Abstract:

Chronicobstructivepulmonarydisease(COPD)isaprogressiveanddebilitatingrespiratorydiseasethatrequirescontinuousmanagementandtreatment.However,thehighprevalenceofreadmissionandmortalityratesamongCOPDpatientshighlightstheneedformoreeffectivehealthcareinterventions.Onesuchinterventionistheuseofextendedorcontinuityofcare,whichaimstoprovideongoingmanagementandsupporttoCOPDpatients.ThispaperreviewsthecurrentliteratureontheclinicalvalueofextendedorcontinuityofcareinCOPDpatientsandpresentsevidenceforitseffectivenessinreducinghospitalizationrates,improvingqualityoflife,andenhancingpatientoutcomes.

Introduction:

COPDisachronicrespiratorydiseasecharacterizedbyairflowrestriction,chronicbronchitis,andemphysema.Itisaleadingcauseofmorbidityandmortalityworldwide,withanestimated3milliondeathsperyear.COPDisassociatedwithsignificantdisability,reducedqualityoflife,andincreasedhealthcareutilization,includinghospitalization,emergencydepartmentvisits,andreadmissionrates.ThemanagementofCOPDrequiresacomprehensiveandmultidisciplinaryapproachthatinvolvespharmacologicalandnon-pharmacologicalinterventions,suchassmokingcessation,pulmonaryrehabilitation,andoxygentherapy.However,despitetheseinterventions,manyCOPDpatientscontinuetoexperienceworseningsymptomsandfrequenthospitalizations.

ExtendedorcontinuityofcareisahealthcareinterventionthatprovidesongoingmanagementandsupporttoCOPDpatientsoutsideofthehospitalsetting.Thisinterventioninvolvesarangeofservices,includingeducation,self-managementstrategies,regularfollow-upappointments,andaccesstospecializedcare.Theaimofthisinterventionistoimprovepatientoutcomes,reducehospitalizationrates,andenhancequalityoflife.Inthispaper,wereviewthecurrentliteratureontheclinicalvalueofextendedorcontinuityofcareinCOPDpatientsandpresentevidenceforitseffectiveness.

Asystematicreviewandmeta-analysisof23randomizedcontrolledtrials(RCTs)involving4,659COPDpatientsfoundthatextendedcareinterventionsreducedhospitalizationsby23%(RR0.77;95%CI0.67-0.87)andall-causemortalityby22%(RR0.78;95%CI0.62-0.97)comparedtousualcare.Thisstudyalsofoundthatextendedcareinterventionsimprovedqualityoflifeandreducedhealthcarecosts.TheauthorsconcludedthatextendedcareinterventionsareaneffectiveapproachtomanagingCOPDpatientsandshouldbeimplementedinclinicalpractice.

Discussion:

However,despitethebenefitsofextendedcareinterventions,theirimplementationinclinicalpracticeremainslimited.Barrierstoimplementationincludealackofresources,afragmentedhealthcaresystem,andalackofawarenessandunderstandingofthevalueofextendedcareinterventionsamonghealthcareprovidersandpatients.

Conclusion:

ExtendedorcontinuityofcareisavaluablehealthcareinterventionformanagingCOPDpatients.Theevidencereviewedinthispapersupportstheuseofextendedcareinterventionsinclinicalpracticetoimprovepatientoutcomes,reducehospitalizationrates,andenhancequalityoflife.However,thebarrierstoimplementationofextendedcare
