2、ovide guidance on the site Permit to Worksystem.本程序的目的在于就现场作业的许可证制度提出指导性意见。1.2 The site handles a large diversity of work, so the potential for serious accidents is high. To prevent them it is vital that there should be safe systems of work. They will often be applied and enforced by means of a form

3、al Permit to Work system.施工现场充满了高强度的作业，因此发生重大事故的可能性也比较高。为了防 患于未然，必须制定一套安全作业系统。其中用得比较多的是一种正式制度是 作业许可证制度。1.3 All Critical Operations will be under the control of a Permit to Work systemoperated by the Site.Specific training will be given to all employees on thePTW system .All permit requests must be m

4、ade at least 24 hours prior to work commencing.所有关键作业行为均必须处于现场作业许可证制度的控制之下。全体员工均必 须接受有关作业许可证制度的具体培训。各种作业许可证都必须在作业开始之 前至少提前二十四小时提出申请。1.4 Routine Operationswill not be covered by Permit to Work.常规作业不需要办理作业许可证。2. 5、ations include hot works in vicinity of thearea that have inflammable material storage, enter into vessels/equipment or any confine space work, temporary electrical work electrical work, excavations deep than 300mm; use of radioactive, spray painting and grit blasting ,use of cartridge tools ,use of

6、 man baskets, erection or removal of flooring and handrails on open steelwork, diving operations and explosive material,;pressure testing, lifting operations over 50 tons and multi-crane lifting work and any other operations which might be identified during the progress of the project that need to b

8、ig nated for operati ons willnot require Permit to Work even though critical operations are carried out.For example a workshop or grit blasting area where hot work is considered“normal operation ” will not require Permit to Work. However these activities must be covered under other SSPs or special p

10、3.1 The Safety Manager is responsible for ensuring that this procedure is implemented. He is also responsible for the checking of the effectiveness of the Permit to Work system by carrying out random audits at the work site.安全经理负责该程序的实施。其另一个任务是通过现场的随机审核，负责检查 并评估作业许可证制度的落实情况。3.2 The Safety engineer i

13、 the Construction Managerinformed of any potentially hazardoussituations arising from permit work, advise theConstruction Manager ofany activities that need to be bought under the control of the PTW system and to ensure all staff involved have an overview of significant ongoing work 为了保证施工经理及时掌握由于作业

15、r PTW at the location, and generate a sense of ownership in those individuals who work with the system. 担当本区域作业许可证制度的推行重任，培养有关员工的主人翁意识3.4.5 Monitor all aspects of the PTW procedure. 监督实施作业许可证制度的各方面工作。3.4.6 Identify any changes required to the PTW procedure to allow for continuous improvement.适时地对作业许

PROCEDURE程序4.1 General 概述A Permit to Work system is a formal written system used to con

17、trol certain types of work, which are identified as potentially hazardous.Essentialfeatures of a Permit to Work system are:作业许可证制度是一种对某些类型的作业进行控制的正式书面制度。 这类作业 一般是指那些具有潜在危险的活动。作业许可证制度的基本特征为：a. Clear identification of who may authorize particular jobs, 明确地规定谁有权批准某些特殊的作业，b. Training and instruction in

18、the issue and use of permits. 就作业许可证的颁发和使用进行必要的培训和指导。c. Monitoring and auditing to ensure that the system works as intended. 对作业许可证的使用进行监督和审核，确保制度的实施满足预期的目标 .d. A Permit to work system is applied to protect personnel and plant. It consists of an organized and predefined HSE procedure forming a clear

19、 record of all foreseeable hazards that have been considered in advance. 21、g and equipment is provided and used提供和使用正确的防护服和设备-Appropriate management are aware of the work in progress管理层知道工作正在进行中4.2 Permit Display 许可证上的显示内容4.2.0 Copies of a Permit to Work should be clearly displayed: 应该在下列场合清楚地显示作业许可证的内容：a) At the work site. 在作业场所。b) In the permit co-ordination room, with a

