时间就是金钱，效率就是生命！有机产品认证文件资料清单（植物生产和初加工）Document Check List for Organic Product Certificate (for plant production and its initial processing)申请单位应提供以下文件资料并按序号编排、装订后提交：Application unit must arrange files according to the following sequence number, submit after binding.第一部分项目基本情况资料 Section 1: General Information of Programme1. 营业执照副本复印件（需加盖最新年检章）和组织机构代码证复印件Copy of business license with annual checked seal and the copy of organization code material;2. 土地使用权证明及合同 Land use documents and contracts要求：Requests（1）土地承租合同书（应体现范围、面积、详细地址、合同有效期限）；Land ownership document or land rent ontract with scale, detailed address and duration of contrat;（2）申请单位与生产者签订的有机种植合同书（若申请单位不是直接生产者，应体现范围、面积、详细地址、合同有效期限及有机生产的相关要求）；Organic planting contract between application unit and production person. It is necessary to offer scale, detail address, and the relative requirements for organic production in case of application unit is not production person.（3）新开垦的土地必须出具县级以上政府部门开具的开发批复。If the land is new-reclamation, approving documents from governmental department is necessary3. 农户清单（适用于公司+农户、公司+基地+农户、公司+合作社的组织模式）Farmer list which is only required for the organization model of company+farmer, company+ base+farmer, company+co-operative with the requirements below:要求：（1）农户姓名、身份证号、地块编号、种植品种、面积等；Farmer names, identity cards, land numbers, planting kinds and areas etc;（2）公司对基地和（或）农户的管理制度（应体现有机生产的相关要求和对农户有效的管理）。Management regulations of company for base and the effective management of the farmesr should be reflected the related requirements for organic production.4. 产地（基地）区域位置图绘制 Administrational map of production place or base with the requirements below:要求：（1）地块分布图（多地块和分布分散情况下，应提供全部地块分布情况的地图）；Land distribution figure or all of them in case of more land and in disperse（2）地块图（每个地块的形状、面积、边界、缓冲带；产地周围临近地块情况；Land figures with every land form, area, boundary and buffer zone, land situation round it;（3）生产基地内主要建筑、树林、溪流等标示物以及河流、水井和其他水源的位置）。Mark the locations of main building, trees, headwater etc. in base and positions of rivers, wells and other water sources.5. 本年度产地环境质量监（检）测报告Environment reports for production place this year with the requirements below:要求：（1）环境监（检）测机构应具有相关资质；The related certificate of testing organization;（2）土壤和水的检测报告委托方应为申请单位。Soil and water reports for the application unit.（3）大气可提供检测报告或当地县级（农业）环保部门出具的证明。The test report of atmosphere or the certificate issued by local county (agricultural) environmental protection department need to be provided.6. 产品初加工（如适用）Primary process of product with the requirements below:要求：（1）加工厂区平面图（应标明加工厂周边环境、加工、包装车间、仓库及相关设备的分布）；Processing area floor plan(need to indicate surrounding land situation, processing workshop, packing workshop, storehouse and their equipments;（2）QS证书复印件（如适用）；The copy of sanitary license (if applicable)（3）本年度加工用水的检测报告（依据GB 5749）；Processing water report this year according to GB 5749;（4）如为委托加工，提供委托加工合同。Provide the processing contract in case of consigned processing.7. 有机产品生产、加工规划（包括对生产、加工环境适宜性的评价，对生产方式、加工工艺和流程的说明及证明材料，农药、肥料、食品添加剂等投入物质的管理制度以及质量保证、标识与追溯体系建立、有机生产加工风险控制措施等）Plans for organic processing and production (including the suitability evaluation of the production and processing environment, instructions and documents of processing technology and processing line, the management system of pesticides, fertilizers, food additives and quality assurance, identification and traceability system, risk control measures of organic production and processing, etc); 8. 有机转换计划（适用于多年生作物存在平行生产的情况）Organic conversion plan in case of parallel production of perennial plants;9. 通过其他认证机构的认证（如适用）Certification by the other certification body (if applicable):要求：（1）其他认证机构认证证书和检查报告复印件；The copy of certificate and the inspection reports;（2）该认证机构提出的不符合项的整改材料；The corrective materials of nonconformities suggested by other certification body;（3）目前在该认证机构的认证情况。