乙方为甲方代办个人国际旅行医疗保险（保额不低于三万欧元）。乙方为甲方订购往返航程机票。3、 甲方需支付代办签证中介服务费用_元/人（注：该费用不包括甲方在办理公证、认证以及领馆签证费、翻译产生的费用），及前往目的地国家保证金_万元人民币/欧元/人，个人国际旅行医疗保险费用 元/人（以保单实际发生额的保费计算）。甲方支付的所有费用，乙方必须开具相应的收款收据为凭（如甲方要求开具正式发票，则须另外支付发票的所有税收金额）。第二条：双方特别约定1、 甲方提交签证所需资料并经审核，乙方愿意为甲方受理所有的资料。材料送/寄往签证处后，甲方因个人原因中途不履行合同，乙方有权不退还甲方所支付的代办签证中介服务费。所交保证金可退还。2、 签证获准后，甲方因个人原因等诸多问题不想前往签证国，其后果由甲方自行承担。乙方将不退还甲方所支付的中介服务费及其他费用。3、 乙方杜绝甲方以代办签证渠道，进行偷私渡等违法行为。4、 甲方在签证期内，按时归国，乙方将保证金全额退还给甲方。但不计利息。5、 甲方自愿参加进行按时返回的联保制。6、 甲方提供为其担保按时返回的在职公务员作为担保人。担保人应为被担保人担保在签证期内回国，如被担保人没有按时返回国内的，一切法律责任由担保人承担联带责任。第三条：双方履行责任甲方：1、保证护照，身份证，户口簿和其它材料等真实有效。因虚假材料造成后果由客人自负，乙方有权扣留虚假材料，并向有关部门通报。2、遵守中华人民共和国出入境相关法律条文条规，遵守各个目的地国家的法律法规，并遵守前往_（目的地国家）注意事项。3、即时缴纳代办签证费用，积极履行本合同的执行。乙方：1、严格遵守中华人民共和国出入境签证相关法律条文条规。2、乙方为甲方代办真实有效的前往目的地国家签证。第四条：双方违约责任如甲方违约，在签证期内不回国，乙方有权中止合同中的相应条款继续履行，对担保人进行起诉应负的法律责任，并对该事项向中国有关部门和国外有关部门进行报案。甲方所交的保证金不予退还，任由乙方全权处理。甲方产生的不良影响由甲方自行承担。如乙方违约，甲方有权要求不支付乙方所造成的经济损失，并退还保证金和已交的中介服务费。乙方不承担除甲方已交中介服务费以外的其它任何费用。 第五条：其他双方协定的补充条款 1、甲方须在回国后五日内由本人到达西班牙驻沪总领事馆报到销签后，由本人向乙方提供报到销签后的护照，经乙方向领事馆进行确认（确认所需时间三个工作日），到乙方退还原已交回国保证金。没有在回国后五日内到达领事馆报到销签的，没经乙方进行确认的，其原已交回国保证金乙方有权不退。 2、甲方自身原因造成出入境不成功的，其责任全由甲方自己负责。 3、乙方仅限于对签证的合法性和有效性负责。不负其他任何责任。第六条：不可抗力如遇到目的地国家暴乱或者国家政策改动等不可抗力因素，导致甲方无法入境，导致乙方在工作日内，无法准时代办签证完毕的同时，乙方应尽快通知甲方，并为甲方代办签证相应的事后处理。甲乙双方采取一切措施，以恢复本合同的履行。如因不可抗力所造成的甲方的损失，乙方不承担。第七条：法律适用本合同适用中华人民共和国法律法规，符合中华人民共和国合同法。合同中执行如有异议，双方协商解决或向乙方所在地仲裁机构或法院仲裁。备注：本服务协议一式二份，甲、乙双方各执一份为凭。本协议自甲、乙双方签字之日起即生效。甲方盖章：甲方（签名）：日期：乙方：浙江长诚出国服务有限公司乙方（签名）： 日期：Visa service Agent Agreement Party A： Certificate No.：_Address:_ Tel：_Party B：Zhejiang Changcheng Go Abroad Service Co., Ltd. Address：No.26 Hecheng Road, Qingtian County, Zhejiang Province.Postal Code：323900Tel6831233 6831728One：Content of Visa Service1、Party A offers the documents such as Passport, ID card, Household register and other documents needed.2、Party B is the agency of Party A to supply the Visa of _(Country), which should conform to the international standards, and also International entry and exit rules. Party B is the agency of Party A to buy the international travel insurance (Coverage not lower than thirty thousand EUR). Party B is also the agency of Party A to buy the round trip air ticket.3、Party A need to pay the agent service fee _RMB/ Person ( Mark: This fee hasnt included the notarization, authentication, documents translation charge and consulate visa fee), and the security deposit for going abroad _ RMB/ EUR/Person, personal international travel insurance cost _RMB /Person(Insurance cost based to the exactly amount in the Insurance Policy). Party B should issue the receipt for all the cost paid by Party A ( Extra tax fee should paid by Party A if the formal invoice is requested)Two：Special agreement between two Parties1、Party A offers the documents needed by supplying Visa, Party B is willing to accept and collate all the documents. If Party A breach the contract because of personal reason after Party B send those documents to visa office, the agent service fee is non refundable, and the security deposit can be return back. 2、If Party A doesnt want to the visa country due to some personal reasons after getting