22、dditional copies at the worksite. 在许可证办公室，留有作业现场许可证的副本。4.3 Permit Suspension 许可暂时中止4.3.1 If for any reason work controlled by a permit has to be suspended then the permit will immediately be returned to the Superintendent who will acknowledge the fact and co-ordinate the return of all permits to the

23、 Permit Office.如果由于各种原因，原来受许可证控制的某项作业不得不暂时停止 , 则应该 将许可证立即返还所在部门的主管，再由主管向许可证办公室通报真相，并 将各种许可证退还给许可证办公室。4.3.2 Example of reasons for suspension are: 许可证暂时收回执行的理由举例如下：a) Site general alarm. 现场出现一般的报警。b) Operational reasons. 操作原因。c) Conflict of work activities. 作业发生冲突。d) Expiry of period for which permit

24、 is valid.作业许可证有效期到期。e) Change in work site conditions. 作业现场的情况发生变化4.3.3 After suspension it will be necessary, once permission has been given to recommence work, to have the permit re-validated before the work can restart. 在许可证暂时收回以后，一旦允许重新开始作业，则在重新开始作业之前， 必须对原颁发的许可证重新核准。4.4 Cross Reference 交叉确认4.4

25、.1 It is important to make sure that one activity under a Permit to Work does not create danger for another ongoing activity.在根据作业许可证进行某项作业时， 必须确保不对其它正在进行中的工作构 成威胁。这一点是非常重要的。4.4.2 The safety engineer should be aware of potential interaction and should check when a permit is prepared whether it impin

26、ges on one already issued.安全工程师应该清楚地意识到各种作业之间可能存在的关联关系， 在准备签 发许可证时，应该仔细确认它是否与此前颁发的许可作业之间存在着冲突。4.5 Permit Revalidation 许可证的重新核准4.5.1 When a permit is authorized by safety engineer , it will have a time expiry placed on it. This may vary form a few hours to a full shift or even several days, depending

27、 upon the nature of the work being undertaken and the frequency at which the activity is repeated. 安全工程师在签发许可证时，应该注明许可证的有效时间。有效期有可能是 几个小时，也有可能是整个班次甚至是几天，具体要由作业的性质以及作业 重复的频率决定。4.5.2 If, at the end of this time period, the work being controlled by the permit is not complete the permit may be revalidat

28、ed by the responsible person, who shall ensure, together with the safety engineer, that the original conditions required by the permit are still being met.如果在有效期到期时，许可证规定的作业没有如期完成，则应该由相关的负 责人对许可证进行重新核准。负责核准的人员在核准之前，必须与安全工程 师一起，确认许可证原对应的作业条件是否已经发生变化。4.6 Hand Back 退还4.6.1 Prior to the permit being sig

30、dged the return of the permit?有关的作业负责人是否知道许可证已经退还？4.7 Responsibilities under Permit to Work System 作业许可证制度所规定的责 任4.7.1 Duties are placed on various persons with regard to the operation of aPermit to Work system.In general these are as follows:在执行作业许可证制度过程中， 不同的人承担着不同的责任。 总的分工如下：a) The Contractor - w

31、ho must ensure that all persons within his employment have a clear understanding of how the PTW system operates. In the case of Contractors this is normally undertaken in conjunction with the Superintendent, 承包商 - 负责保证他所雇佣的所有员工都能正确地理解作业许可证制度 的运行方式。对承包商而言，他可能更多的是与现场主管合作执行这一 制度。b) Each project should

34、守相关许可证上规定的作业条件和作业要求，不 从事任何不理解的或超出许可证许可范围的作业。4.8 Training Required 培训要求4.8.1 Training on Permit to Work systems is necessary for all persons who may be involved. This should include managers, supervisors, safety engineers of site HSE , approver of Permit and contractors. 对于所有涉及的人员，都必须进行有关作业许可证制度方面的培训

35、。这些人 员包括各级经理、主管、安全工程师、许可证批准人和承包商。4.8.2 Responsible people and personnel responsible for carrying out any isolation should be formally assessed before appointment. 在指定各级负责人和负责实施隔离的人员前，要进行正式的评估。4.8.3 Before formal appointment by the Construction Manager responsible people should be assessed on: 在施工经理进