More information about the certifications from other certification body.第二部分 质量管理体系文件 Section 2 : Quality Management System Documents1. 质量管理手册 Quality Management Manual要求：请参考GB/T19630.4有机产品 第4部分：管理体系4.2.4编制Request: please refer to the code of 4.2.4 management system in the fourth part of GB/T19630.4 Organic product.2. 作物生产、加工操作规程（请参考GB/T19630.1有机产品第1部分：植物生产和第4部分的4.2.5编制）Procedures of processing and operating (please refer to the code of 4.2.5 processing and the fourth part in the first part of GB/T19630.1 Organic product .要求：requests（1）作物种植生产技术规程 Processing technology procedure of crops planting process至少包括：a.种子和种苗的选定、处理方法以及播种育苗的规程；b.获得有机种子和种苗的计划；c.土壤肥力的保持与管理措施（若为自制肥料，提供原料的来源、比例、名称、自制方法）；d.常发病、虫、草害的名称与防治措施（若为自制生物农药，提供原料的来源、比例、名称、自制方法）；e.灌溉水的来源及灌溉管理；f.作物的轮作计划和间、套作计划及轮作、间套作作物的种植规程（一年生作物轮作计划至少包含三种作物）；At least Including:(a) procedure for choosing and handling methods for seed and nursery;(b) plan to get organic seed and nursery;(c) soil fertility preservation and management measurements; It is necessary to offer raw material sources, proportion, names and self-making method in case of self-making fertilizer;(d) names of common disease, insect and grass hazards and their prevention measurements; It is necessary to offer raw material sources, rates, names and self-making method in case of self-making pesticide;(e) irrigation water sources and managements;(f) plans and procedure for plant rotation and interplanting; No less than three plants are required for rotation plan on annual plant;（2）防止有机生产、加工和经营过程中受禁用物质污染所采取的预防措施；The measures to prevent the organic producing, processing and managing from polluting by the banned substances ;（3）植物产品收获规程及收获、采集后运输、临时保管规程；Procedure for plant product harvest and procedure for transportation and temporary storage after harvest and collection;（4）产品初加工（详细描述各加工工序的操作方法）、包装和储藏规程；Procedure for primary process covering detailed operation of every steps, packing and storage;（5）加工厂卫生管理与有害生物控制规程；Control procedures of processing health management and pest;（6）运输工具、机械设备及仓储设施的维护、清洁规程；Maintenance and cleaning procedures of transportation, mechanical equipment and storage facilities ;（7）防止有机产品与非有机产品混杂所采取的措施（存在平行生产的企业须提交）；Measures of preventing cross contamination between organic and conventional products (only for parallel production);（8）标签及生产批号的管理规程；Management discipline of label and the production batch number;（9）员工福利与劳动保护规程。Procedure of employee welfare and labor protection.第三部分记录文件和其他相关资料（记录应清晰准确，应由记录人亲笔签字确认。记录应至少保留5年） Section 3: Record file and other related information (Records should be clear and accurate. It should be signed and verified by the recorder and be retained for at least 5 years.)1. 购买种子或种苗的证明文件（种类、来源、数量、购买单据、非转基因证明、常规种子或种苗未经禁用物质处理的证明、种子包装袋复印件等信息）；Certificate for purchased seed and nursery, which covers kinds, sources, amount, invoices, certificate for non-GM,proof of seed or nursery without treatment by any prohibited materials, and copy of seed package etc.;2. 肥料生产过程记录（若自制肥料）；Record of fertilizer production, which is only for self-making fertilizer;3. 农事活动记录（土地耕作、播种、施肥、病虫草害防治、灌溉、重大事件）；Records of farming activities(land cultivation, planting, fertilization, diseases, pests and weeds control, irrigation, a major event, harvest)4. 所有生产投入品的台帐记录（来源、购买数量、使用去向与数量、库存数量等）及购买单据；Records of all inputs covering sources, purchase amount, used places and amount, and storage amount as well as invoices etc.;5. 植物收获记录，包括品种、数量、收获日期、收获方式、生产批号等；Harvest records including kinds, amount, harvest date, harvest model and product numbers etc.;6. 加工记录（包括原料购买、入库、加工过程、包装、标识、储藏、出库、运输记录等）； Processing records (including raw material purchase, s