the visa, he/ she should bear the liability on his/ her own. The agent service fee and other cost needed in supplying visa will not return to Party A. 3、Party A is not allowed to use the agent visa service to do smuggling or other illegal act.4、Party B should return the full security deposit back to Party A when Party A back to China within the visa period, and free of interest.5、Party A is willing to participate in the joint guaranty system for back to China within visa period. 6、Party A should find a civil servants to be the guarantor who guarantee Party A will back to china on time, if Party A hasnt back to China on time, the guarantor should bear the relative legal responsibility.Three：The responsibility of two PartiesParty A: 1、Guarantee the documents supplied such as Passport, ID card, Household register are real and effective. Party A should bear all the liability causing by supplying false information, Party B has the right to inform the relevant departments and detain the false material.2、Obey the relative rules of entry and exit of Peoples republic of China, obey the laws and regulations of destination country, and also obey the _(Visa country) announcements.3、Pay the agent visa service fee on time, and act according to the contract.Party B: 1、Strictly adhere the relative rules of entry and exit visa of Peoples republic of China.2、Party B should offer Party A the real and effective Visa of destination country. Four：The Liability for Breach of two PartiesIf Party A breach the contract by not back to China within the visa period, Party B has the right to stop performing the relative contract, and indict the legal responsibility of guarantor, and report to relative department of China and destination country. All the security deposit will be not refundable and freely disposed by Party B. Party A should bear all the harmful effect.If Party B breach the contract, Party A has the right to ask a refund of security deposit and other agent fee, and not to pay any loss of Party B. Party B will not carry any other charge except the agent fee paid by Party A.Five：Supplemental Terms of two Parties 1、Party A himself/ herself should go to the consulate of Spain in Shanghai to cancel the visa within 5 days after back to China. Then Party A shows the passport which has the stamps of canceling visa to Party B, after confirm with consulate ( About 3 working days), Party B should return the security deposit to Party A. Party B has the right to retain the security deposit only if Party A hasnt gone to consulate and cancel the visa within 5 days after back to China, and hasnt shown to Party B. 2、Party A should bear all the responsibility if he/ she is not allowed to go abroad by the personal reason.3、Party B only undertake responsibility for the legality and effectiveness of visa, and is not liable for other responsibilitySix：Force majeureIf Party A can not entry the destination country or Party B can not supply the visa on time due to the Force majeure such as riot or policy changed, Party B should inform Party A as soon as possible, and handle the
